Large-Scale Whole-Genome Sequencing of Three Diverse Asian Populations in Singapore.

Underrepresentation of Asian genomes has hindered population and medical genetics research on Asians, leading to population disparities in precision medicine. By whole-genome sequencing of 4,810 Singapore Chinese, Malays, and Indians, we found 98.3 million SNPs and small insertions or deletions, over half of which are novel. Population structure analysis demonstrated great representation of Asian genetic diversity by three ethnicities in Singapore and revealed a Malay-related novel ancestry component. Furthermore, demographic inference suggested that Malays split from Chinese ∼24,800 years ago

The introduction of genetic counseling in Ethiopia: Results of a training workshop and lessons learned.

BACKGROUND: Over the past two decades non-communicable diseases (NCDs) have steadily increased as a cause of worldwide disability and mortality with a concomitant decrease in disease burden from communicable, maternal, neonatal and nutritional conditions. Congenital anomalies, the most common NCD affecting children, have recently become the fifth leading cause of under-five mortality worldwide, ahead of other conditions such as malaria, neonatal sepsis and malnutrition. Genetic counseling has been shown to be an effective method to decrease the impact of congenital anomalies and genetic condit

[Psychiatry and genetics: A bond to face uncertainty].

This article complements a previous one, in which theoretical concepts about etiologic categories, suspicion criteria and timely referral to the specialist were explained. This time we will focus on genetic counselling (GC), describing this process and its characteristics, in particular we will emphasize on psychiatry genetic counseling (PGC). PGC has particular characteristics considering the fact that there are still no tools (genetic testing) for diagnosis and that most mental pathologies are multifactorial inheritance disorders. This is complex, since it results from the interaction of gen

The Alberta Hereditary Diseases Program: a regional model for delivery of genetic services.

Genetic counselling and related services are generally provided at major university medical centres because they are very specialized. The need for rurally based genetic services prompted the inclusion of an outreached program in the Alberta Hereditary Diseases Program (AHDP), which was established in 1979; the AHDP was designed to provide services to the entire province through two regional centres and seven outreach clinics. There is a community health nurse in almost every health unit whose duties are either totally or partially devoted to the AHDP; thus, genetic help and information are as

The human language faculty: genetically determined? An approach to testable hypotheses.

A kind of generation gap seems to divide researchers interested in the evolutionary status of the human language capacity. There is still opposition to the theory of a genetically determined universal or primal grammar. But although not well acquainted with the transformational-generative linguistic methodology that led to the hypothesis, an increasing number of linguists base their work with a "new grammar" on the acceptance of innateness of the human language faculty. It is proposed that dyslexia offers a hypothesis testable with the new advanced gene mapping, and that Creole Languages and S

A historical perspective on Lionel Penrose: Scientist, geneticist and dedicated opponent of eugenics.

The paper explores Lionel Penrose's scientific work. Penrose investigated the causes of mental disorders from clinical and genetic points of view. His investigations on phenylketonuria and Down syndrome helped to demonstrate the heterogenous character of mental disorders, whose causes can range from genetic with high penetrance, to largely environmental. He was specifically selected by JBS Haldane to become University College London's third Galton Chair as a result of his Colchester survey investigations. He became the first Galton Chair who had medical training. He never concealed his distast

Susceptibility genes and the question of embodied identity.

Drawing on an assumption of the co-construction of the material and the social, late-onset Alzheimer's disease (AD) is used as an illustrative example to assess claims for an emergent figure of the "individual genetically at risk." Current medical understanding of the genetics of AD is discussed, followed by a summary of media and AD society materials that reveal an absence of gene hype in connection with this disease. Excerpts from interviews with first-degree relatives of patients diagnosed with AD follow. Interviewees hold complex theories of causation. After genetic testing they exhibit fe

Implementing genetic counselling supervision in southwestern Europe: the MAG-SUDOE programme.

BACKGROUND: Advances in medical genetics and the expansion of personalized medicine have made genetic counselling essential. The increasing workload of genetics professionals highlights the need for strategies that strengthen the services resilience. Genetic counselling supervision (GCS) is a structured routine of reflective practice that promotes safe practice, prevents burnout, and prepares professionals to adapt to new challenges, while supporting continuous improvement. However, GCS practices vary widely between countries: in many, these are not even known, while in others they are mandato

A Comprehensive Approach to Clinical Decision Support in the Return of Genome Informed Risk Assessments to Primary Care Pediatricians.

BACKGROUND: Primary care pediatricians play an important role in genetic testing, including referrals, test ordering, responding to results, assessing risk, treatment, and managing care. As genetic testing rapidly evolves to include new tests identifying patients at risk for certain conditions, alert-based clinical decision support is insufficient in assisting pediatric primary care providers in working with patients, parents, genetics, and other specialties. Supporting pediatricians in the return of these results requires addressing gaps in genetics training and integrating genetics into prac

'Steep learning curves' to 'Smooth Sailing': A reappraisal of telegenetics amidst the COVID-19 pandemic.

The COVID-19 pandemic has pushed medical providers to trial telemedicine on a scale that lacks precedent. In genetic medicine, nearly overnight genetics providers were asked to transition to telemedicine platforms, irrespective of their previous experience with these modalities. This push to telegenetics prompted a reappraisal of the practice, as genetics providers learned firsthand about the feasibility, benefits, and drawbacks of telegenetics and telesupervision, all of which raise questions about the potential incorporation of these platforms beyond the pandemic. Adding to nascent literatur

Breast Fellows Forum: A Multidisciplinary Core Curriculum Model for Surgical Oncology Fellowships.

Subspecialty fellowship training is becoming more commonplace and medical practice more multidisciplinary. However, specialized training programs are challenged to provide a core curriculum that covers the interdisciplinary aspects of their field. Here we present a joint, interdisciplinary, interactive didactic program for breast fellowships in surgery, pathology and imaging. A yearlong core curriculum composed of topics of relevance to breast fellows involved in diagnostics and clinical care was created. Two hour long fellow-run interactive in-person sessions occur monthly. Additionally, facu

Cardiomyopathy due to PRDM16 mutation: First description of a fetal presentation, with possible modifier genes.

PRDM16 (positive regulatory domain 16) is localized in the critical region for cardiomyopathy in patients with deletions of chromosome 1p36, as defined by Gajecka et al., American Journal of Medical Genetics, 2010, 152A, 3074-3083, and encodes a zinc finger transcription factor. We present the first fetal case of left ventricular non-compaction (LVNC) with a PRDM16 variant. The third-trimester obstetric ultrasound revealed a hydropic fetus with hydramnios and expanded hypokinetic heart. After termination of pregnancy, foetopathology showed a eutrophic fetus with isolated cardiomegaly. Endocard

Multiplexed Assays of Variant Effect and Reclassification of TYR Variants in Chinese Patients with Oculocutaneous Albinism.

Oculocutaneous albinism type 1, caused by mutations in the tyrosinase gene TYR gene, is the most common form of albinism worldwide. Owing to the variants of uncertain significance (VUSs) classified by the American College of Medical Genetics and Genomics guidelines, patients who carry these VUSs do not receive a definitive molecular diagnosis. To clarify the pathogenicity of the VUS variants, we conducted multiplexed assays of variant effect to reclassify the TYR VUS variants. Protein expression, melanin production, enzyme activity, and subcellular localization were applied to 46 selected vari

Osteogenesis imperfect: clinical and epidemiological findings in a series of pediatric patients.

BACKGROUND: Osteogenesis imperfecta (OI) is the most common hereditary bone disorder with an incidence of one in 10,000-25,000 births. It is caused mainly by mutations in the genes that code for Type I collagen chains. In most cases, it shows an autosomal dominant inheritance pattern. OI is characterized by an increase in bone fragility that leads to frequent fractures, which cause pain, deformity and disability associated with other alterations. The objective of this study was to present the clinical and epidemiological characteristics of a series of pediatric patients diagnosed with OI evalu

Ophthalmic Pathology: Implications for Modern Medical and Surgical Ophthalmology - The 2024 Gordon K. Klintworth Lecture.

BACKGROUND: Ophthalmic pathology has been of paramount importance for the development of medical and surgical ophthalmology. Unfortunately, many laboratories are being closed down due to short-sighted financial reasons. This lecture aims at demonstrating the continuing significance of this subspecialty even in modern times. SUMMARY: Various examples from different structures of the eye (conjunctiva, cornea, retina) are illustrated to document the relevance of a profound knowledge of ophthalmic pathology for the advancement of both medical and surgical treatment. KEY MESSAGE: The impact of op

Genetic services at a center for craniofacial anomalies.

Advances in medical genetics and syndrome delineation have demonstrated that many clinical entities are an expression of genetic variability. Assessment of the population at the Center for Craniofacial Anomalies of the University of Illinois Medical Center at Chicago provided a measure of the need for genetic diagnosis and counseling. While it is recognized that this experience is not necessarily representative of that prevailing at other centers, this report provides a basis for interinstitutional comparisons. Public recognition of the need and consequent demand for genetic services is increa
