Gentamicin resistance among Escherichia coli strains isolated in neonatal sepsis.

Neonatal sepsis is a significant cause of morbidity and mortality among term and preterm infants. Ampicillin and gentamicin are standard empiric therapy for early onset sepsis. Four cases of neonatal sepsis secondary to Escherichia coli (E. coli) found to be gentamicin resistant occurred within a five week period in one neonatal intensive care unit (NICU). To determine whether these cases could be tied to a single vector of transmission, and to more broadly evaluate the incidence of gentamicin resistant strains of E. coli in the neonatal population at our institution compared to other centers,

Nephroprotective Effect of Echinodorus macrophyllus Micheli on Gentamicin-Induced Nephrotoxicity in Rats.

BACKGROUND/AIMS: Leaves of Echinodorus macrophyllus (EM), from the Alismataceae family, have been used in Brazilian folk medicine for their anti-inflammatory and diuretic properties. In this work, the diuretic and nephroprotective activities of crude extracts of EM were evaluated. METHODS: Normal Wistar rats were given 0.9% NaCl containing either EM (10-300 mg/kg), furosemide (13 mg/kg) or arginine vasopressin (0.2 mg/kg). Thereafter, the rats were individually housed in metabolic cages, and urine volume was measured every 30 min for a total of 3 h. Acute kidney injury was induced by gentamic

Treatment of full-thickness pressure ulcers with a gentamicin sponge: a case report.

BACKGROUND: Five pressure ulcers (stage III and stage IV) were successfully treated with collagen sponges filled with gentamicin. CASE DESCRIPTION: All ulcers occurred in an immobile 94-year-old woman with advanced dementia. One of the pressure ulcers, located on the patient's sacrum, was considered life-threatening due to size and depth. All ulcers were treated with a collagen sponge filled with gentamicin; the sponge was replaced at variable intervals, ranging from 6 days to 2 months, depending on absorption of the collagen. All 5 pressure ulcers improved based on reductions in wound depth

Short-term gentamicin therapy and risk of renal toxicity in patients with bacteraemia.

The renal safety of gentamicin has been questioned even when dosed once daily. This retrospective cohort study comprised adult patients with community-acquired bacteraemia and no record of chronic renal disease or malignancy. We included patients treated with gentamicin once daily for ≤ 5 days (165 exposed patients) and a balanced sample of patients not receiving aminoglycosides (150 non-exposed patients). The primary endpoint, which was an elevation in plasma creatinine of > 40 μmol/l from baseline, was equally common among exposed (7.9%) and non-exposed patients (8.7%) (p = 0.80). Among 26 p

CLIMP-63 is a gentamicin-binding protein that is involved in drug-induced cytotoxicity.

Aminoglycoside-induced nephrotoxicity and ototoxicity is a major clinical problem. To understand how aminoglycosides, including gentamicin, induce cytotoxicity in the kidney proximal tubule and the inner ear, we identified gentamicin-binding proteins (GBPs) from mouse kidney cells by pulling down GBPs with gentamicin-agarose conjugates and mass spectrometric analysis. Among several GBPs specific to kidney proximal tubule cells, cytoskeleton-linking membrane protein of 63 kDa (CLIMP-63) was the only protein localized in the endoplasmic reticulum, and was co-localized with gentamicin-Texas Red (

Novel bioactive hydrophobic gentamicin carriers for the treatment of intracellular bacterial infections.

Gentamicin (GEN) is an aminoglycoside antibiotic with a potent antibacterial activity against a wide variety of bacteria. However, its poor cellular penetration limits its use in the treatment of infections caused by intracellular pathogens. One potential strategy to overcome this problem is the use of particulate carriers that can target the intracellular sites of infection. In this study GEN was ion-paired with the anionic AOT surfactant to obtain a hydrophobic complex (GEN-AOT) that was formulated as a particulated material either by the precipitation with a compressed antisolvent (PCA) met

Initial low-dose gentamicin for Staphylococcus aureus bacteremia and endocarditis is nephrotoxic.

BACKGROUND: The safety of adding initial low-dose gentamicin to antistaphylococcal penicillins or vancomycin for treatment of suspected Staphylococcus aureus native valve endocarditis is unknown. This study evaluated the association between this practice and nephrotoxicity. METHODS: We performed a prospective cohort study of safety data from a randomized, controlled trial of therapy for S. aureus bacteremia and native valve infective endocarditis involving 236 patients from 44 hospitals in 4 countries. Patients either received standard therapy (antistaphylococcal penicillin or vancomycin) plu

Role of tissue kallikrein in prevention and recovery of gentamicin-induced renal injury.

Gentamicin is an aminoglycoside antibiotic that induces severe nephrotoxicity and acute renal failure. In the current project, we investigated the protective effects of tissue kallikrein (TK) protein administration (1 mug/h via osmotic minipumps) on kidney damage, apoptosis, and inflammation both during and after a 10-day regimen of gentamicin (80 mg/kg body weight/day sc) in Sprague-Dawley rats. TK infusion during gentamicin treatment significantly attenuated drug-induced renal dysfunction, cortical damage, and apoptosis. Moreover, TK reduced inflammatory cell accumulation in conjunction with

Vestibular function at the end of intratympanic gentamicin treatment of patients with Ménière's disease.

The aim of this study is to analyze the effects of intratympanic gentamicin injections on vestibular function in 33 patients with unilateral Meniere's Disease (according to AAO-HNS guidelines 1995) that had been unresponsive to medical therapy for at least one year. In such patients, the results of bedside examination of vestibular function vestibular examination is compared to those from laboratory tests. Intratympanic gentamicin injections (27 mg/ml) were performed at weekly intervals until symptoms or signs of vestibular hypofunction developed in the treated ear. Vestibular function was eva

Drop attacks and vertical vertigo after transtympanic gentamicin: diagnosis and management.

Drop attacks represent a significant problem during the natural course of Meniere's disease. They are characterized by a sudden fall to the ground without loss of consciousness. Diagnosis is clinical and based on the typical description of the patient. Involvement of vertical canal is possible during Meniere's disease and also after gentamicin application. Treatment of drop attacks is still a matter of discussion; most cases have a benign course with spontaneous remission and no treatment is necessary. In severe cases, aggressive treatment (surgical or pharmacological) is necessary. A case of

Efficacy of an ototoxic aminoglycoside (gentamicin) on the differentiation of the inner ear of cichlid fish.

Previous investigations revealed that the growth of fish inner ear otoliths depends on the amplitude and the direction of gravity, thus suggesting the existence of a (negative) feedback mechanism. In the course of these experiments, it was shown that altered gravity both affected otolith size (and thus the provision of the proteinacious matrix) as well as the incorporation of calcium. It is hitherto unknown, as of whether sensory hair cells are involved either in the regulation of otolith growth or in the provision of otolithic material (such as protein or inorganic components) or even both. Long-term results of gentamicin inner ear perfusion in Ménière's disease.

Chemical perfusion of the inner ear is an increasingly popular treatment for Ménière's disease. The authors report on the long-term results of 22 patients treated with gentamicin delivered via a round window micro-catheter. Patients with Ménière's disease underwent continuous, low dose (10 mg/ml) gentamicin infusion at 5 microlitre per hour for 10 days, through a micro-catheter placed into the round window niche. Vertigo was controlled in 95 per cent, whilst preserving hearing in 77 per cent. Hearing improved in three patients. Three patients with pre-operative serviceable hearing (PTA < 50 db

Intratympanic gentamicin for the treatment of Meniere's disease and other forms of peripheral vertigo.

Titration therapy with intratympanic gentamicin offers class A or B control of vertigo in 87% (range, 75%-100%) of patients with unilateral Ménière's disease. The risk of additional hearing loss is about 21% (range,0-37%). Vertigo may recur, however, in nearly one third of patients over time. These recurrences can also be treated by intratympanic gentamicin with a similar risk of hearing loss. The salient effect of intratympanic gentamicin is probably the reduction of vestibular function through damage to hair cells, but a complete ablation of function does not seem to be necessary for vertigo

Protective effects of glycyrrhizin on gentamicin-induced acute renal failure in rats.

The aim of this study was to investigate the effects of glycyrrhizin (200 mg/kg/day) on renal function in association with the regulation of aquaporin 2 water channel in rats with gentamicin (100 mg/kg/day)-induced acute renal failure. Polyuria in rats with gentamicin-induced acute renal failure was associated with down-regulation of renal aquaporin 2 in the inner and outer renal medulla, and cortex. Glycyrrhizin administration restored the expression of aquaporin 2 with paralleled changes in urine output. Changes in renal functional parameters, such as creatinine clearance, urinary osmolality
