Long-term stability of gingival margin and periodontal soft-tissue phenotype achieved after mucogingival therapy: A systematic review.

BACKGROUND: The aim of this systematic review was two-fold: (i) to evaluate the long-term (≥5 years) stability of the gingival margin position, keratinized tissue width (KTW) and gingival thickness (GT) in sites that underwent root coverage (RC) or gingival augmentation (GA); and (ii) to assess the influence of different local variables on the long-term stability of dental and gingival tissues. MATERIALS AND METHODS: Randomized controlled trials (RCTs) and non-RCTs reporting short-term (i.e., 6-12 months after baseline surgical intervention) and long-term (≥5 years) follow-up data after surgi

Bone marrow mesenchymal stem cell sheets with high expression of hBD3 and CTGF promote periodontal regeneration.

The multi-bacterial environment of the oral cavity makes it hard for periodontal regeneration. As a class of antimicrobial peptide, beta defensin has been found to show broad-spectrum antibacterial ability. In addition, connective tissue growth factor (CTGF) is demonstrated to play a great role in multi-physiological events such as angiogenesis, wound healing and, more importantly, fibrogenesis. In this study, human β defensin 3 (hBD3) and CTGF were co-transfected into bone marrow derived mesenchymal stem cells (BMSCs) for preparing cell sheets. The transfection efficiency was detected through

Periodontal regeneration: a challenge for the tissue engineer?

Periodontitis affects around 15 per cent of human adult populations. While periodontal treatment aimed at removing the bacterial cause of the disease is generally very successful, the ability predictably to regenerate the damaged tissues remains a major unmet objective for new treatment strategies. Existing treatments include the use of space-maintaining barrier membranes (guided tissue regeneration), use of graft materials, and application of bioactive molecules to induce regeneration, but their overall effects are relatively modest and restricted in application. The periodontal ligament is r…

The recent advances in scaffolds for integrated periodontal regeneration.

The periodontium is an integrated, functional unit of multiple tissues surrounding and supporting the tooth, including but not limited to cementum (CM), periodontal ligament (PDL) and alveolar bone (AB). Periodontal tissues can be destructed by chronic periodontal disease, which can lead to tooth loss. In support of the treatment for periodontally diseased tooth, various biomaterials have been applied starting as a contact inhibition membrane in the guided tissue regeneration (GTR) that is the current gold standard in dental clinic. Recently, various biomaterials have been prepared in a form o…

Remodeling of the Intra-Conduit Inflammatory Microenvironment to Improve Peripheral Nerve Regeneration with a Neuromechanical Matching Protein-Based Conduit.

Peripheral nerve injury (PNI) remains a challenging area in regenerative medicine. Nerve guide conduit (NGC) transplantation is a common treatment for PNI, but the prognosis of NGC treatment is unsatisfactory due to 1) neuromechanical unmatching and 2) the intra-conduit inflammatory microenvironment (IME) resulting from Schwann cell pyroptosis and inflammatory-polarized macrophages. A neuromechanically matched NGC composed of regenerated silk fibroin (RSF) loaded with poly(3,4-ethylenedioxythiophene): poly(styrene sulfonate) (P:P) and dimethyl fumarate (DMF) are designed, which exhibits a matc

Recurrent laryngeal nerve regeneration using an oriented collagen scaffold containing Schwann cells.

OBJECTIVES/HYPOTHESIS: Regeneration of the recurrent laryngeal nerve (RLN), which innervates the intrinsic laryngeal muscles such that they can perform complex functions, is particularly difficult to achieve. Synkinesis after RLN neogenesis leads to uncoordinated movement of laryngeal muscles. Recently, some basic research studies have used cultured Schwann cells (SCs) to repair peripheral nerve injuries. This study aimed to regenerate the RLN using an oriented collagen scaffold containing cultured SCs. STUDY DESIGN: Preliminary animal experiment. METHODS: A 10-mm-long autologous canine cerv

Revisiting the Peripheral Nerve Injury and Regeneration.

Peripheral nerve injury significantly impacts patients' quality of life, with poor nerve regeneration and insufficient functional recovery being urgent challenges. Even with early medical and surgical interventions, desired outcomes are often not achieved. Our research focuses on effective surgical and rehabilitation strategies, as well as the development of innovative technologies. Peripheral nerve injury repair is analyzed from three main perspectives: first, improving intrinsic axonal growth capacity, which involves signaling pathways and the regulation of neuromodulatory factors; second, e

Microsurgical regenerative approach to intrabony defects.

Periodontal regeneration is a validated treatment for intrabony defects, aiming to restore lost periodontal tissues through pocket depth reduction, clinical attachment, and bone gain with minimal recession. Despite its proven efficacy, outcomes vary widely due to patient-related factors (e.g., oral hygiene and smoking), defect characteristics, and surgical techniques. Minimally invasive approaches, supported by magnification and microsurgical tools, have enhanced surgical precision and clinical predictability. This paper presents a stepwise surgical protocol for minimally invasive periodontal

Open Wound Healing in Guided Bone Regeneration Using a Magnesium Membrane: A Paradigm Shift.

Guided bone regeneration (GBR) is essential in implant dentistry for managing bone deficiencies. Despite its high success rate, complications like wound dehiscence, membrane exposure, and infection can compromise outcomes. These issues are influenced by patient health, surgical technique, and the implanted biomaterials. Membrane exposure may lead to significant bone loss and jeopardise treatment success. A resorbable magnesium membrane has been recently introduced for GBR, but soft tissue healing complications have not yet been reported. This case series reviews four instances of wound dehisce

Additive manufacturing of Schwann cell-laden collagen/alginate nerve guidance conduits by freeform reversible embedding regulate neurogenesis via exosomes secretion towards peripheral nerve regeneration.

Peripheral nerve injury is a common clinical problem that could be debilitating to one's quality of life. The complex nerve guidance conduits (NGCs) with cells in order to improve nerve regeneration. Therefore, we used freeform reversible embedding of suspended hydrogels to fabricate Schwann cells (SCs)-laden collagen/alginate (Col/Alg) NGCs. First, we evaluated Col influence on the characteristics of NGCs. After which, Wharton's jelly mesenchymal stem cells (WJMSC) are seeded onto the inner channel of NGCs and evaluated neural regeneration behaviors. Results indicated the SCs-laden NGCs with

Repairing the human brain after stroke. II. Restorative therapies.

Spontaneous behavioral recovery is usually limited after stroke, making stroke a leading source of disability. A number of therapies in development aim to improve patient outcomes not by acutely salvaging threatened tissue, but instead by promoting repair and restoration of function in the subacute or chronic phase after stroke. Examples include small molecules, growth factors, cell-based therapies, electromagnetic stimulation, device-based strategies, and task-oriented and repetitive training-based interventions. Stage of development across therapies varies widely, from preclinical to late-ph

Extraction site reconstruction for alveolar ridge preservation. Part 2: membrane-assisted surgical technique.

Alveolar ridge resorption has long been considered an unavoidable consequence of tooth extraction. Guided bone regeneration techniques and the use of bone replacement materials have both been shown to enhance socket healing and to potentially modify the resorption process. This article will describe a surgical technique using textured, high-density polytetrafluoroethylene (PTFE) membrane and particulate bone replacement materials for graft containment and prevention of soft tissue ingrowth into healing extraction sites. The technique described does not require primary closure, facilitating the

Current developments in cell- and biomaterial-based approaches for stroke repair.

INTRODUCTION: Stroke is one of the most devastating diseases and a leading cause of mortality worldwide. So far, clinical management of stroke involves surgical clot retrieval or thrombolytic treatment inducing reperfusion of the occluded vessels in the cerebral infarcted area, which is dependent on early intervention following insult. New treatment strategies involve the promotion of angiogenesis and neuroplasticity, stimulation of endogenous neurogenesis, remyelinization, and immunomodulation by means of cell transplantation and sustained drug delivery. AREAS COVERED: This review describes

Histologic evaluation of new attachment utilizing a titanium-reinforced barrier membrane in a mucogingival recession defect. A case report.

Guided tissue regeneration was USED to treat a localized, deep, buccal recession on a mandibular left central incisor. This tooth was labially displaced and scheduled for extraction for orthodontic reasons. The mucogingival defect had 7 mm of facial gingival recession, and 4 mm mid-buccal probing depths. The attachment loss was 11 mm. There was no attached gingiva. At surgery the root was notched as landmarks for assessing the histological examination. A titanium reinforced barrier membrane was left in place for 4 weeks. At the time of extraction, 9 months post-initial treatment, there was a g…
