Ten individuals with idiopathic neutropenia and similar numbers of normal and abnormal controls were tested for mobilization of their marginal granulocyte pools and bone marrow reserve by using epinephrine and hydrocortisone intravenously. Individuals with 'benign' idiopathic neutropenia appeared to have a normal response while half the abnormal controls responded poorly. It is suggested that granulocyte mobilization tests are valuable in the assessment of individuals with neutropenia.

The presence of lymphocytotoxic antibodies was studied in 22 patients receiving granulocyte transfusions. Due to previous blood transfusions 7 out of 22 patients had lymphocytotoxic antibodies before granulocyte transfusions were started. 6 out of 15 unsensitized patients developed antibodies during granulocyte transfusion therapy, and 9 out of 15 patients had no detectable antibodies at any time. Transfusion reactions occurred in the presence of lymphocytotoxic antibodies not directed against donor cells.

The granulocyte function was determined in healthy persons before and immediately after the infusion of 500 ml of dextran for 4 hours. An adult with long-standing neutropenia had the functional granulocyte abnormalities typical of the lazy leucocyte syndrome. Scanning electron microscopy of the patient's neutrophils showed alteration in the surface configuration of the cell with coarsening of the normal fine ruffles and the appearance of knob-like projections. Similar functional and anatomical changes were induced in normal neutrophils by treatment with vinblastine. The ingestion- and Nitro Blue Tetrazolium-(NBT)-reduction-capacity of neutrophil leukocytes in three newborn infants with lethal septicemia was studied using a modified NBT-test. In comparison to 25 controls in healthy newborns (umbilical vein-blood) the results indicate a failure in the functioning of leukocytes. Besides other disturbances the impaired ingestion- and NBT-reduction-capacity of the neutrophil granulocytes during an infection probably leads to septicemia.

Twenty-three severely neutropenic patients were given 31 courses of ABO and HL-A compatible granulocyte-rich plasma transfusions between 1974--76 at the National Institute of Haematology and Blood Transfusion. The criteria for the effectiveness of therapy were disappearance of fever and survival for over 3 weeks after granulocyte transfusion. Of the 28 evaluated cases granulocyte transfusions were effective in 16 and ineffective in 12 cases. The effectiveness of granulocyte transfusion was favourably influenced by early introduction of therapy, and continuation for several days.

Thirty episodes of infection in severely neutropenic children with acute leukaemia and aplastic anaemia resistant to antibiotics were treated with total of 51 granulocyte transfusions collected from normal donors by means of an Aminco continuous flow cell separator.
