Bacillus cereus necrotizing pneumonia in a patient with nephrotic syndrome.

Bacillus cereus (B. cereus) is a Gram-positive rod that is widely distributed in the environment and can be a cause of food poisoning. We herein present a case of B. cereus necrotizing pneumonia in a patient with nephrotic syndrome under corticosteroid treatment after developing transient gastroenteritis symptoms. B. cereus was isolated from bronchial lavage fluid and transbronchial biopsy specimens. A multiplex polymerase chain reaction analysis of the toxin genes revealed a strain possessing enterotoxicity. The patient recovered after one week of intravenous meropenem followed by a combinati

Rothia dentocariosa bacteremia in children: report of two cases and review of the literature.

Rothia dentocariosa, a pleomorphic, fastidious, Gram-positive rod, is a normal inhabitant of the oropharynx. It is a well-known causative agent of dental plaques and periodontal disease. Generally regarded as of low virulence to humans, R. dentocariosa has been increasingly recognized as a pathogen in adults and often associated with infective endocarditis. It should not necessarily be regarded as a contaminant when the isolate comes from areas other than the oropharynx, especially from the blood. We report two cases of R. dentocariosa bacteremia, including an 8-month-old boy with repaired tra

[Pharyngitis with necrotising pneumonia caused by Arcanobacterium haemolyticum].

A 20-year-old woman developed symptoms of pharyngitis followed by generalised skin rash and pulmonary infiltrates with cavitation. Arcanobacterium haemolyticum was identified in blood culture, which was susceptible to the antibiotics given. After initiating pathogen-directed therapy, the patient recovered completely. A. haemolyticum is a Gram-positive rod that can grow under aerobic and anaerobic conditions. The pathogen causes a characteristic haemolysis when cultured on human blood agar. A. haemolyticum causes pharyngitis and skin rash, particularly in adolescents. If the infection is not tr

Mycobacterium setense isolated from a cat with atypical mycobacterial panniculitis.

Atypical mycobacterial panniculitis was diagnosed in a cat. Mycobacterium setense was identified as causative agent by 16 S rRNA gene sequence analysis. This a gram-positive rod-shaped acid-fast bacterium belonging to Mycobacterium fortuitum group was never reported before in diseased animals. Resistance to doxycycline and clarithromycin was detected. During treatment with pradofloxacin, additional resistance to fluoroquinolones developed which was due to a mutation in the gyrase gene gyrA (S90W exchange). Despite of antimicrobial treatment for 33 months the patient did not fully recover. Spec

Toxic Hemolysis in a Young Healthy Female.

Clostridium perfringes is a gram-positive, rod-shaped, anerobic, spore-forming pathogenic bacterium. C. perfringes is commonly known to cause tissue necrosis and gas gangrene. However, severe life-threatening complications like hemolysis due to clostridium sepsis are rarely encountered. We present a case of a 35-year-old female presenting with endometritis complicated with severe hemolysis in the setting of sepsis secondary to C. perfringes. Her hospital course was complicated with multi-organ involvement, requiring broad-spectrum intravenous antibiotics. Eventually, she made an uneventful rec

Isolation of an anaerobic bacterium which reductively dechlorinates tetrachloroethene and trichloroethene.

Strain TEA, a strictly anaerobic, motile rod with one to four lateral flagella and a crystalline surface layer was isolated from a mixed culture that completely reduces chlorinated ethenes to ethene. The organism coupled reductive dehalogenation of tetrachloroethene or trichloroethene to cis-1,2-dichloroethene to growth, using molecular hydrogen as the electron donor. It was unable to grow fermentatively or in the presence of tri- or tetrachloroethene with glucose, pyruvate, lactate, acetate or formate. The 16S rDNA sequence of strain TEA was 99.7% identical to that of Dehalobacter restrictus.

Genome-guided antimicrobial potential of Bacillus stercoris from coastal sand with activity against multidrug-resistant bacteria, including MRSA.

Coastal beach environments harbor diverse bacterial communities, yet their antimicrobial potential remains largely unexplored. This study evaluates the antimicrobial potential of a bacterial isolate 2AT10 from Brazilian beach sand through in vitro, in silico, and ex vivo assays. Isolate 2AT10, a rod-shaped Gram-positive bacterium, exhibited broad-spectrum antimicrobial activity, with its culture supernatants showing greater efficacy against Gram-positive bacteria, including multidrug-resistant strains such as methicillin-resistant Staphylococcus aureus (MRSA). Antimicrobial activity from two d

Clinical utility of universal 16S rRNA PCR using sterile body fluids: lung abscess caused by Nocardia farcinica.

Nocardiosis is an infectious disease caused by Gram-positive rod-shaped bacteria and presents as a suppurative granulomatous disease in patients with compromised immune systems. Few studies have investigated the clinical utility of the universal 16S rRNA polymerase chain reaction (PCR) method using sterile body fluids for diagnosing nocardiosis. A 64-year-old female patient was admitted to Chosun University Hospital with the complaint of fever. Computed tomography scans of her chest revealed the presence of empyema and an abscess in the right lung. Pus samples were collected using closed chest

[Sporotrichoid nocardiosis by Nocardia brasiliensis].

Nocardia brasiliensis is a gram-positive, branched, aerobic, acid-resistant, rod-shaped bacillus that inhabits in soil, rotten organic matter and waters. Cutaneous nocardiosis in immunocompetent individuals can manifest in three different forms: actinomycetoma, superficial skin infection or lymphocutaneous infection. A case of an 85-year-old woman with an ulcerated lesion on the back of her left hand with erythematous papules in the lymphatic tract infected by N. brasiliensis is described. The microorganism was presumptively identified by conventional and inexpensive methods for a medium compl

Microbiological findings in prepubertal and pubertal girls with vulvovaginitis.

UNLABELLED: Vulvovaginitis is a common and challenging gynaecological problem in prepubertal and pubertal girls. Such an infection, owing to a wide range of aetiologies, if not responding to hygienic measures, needs further investigation through vaginal cultures, since treatment should be tailored accordingly. This study aimed to investigate the pathogens isolated in prepubertal and pubertal girls with signs and symptoms of vulvovaginitis. A total of 2314 symptomatic girls, 1094 prepubertal and 1220 pubertal, aged 2 to 16 years, were included. Vaginal samples were inoculated on specific cultur

Cellulitis caused by Roseomonas mucosa in a child: a case report.

BACKGROUND: Roseomonas mucosa (R. mucosa) is a pink-pigmented, Gram-negative short rod bacterium. It is isolated from moist environments and skin, resistant to multiple drugs, including broad-spectrum cephalosporins, and a rare cause of infection with limited reports. R. mucosa mostly causes catheter-related bloodstream infections, with even fewer reports of skin and soft tissue infections. CASE PRESENTATION: A 10-year-old boy received topical steroid treatment for sebum-deficient eczema. A few days before the visit, he was bitten by an insect on the front of his right lower leg and scratched

Spoilage bacteria of fresh broiler chicken carcasses.

Studies were conducted to identify the bacteria responsible for spoilage of fresh broiler chicken carcasses and to characterize the off-odors these bacteria produce. Broiler carcasses were collected from processing plants in the northeast Georgia area, the southeastern U.S., Arkansas, California, and North Carolina. The carcasses were allowed to spoil under controlled conditions at 3 C and spoilage bacteria were isolated. Each spoilage bacterium was separately inoculated into a sterile chicken skin medium, incubated at 25 C for 48 h, and subjectively evaluated for odor. The bacteria isolated f

Schaalia turicensis endometritis in a diabetic patient: MALDI-TOF MS overcomes diagnostic barriers in a non-IUD-associated case.

Schaalia (Formerly Actinomyces) turicensis is a single-cell prokaryotic microorganism and a common component of the normal human oral flora. If it enters other parts and causes infection, it may lead to actinomycosis. We present the case of a 74-year-old female patient from Fuyang who developed bacterial endometritis due to Schaalia turicensis. The bacterial culture of painless cervical dilation with drainage and curettage showed slow-growing, Gram-positive rods, which were identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) as Schaalia tur…

[A case of Nocardia farcinica pneumonia treated with sulfamethoxazole-trimethoprim monitoring its serum concentration].

A 75-year-old male suffered from interstitial pneumonia in December 2000 and treated with predonisolone. The treatment was effective, and the dosage of predonisolone had been gradually tapered. In January 2001, when the dosage was 30 mg/day, he complained of cough and yellowish sputum. The chest X-ray and CT revealed bilateral infiltrations with cavities. He was treated with cefozopram and fluconazole. However, there were no improvements. The sputa of the 2nd, 3rd, 6th and 8th hospital days showed the presence of gram-positive branched rods, which were identified as Nocardia farcinica. Therefo

Lactobacillus casei pneumonia and sepsis in a patient with AIDS. Case report and review of the literature.

Lactobacilli are ubiquitous gram-positive anaerobic rods present in the normal bacterial flora of the mouth, vagina and gastrointestinal tract. Although they are usually non pathogenic, serious infections have occasionally been described in transplant recipients or severely ill patients. Only 4 cases have been reported involving AIDS: one had predisposing conditions other than AIDS, and none of the others had pure growth of lactobacilli. We report a case of community-acquired Lactobacillus casei pneumonia in a CD4 lymphocyte-depleted AIDS patient. Lactobacillus was isolated in pure growth in r

Comparative in vitro beta-lactam activity against aerobic and anaerobic surgical isolates.

The comparative in vitro activity of cefoxitin, piperacillin, cefotetan, and ceftizoxime was compared against 843 surgical microbial isolates. All compounds exhibited excellent activity against the streptococcal isolates. Antimicrobial activity was poor for Staphylococcus aureus and S. epidermidis. Cefotetan activity against the Enterobacteriaceae was comparable to ceftizoxime (greater than 95% susceptible). Resistance rates of 0, 1, 5, and 2% were observed with cefoxitin, piperacillin, cefotetan, and ceftizoxime against the anaerobic cocci and anaerobic gram-positive non-spore-forming rods. N
