The neuropharmacology of sleep paralysis hallucinations: serotonin 2A activation and a novel therapeutic drug.

Sleep paralysis is a state of involuntary immobility occurring at sleep onset or offset, often accompanied by uncanny "ghost-like" hallucinations and extreme fear reactions. I provide here a neuropharmacological account for these hallucinatory experiences by evoking the role of the serotonin 2A receptor (5-HT2AR). Research has shown that 5-HT2AR activation can induce visual hallucinations, "mystical" subjective states, and out-of-body experiences (OBEs), and modulate fear circuits. Hallucinatory experiences triggered by serotonin-serotonergic ("pseudo") hallucinations, induced by hallucinogeni

New psychoactive substances: a review and updates.

New psychoactive substances (NPS) are a heterogeneous group of substances. They are associated with a number of health and social harms on an individual and societal level. NPS toxicity and dependence syndromes are recognised in primary care, emergency departments, psychiatric inpatient and community care settings. One pragmatic classification system is to divide NPS into one of four groups: synthetic stimulants, synthetic cannabinoids, synthetic hallucinogens and synthetic depressants (which include synthetic opioids and benzodiazepines). We review these four classes of NPS, including their c…

[Psychedelics and the unconscious: What are their clinical implications?].

The renewed interest in psychedelics in psychiatry calls for a reexamination of the concept of the unconscious. By profoundly altering states of consciousness, these substances appear to facilitate access to psychological content that is usually difficult to access. Their therapeutic value depends heavily on the therapeutic setting and the process of integration.

Embodied Transformation: Psychedelic Therapy Through the Lens of Merleau-Ponty's Phenomenology.

This paper develops a phenomenological account of the therapeutic significance of psychedelic-induced mystical experience, drawing on the philosophy of Maurice Merleau-Ponty. The central argument holds that the enduring therapeutic benefits of psychedelics emerge through a radical transformation in the structure of lived experience. Two prominent objections to the therapeutic value of mystical experience are examined and shown to rest on implicit Cartesian assumptions that obscure the embodied, situated character of human experience. Using Merleau-Ponty's concepts of the body-subject and the i

Indigenous Knowledge Systems & Psychedelic Science: Towards Ethical and Reciprocal Collaboration.

Indigenous Peoples have cultivated and protected natural psychoactive medicines through ceremony, kinship, and spiritual responsibility across generations, yet their long-standing contributions have often been marginalized through extractive research, commercialization, and policy exclusion. It is Indigenous communities that have stewarded and gained expertise working with psychoactive medicines for centuries, yet they remain underrepresented within the scientific discourse. This commentary advances the case for reciprocal and equitable collaboration in psychedelic science, grounded in Indigen

Classic Psychedelics for Chronic Pain: A Critical Review of the Literature and Practical Advice for Clinicians.

Chronic pain is common, costly, and for many, remains inadequately treated by existing pharmacologic and non-pharmacologic approaches. In parallel with growing dissatisfaction with conventional therapies, classic serotonergic psychedelics, such as psilocybin, lysergic acid diethylamide (LSD), ayahuasca, N,N-dimethyltryptamine (DMT), and mescaline, administered alone or within psychedelic-assisted therapy models, have re-emerged as potential therapeutic tools for a range of health conditions, including chronic pain. In this review, we examine putative mechanisms of action relevant to pain, incl

Challenges with clinical trial participants in studies with classical psychedelics: A position statement from the National Network of Depression Centers' task group on psychedelics and related compounds.

RATIONALE: Classical psychedelics-a broad class of compounds that include psilocybin, lysergic acid diethylamide, dimethyltryptamine, and mescaline-have shown significant promise for the treatment of mental health conditions in recent clinical trials. Organizations such as the National Network of Depression Centers (NNDCs) can play a pivotal role in uniting researchers and clinicians working in this field to explore and synthesize existing evidence as well as characterize emerging challenges. OBJECTIVES: We outline several categories of challenges that have emerged in the context of clinical

Psychedelics as a potential treatment for borderline personality disorder: A narrative review.

Borderline personality disorder (BPD) is a serious mental illness with high rates of morbidity and stigma; however, successful remission is frequently limited by a paucity of accessible treatment options. In an era of growing interest in psychedelics as novel psychiatric treatment modalities, patients with BPD are often excluded from research due to perceived safety risks, particularly pertaining to suicide and substance misuse. However, there is evolving evidence that psychedelic treatment may effectively target core BPD symptoms, in addition to those of the mood and anxiety disorders frequen

Psychological Adverse Effects of Psychedelic Use.

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. These discussions have given rise to rounds reports that will prove useful for clinicians practicing at the interface of medicine and psychiatry. Prim Care Companion CNS Disord 2026;28(3):25f0417

The purpose of the psychosocial protocol in the psychedelic-assisted therapy: A scoping review.

With growing research on the use of psychedelics to treat mental health conditions, greater attention to the psychosocial procedures accompanying substance administration is warranted. This scoping review aims to categorize psychosocial protocols used in research involving psychedelics as psychiatric treatment according to their purpose, denomination, format, therapeutic orientation, formalization, and duration. Experimental and observational studies were identified through online search platforms, covering Ayahuasca, Dimethyltryptamine, 5-methoxy-N,N-dimethyltryptamine, Lysergic Acid Diethyla

Intersections of Psychedelics, Mystical Experiences, and Hypnosis.

The potential therapeutic benefits of psychedelic, mystical experiences, and peak spiritual experiences are of increasing interest among clinicians, researchers, and the public. Several studies have proposed potential parallels between the altered states of consciousness associated with psychedelics and hypnosis. Although there has been increased interest in psychedelics, mystical experiences, hypnosis, and non-ordinary states of consciousness, much remains unknown. The present special issue addresses many of these questions, such as: What are the similarities and differences in hypnotic and p

Psychological support can be distinguished from psychotherapy: Clarifications for future empirical work.

Classic psychedelic studies involve interpersonal interactions with participants before, during, and after the acute effects of psychedelics. In the field of psychedelic research, there is currently a debate about whether this interpersonal interaction is most accurately considered "psychological support," "psychotherapy," or something else. Proponents of the psychological support label describe the interpersonal interaction in most classic psychedelic studies as aimed at safety, while proponents of the psychotherapy label argue that the interpersonal interaction involves intervening on psycho

Mind-Manifesting Hypnosis: Phenomenological Similarities and Differences in Hypnotic and Psychedelic Contexts.

In the 1970s, researchers and theoreticians of states of consciousness jointly discussed hypnosis and psychedelic alterations of consciousness, but recent research has mostly kept these topics apart. This paper discusses the similarities and differences of hypnosis and psychedelic alterations of consciousness, stressing that states of consciousness should not be defined by their preceding contexts. Predictors of positive responses to psychedelics (e.g. absorption and openness to experience) also predict hypnotic responsiveness. Most experiential changes (e.g. changes in bodily sensations and i

Psychedelic studies in nonhuman primates: Past and future.

Studies of serotonergic or 'classic' psychedelics in nonhuman primates (NHPs) have provided valuable information about the drugs' effects on the brain and behavior in closely related species to humans. Psychedelics induce characteristic changes to both spontaneous and operant behaviors in NHPs, though variability exists in the different effects reported by different studies; this variability could be due to factors like differences across drugs, differences in dose ranges across studies, and inter-individual variability in drug responsiveness. Several effects of psychedelics in NHPs mirror tho

Exploring serotonergic psychedelics as a treatment for personality disorders.

Both psychotherapeutic interventions and pharmacological agents have demonstrated limited efficacy in the treatment of personality disorders (PDs). Emerging evidence suggests that psychedelic therapy, already showing promise in treating various psychiatric conditions commonly comorbid with PDs, may exert therapeutic effects by promoting adaptive changes in personality. Thus, psychedelic therapy could hold potential for addressing core features of PDs through shared mechanisms of personality modulation. Although historical literature and observational studies suggest the potential clinical util

Psychedelic-assisted psychotherapy: The need to monitor adverse events.

The therapeutic use of psychedelics for mental health issues holds considerable promise. However, systematic assessment of adverse events associated with these substances has received relatively little attention. Here, we discuss several considerations concerning the assessment of adverse events in psychedelic-assisted therapies. We discuss the preference for using the term "adverse effects" over "side effects", as well as the ongoing debate regarding which substances are classified as psychedelic. We also provide recommendations on when and how to assess adverse effects, for example the impor
