Benign symmetric lipomatosis of the tongue in Madelung's disease.

INTRODUCTION: Madelung's disease is characterized by benign, non-encapsulated accumulations of fat in a symmetrical manner. Although rare, s … usions and buccal displacement of the teeth. The masses were soft and non-tender. Bilateral partial glossectomy was performed under general anaesthesia. Histology revealed adipose tissue, interspersed within the lingual musculature. DISCUSSION: Lipomas in Madelung's disease are characterized by multiplicity, non-encapsulation and invasiveness. Over the years …

[Lingual-mandibular remodeling resection in prognathism].

The combination of --hypoplastic upper jaw, with narrow soft palate, --glossoptosis (or low, propulsive tongue), associated with protrusion of lower jaw, is well known. Early, exclusively orthopedic management combines enlargement of the superior arcade; functional/postural re-education of the tongue. Surgical management comes in later; it associates modeling resection of the tongue intended to position the tongue against the palate, with Chevron's mandibular resection, suppression of the first premolar, and salvaging the mental nerve. This type of osteotomy is indicated whenever a 6 to 8 mm m

A Rare Case of Malignant Transformation of Oral Lichen Planus of the Mandible.

Oral lichen planus (OLP) is an immune-mediated mucocutaneous disease associated with an increased risk in oral squamous cell carcinoma (OSCC … d with a composite resection including a segmental mandibulectomy, buccal mucosa resection, partial glossectomy, and ipsilateral neck dissection. This was reconstructed with a free fibula osteo-septo-cutaneous flap. Mandibular OSCC is a rare complication of OLP with few reports on effective reconstructive interventions. The case represents the youngest report …

Modification of the anterior-posterior tongue rotation flap for oral tongue reconstruction.

The aim of this study is to evaluate the efficacy and long-term outcome of a modification of the anterior-posterior tongue rotation flap used for reconstruction of tongue following cancer ablation. From April 2016 to September 2017, 21 consecutive patients with tumor classification (T2), lymph node classification (N0) tongue cancer on the anterior and middle one-third portion were selected for this study. A modification of the anterior-posterior tongue rotation flap was selected for tongue reconstruction after the cancer resection. Patient's scorings of swallowing, chewing, speech and taste we

The surgical management of severe macroglossia in systemic AL amyloidosis.

Amyloidosis is a disease characterised by the deposition in body tissues of amyloid: abnormal protein in a beta pleated sheet formation. It is a systemic disorder and macroglossia may be seen in all forms. Changes to the normal architecture of the tissues and systemic features of the disease and its underlying cause can complicate the surgical management of the enlarged tongue.

The expansion of the hypopharynx by correction of glosso-larynx.

The width of the hypopharynx and oropharynx before and after the operation correction of glosso-larynx (CGL) was compared by cranial X-ray film. After the operation, the hyoid bone was seen to have moved downward, and the cranial bone was observed to have rotated forward. As a result, the hypopharynx and the oropharynx were expanded. Expansion of the hypopharynx resulted in the decreased resistance of flow decreased and the increased in airflow.

Examination of swallowing after total laryngectomy using manofluorography.

Investigation of postlaryngectomy dysphagia is usually limited to the standard barium swallow. Manofluorography (mano, manometry; fluoro, videofluoroscopy; graphy, picture) is a new technique that permits analysis of simultaneous manometry and videofluoroscopy of deglutition. Manofluorography provides more detailed analysis of the swallowing dynamics during the pharyngeal stage of deglutition than either barium studies or manometry used alone. This study uses manofluorography to examine swallowing in two patient groups, total laryngectomees and total laryngectomees with tongue impairment, to a

Lingual liposarcoma transitioning from an infiltrative lipoma in a dog.

Lingual neoplasms comprise approximately 2-4 % of canine oropharyngeal tumours, with liposarcomas being particularly rare and typically arising in the subcutis of the extremities and trunk. This report describes a 5-year-old spayed female Bedlington Terrier that presented with a ruptured lingual mass with muscular invasion, for which glossectomy was performed. Histopathological examination revealed a neoplastic proliferation of well-differentiated adipocytes that infiltrated the skeletal muscle layer. The deeper portion of the mass had dedifferentiation of adipocytes and malignant transformati…

Plunging Ranula: A Case Report of a Rare Late Complication After Tongue Cancer Surgery.

A plunging ranula is a benign cystic lesion in the neck formed due to mucin extravasated from a salivary gland, most commonly the sublingual gland. Ranulas have been described in association with congenital anomalies, trauma, diseases of the sublingual gland, and HIV; however, rarely, they may result as a complication of various oral and neck surgeries. Here, we report a rare case of plunging ranula that developed in an elderly male as a sequalae to surgery for tongue cancer. The patient had undergone a partial glossectomy with supra-omohyoid neck dissection for tongue carcinoma and nine month…

Adjuvant Postoperative CD40 Agonist and PD-1 Antagonist Combination Therapy in Syngeneic Tongue Cancer Mouse Model.

BACKGROUND/AIM: Using a syngeneic tongue cancer mouse model, the effect of CD40 agonist and PD-1 antagonist combination therapy for local recurrence after surgery was evaluated in a partially depleted CD4 model. MATERIALS AND METHODS: C3H/HeN mice were injected with 0.05 mg of the anti-mouse CD4 clone GK1.5, causing partial depletion of CD4 cells. Tongue cancer was induced by injecting the squamous cell carcinoma (SCC) VII cell line, the tumor was resected by partial glossectomy, and CD40 agonist and/or PD-1 antagonist therapy was administered postoperatively. RESULTS: Partial depletion of C…

Functional Perspectives in Tongue Reconstruction Based on Perforator Free Flap.

This article explores advancements in functional tongue reconstruction after cancer ablation, focusing on the importance of flap selection, positioning, and volume adjustment to restore speech and swallowing function. It highlights advancements such as the perforator flap concept for customized reconstructions and the transition to dynamic techniques with motor-innervated free flaps, aiming to accurately replicate the tongue's inherent functions. The effectiveness of dynamic techniques in improving swallowing efficiency and speech clarity underscores their significant potential in enhancing po

Primary clear cell sarcoma of the tongue and surgical reconstruction: About a rare case report.

Clear cell sarcomas (SCC), also called "soft-tissue melanoma", are rare and aggressive tumors that preferentially affect the lower limbs (tendons and fasciae) and which have also been described in head and neck localizations. Their clinical and immunohistochemical mimicry with melanoma makes it difficult to diagnose sarcomas. SCC treatment is mainly focused on large-scale resection surgery with adjuvant radiotherapy because of their low chemo-sensitivity and extreme lymphophilia. In case of head and neck localization, these treatments may lead to function and aesthetic sequelae thus requiring

[Macroglossia as the initial symptom of amyloidosis].

Amyloidosis is a term that describes a heterogeneous group of disorders in which various tissues contain the characteristic insoluble fibrillar protein known as amyloid. A number of different forms of presentation have been described and include senile, familial or hereditary, systemic, localized, secondary and primary forms. We now report our management of a 79-year old woman who suffered from primary A-lambda-light chain amyloidosis. Amyloid changes first presented in the tissues of the tongue and floor of mouth and were seen clinically as a macroglossia. Further investigation led to a prima

Surgical Surprise and a Ryle's Tube Misadventure: An Intraoperative Gastric Perforation.

Nasogastric tube (NGT) insertion is routinely performed for enteral feeding and gastric decompression and is generally considered safe. However, it can lead to complications ranging from minor discomfort, epistaxis, and sinusitis to severe events such as respiratory injury, esophageal perforation, and, rarely, gastrointestinal perforation. Among these, gastric perforation in adults is extremely uncommon. We describe the case of a 47-year-old woman with carcinoma of the left lateral tongue who underwent near-total glossectomy with bilateral neck dissection, tracheostomy, and reconstruction. Aft…

Patterns of cervical lymph node metastases in oral tongue squamous cell carcinoma: implications for elective and therapeutic neck dissection.

OBJECTIVES: To determine the patterns of lymph node metastases in oral tongue carcinomas, and examine the implications for elective and therapeutic neck dissection. METHOD: The study entailed a retrospective analysis of 67 patients with previously untreated oral tongue squamous cell carcinoma who had undergone simultaneous glossectomy and neck dissection. RESULTS: Of the 40 clinically node-negative patients, 7 patients had metastatic lymph nodes on pathological examination. No occult metastasis was found at level IV. Of the 27 clinically node-positive patients, the incidence rate of level IV…
