Giant cell arteritis as an antigen-driven disease.

Emerging data support the model that giant cell arteritis (GCA) is an antigen-driven disease. Select helper T cells are recruited to the vascular lesions where they produce a defined pattern of cytokines. At least two functionally distinct populations of macrophages are involved and macrophage activation extends into the compartment of circulating phagocytes. Searches for the antigen may be facilitated through structural analysis of specially activated T lymphocytes in the lesions. In addition to the proposed disease-inducing agent, genetic risk factors are important. One of the genetic elemen

Giant cell tumour of the seventh cervical vertebra.

A 43-year-old man presented with bubbly expansion and rarefaction of the body of the seventh cervical vertebra. The lesion involved the right side of the vertebral arch and had a soft-tissue component extending mainly anterior to the vertebra. Histopathologic examination of the specimen obtained by open biopsy revealed a giant cell tumour. Giant cell tumours of the spine, excluding the sacrum, are rare. Radiographically, they may be confused with metastatic carcinoma, plasmacytoma, lymphoma, chordoma and even benign lesions, particularly aneurysmal bone cyst and brown tumour of hyperparathyroi

Occult giant cell arteritis: a diagnosis of suspicion.

BACKGROUND: Giant cell arteritis (GCA), also known as temporal or cranial arteritis is an inflammatory disease of arterial vessels which often involves the short posterior ciliary vessels resulting in an ischemic optic neuropathy. If not recognized and treated appropriately devastating preventable blindness may result. RESULTS: This review will present a case of "occult" giant cell with a discussion of the histopathology, symptomatology, diagnosis and management of this intriguing disease entity. CONCLUSION: Giant cell arteritis may masquerade with subtle easily overlooked symptoms and camou

Giant-cell arteritis of the female genital tract.

A 56-year-old woman with long-lasting fever of unknown origin was diagnosed as having a giant-cell arteritis of the genital tract with no evidence of temporal arteritis. Diagnosis relied on pathological examination, which showed a segmental panarteritis of ovaries, myometrium, endometrium and uterus cervix. Corticosteroid therapy led to clinical cure within a few weeks. Twenty-five cases of giant-cell arteritis of the female genital tract have been published of which only four were associated with temporal arteritis. We recommend that such a diagnosis should be considered in women presenting w

Neurologic aspects of giant cell (temporal) arteritis.

Headache is the most frequent symptom for which a patient with giant cell arteritis (GCA) presents to a neurologist. Amaurosis fugax and ischemic optic neuropathy are well-recognized complications. Less commonly recognized neurologic complications include transient ischemic attacks, cerebral infarctions, acute confusional states (due to multi-infarct dementia), ischemic cervical myelopathy, and ischemic mononeuropathies. Because patients with GCA generally respond well to corticosteroid therapy, prompt diagnosis can minimize neurologic damage.

Giant mast cell granules in a solitary mastocytoma.

A 4-month-old girl had an isolated, reddish brown nodule on her back that blistered repeatedly. Toluidine blue staining of a biopsy specimen showed a dense infiltration of mast cells in the upper dermis. In addition, large granules that stained similarly to the normal mast cell granules were observed. Electron microscopic studies disclosed that these giant granules (2-4 microns diameter) had the characteristic substructures of mast cell granules, that is, lamellar and scroll-like forms. Some giant mast cell granules lost their electron density, suggesting a degranulation process. Aggregates of

Giant cell astrocytoma of the retina in tuberous sclerosis.

The clinical and pathologic features of an unusual retinal glioma that was the only clinically overt manifestation of tuberous sclerosis in a 27-year-old woman are reported. The tumor was composed predominantly of large pleomorphic cells with glassy eosinophilic cytoplasm. Immunologic staining yielded positive results for gamma enolase, but not for glial fibrillary acid protein. The histologic and immunopathologic features of this tumor were essentially identical to the subependymal giant cell astrocytoma found in tuberous sclerosis. This case illustrates the large overlap in dysplastic astroc

Systemic giant cell arteritis and cerebellar infarction.

BACKGROUND AND PURPOSE: Systemic giant cell arteritis causing cerebellar infarction due to intracranial arteritis of the anterior inferior cerebellar artery has not been previously reported. We report this infrequent occurrence and discuss the differential diagnosis. CASE DESCRIPTION: An 85-year-old woman was admitted with a 2-week history of episodic ataxia, unilateral headaches, and vomiting. She had a history of atrial fibrillation and breast carcinoma. Her mental state was initially normal, and there were right-sided cerebellar signs. An ejection systolic murmur was heard, and peripheral

[Giant cell tumors of the pancreas. Clinical presentation].

The authors report the case of a large tumor located at the tail of pancreas. Despite extensive surgery and absence of synchronous metastasis, death occurred after a short period of survival. The authors stress the lack of major clinical features of carcinoma of left pancreas and outline the high malignancy of giant cell type.

[Giant cell tumors of the ribs. Apropos of a case].

The authors report a case of grade I-II giant-cell tumour of the ribs extending to the thorax and the vertebral column. The tumour was treated by surgical resection followed by vertebral curettage in two stages and adjuvant regional radiotherapy. Giant-cell tumours of the ribs are exceptional and rarely found in the literature. They raise a therapeutic problem (surgery is essential, but radiotherapy is controverted), and a prognostic problem (there is always a potential risk of recurrent malignancy).

Giant cell tumor in skeletally immature patients.

The radiographs and clinical and surgical histories of 50 skeletally immature patients with histologically verified giant cell tumor (GCT) of the long and short tubular bones were retrospectively studied to determine the prevalence, location within bone (eg, epiphysis, metaphysis), skeletal distribution, radiographic appearance, and pathogenesis of GCTs. Skeletal immaturity was determined radiographically by the presence of open epiphyses. Patients were selected from a group of 876 patients who were seen in consultation with documented GCT of the tubular bones. Approximately 5.7% of all GCTs o

[Dementia as a presentation symptom of giant cell arteritis].

Giant Cells Arteritis (GCA) can be presented sometimes with atypical manifestations which greatly difficult its diagnosis. We present a case of GCA in a 58-year-old man with dementia as first symptom. In the anamnesis, signs of rheumatic polymyalgia were observed, as well as a recently started dementia with a very disturbed mini-mental test. The analytic study showed anemia (Hb of 7.44 nmol/l, with 3.2 x 10(12) red cells and a GSR of 85 mm). The diagnosis of GCA was supported by a temporal biopsy. In a study of magnetic resonance, there were no evidences of brain infarct areas. After treatment…

Chromosomal abnormalities in giant cell tumors of bone.

Cytogenetic analysis of short-term cultures from ten giant cell tumors of bone revealed clonal and nonclonal chromosome abnormalities in three tumors and nonclonal changes only in seven. None of the clonal aberrations, inv(21)(p11q21) in one tumor, +5 in another, and t(15q22q), dic(4;22)(p16;p1?), double minutes, dicentrics, and ring chromosomes present in three separate clones in the third tumor, were identical to previously reported clonal changes in giant cell tumors. Telomeric associations were found in five tumors. The telomeres of chromosome arms 19q and 15p were particularly frequently

Treatment of polymyalgia rheumatica/giant cell arteritis.

Corticosteroids control arteritis in GCA and suppress polymyalgic symptoms within days of starting treatment. PMR patients can be treated with approximately 15 mg prednisolone/day, reducing the dose to 7.5-10 mg by 8 weeks. GCA is normally controlled on 40 mg prednisolone/day, although patients with persistent visual symptoms may need 60-80 mg. Slow reduction to about 20 mg by 8 weeks should minimize relapses. For both PMR and GCA a maintenance dose of 7.5 mg after 6-9 months should be enough. Steroid withdrawal is possible within 2 years of starting treatment, although some will need 4 years

Giant cell vasculitis with extravascular granulomas in an adolescent.

We describe an 18-year-old white male who developed lower extremity ischemia requiring amputation. He presented at 14 with pulmonary infiltrates, hepatosplenomegaly, fever, rash, adenopathy, uveitis, and arthralgias; clinical and laboratory findings were consistent with Mycoplasma pneumoniae infection. Despite adequate treatment with antibiotics, he developed chronic arthralgias and fevers, with rash and pericardial effusion. Criteria for the diagnosis of systemic lupus erythematosus were not met; juvenile rheumatoid arthritis was diagnosed presumptively. Over the subsequent 4 years he develop

[Unusual forms of the course of giant-cell arteritis].

Giant cell arthritis may be combined with retroperitoneal fibrosis, or lesions of the brachial plexus. Horner's Syndrome may develop. Occult vascular disease of cranial arteries was established in the presence of generalized arthritis, including the vessels of an ovarian tumor. Giant cell arthritis may be generalized at the time of detection, especially in patients presenting unusual clinical courses.
