Holt-Oram syndrome (HOS), which is caused by genetic changes in the TBX5 gene, affects the hands and heart. HOS patients have heart defects, including atrial septal defects (ASD), ventricular septal defects (VSD) and heart conduction disease. Here, we generated a homozygous TBX5 knockout human embryonic stem cell (hESC) line (TBX5-KO) using a CRISPR/Cas9 system. The TBX5-KO maintained stem cell like morphology, pluripotency markers, normal karyotype, and could differentiate into all three germ layers in vivo. This cell line can provide an in vitro platform for studying the pathogenic mechanism

Previous case studies have reported bradycardia-dependent augmentation of J waves in patients with idiopathic ventricular fibrillation (VF). However, the response patterns of J waves to changing heart rates have rarely been evaluated in the general population. In this review, the response patterns of J waves to varying RR intervals were analyzed in the general population during premature beats or pacing and compared with the patterns observed in idiopathic VFs. Furthermore, the mechanism and clinical importance of the J waves are discussed. J waves associated with idiopathic VFs showed bradyca

A healthy heart shows intrinsic electrical heterogeneities that play a significant role in cardiac activation and repolarization. However, cardiac diseases may perturb the baseline electrical properties of the healthy cardiac tissue, leading to increased arrhythmic risk and compromised cardiac functions. Moreover, biological variability among patients produces a wide range of clinical symptoms, which complicates the treatment and diagnosis of cardiac diseases. Ischemic heart disease is usually caused by a partial or complete blockage of a coronary artery. The onset of the disease begins with m

Electrophysiological alterations of the myocardium caused by acute ischemia constitute a pro-arrhythmic substrate for the generation of potentially lethal arrhythmias. Experimental evidence has shown that the main components of acute ischemia that induce these electrophysiological alterations are hyperkalemia, hypoxia (or anoxia in complete artery occlusion), and acidosis. However, the influence of each ischemic component on the likelihood of reentry is not completely established. Moreover, the role of the His-Purkinje system (HPS) in the initiation and maintenance of arrhythmias is not comple

Cardiac sarcoidosis is a component of an often multiorgan granulomatous disease of still uncertain cause. It is being recognized with increasing frequency, mainly as the result of heightened awareness and new diagnostic tests, specifically cardiac magnetic resonance imaging and 18F-fluorodeoxyglucose positron emission tomography scans. The purpose of this case-based review is to highlight the potentially life-saving importance of making the early diagnosis of cardiac sarcoidosis using these new tools and to provide a framework for the optimal care of patients with this disease. We will review

The location of the atrioventricular conduction axis in the setting of atrioventricular septal defect has previously been shown by histology and intraoperative recordings. We have now reassessed the arrangement using phase-contrast computed tomography, aiming to provide precise measurements so as to optimize future surgical repairs. We used the system based on an X-ray Talbot grating interferometer using the beamline BL20B2 in a SPring-8 synchrotron radiation facility available in Japan. We analyzed 18 specimens. The atrioventricular node was found within a nodal triangle 1.7 mm from the coron

In the context of chronic atrial fibrillation (AF) ablation, substrate modulation has demonstrated its efficacy and its positive impact both into conversion of the AF either directly to sinus rhythm (SR) or to atrial tachycardia (AT) and into SR maintenance. However, substrate modulation, which can be performed either by linear lesions deployment or/and by targeting complex and fractionated atrial electrograms (CFAEs), can be a difficult task. We present a safe and non time-consuming technique conceived in our center which we think is of interest for substrate modulation purpose in the AF abla

This article addresses electrophysiologic evaluation of the distal AV conduction system in patients with chronic bifascicular block. It was formerly thought that progression to complete AV block was a common cause of morbidity and even sudden death in this condition, and it was hoped that electrophysiologic evaluation, including measurement of the H-V interval, would facilitate prospective identification and prophylactic treatment of patients prone to these complications. However, a decade of clinical investigation has revealed that progression to complete AV block is rare and that although ca

The past 5 years have seen great advances in the knowledge of neural mechanisms of atrial arrhythmogenesis. Direct autonomic nerve recordings demonstrate that simultaneous sympathovagal discharges and intrinsic cardiac nerve activities are common triggers of paroxysmal atrial tachycardia and atrial fibrillation. While activity of the autonomous nervous system (ANS) is crucial in triggering paroxysmal atrial fibrillation, a high incidence of sympathovagal co-activation at baseline is associated with a high vulnerability to pacing-induced sustained atrial fibrillation, suggesting that ANS has a

Few data are currently available regarding the electrophysiological effects of Propafenon administration in patients with pre-existing excito-conduction system disturbances. We have performed electrophysiological evaluation in 18 patients (15 males and 3 females, mean age 61 +/- 9 years - 4 patients with sinus bradycardia, 5 pts with AVN block, 4 patients with bundle branch block and normal HV and 5 patients with bundle branch block and prolonged HV) before and after intravenous administration of Propafenon (1 mg/Kg B.W. in 5 minutes). Propafenone administration decreased the heart rate, signi

BACKGROUND: Understanding the conduction axis location aids in avoiding iatrogenic damage and guiding targeted heart rhythm therapy.
OBJECTIVE: Cardiac structures visible with clinical imaging have been demonstrated to correlate with variability in the conduction system course. We aimed to standardize and assess the reproducibility of predicting the location of the atrioventricular conduction axis by cardiac computed tomography.
METHODS: We evaluated 477 patients with acquired aortic valve disease by cardiac computed tomography to assess variability in cardiac structures established to relat

BACKGROUND: Congenital heart disease (CHD) is the most common severe congenital anomaly worldwide. Diagnosis early in pregnancy is important, but the detection rate by two-dimensional ultrasonography is only 65%-81%.
OBJECTIVES: To evaluate existing data on CHD and noninvasive abdominal fetal electrocardiography (ECG).
SEARCH STRATEGY: A systematic review was performed through a search of the Cochrane Library, PubMed, and Embase for studies published up to April 2016 using the terms "congenital heart disease," "fetal electrocardiogram," and other similar keywords.

INTRODUCTION: Conduction System Pacing (CSP) is a physiologic alternative to right ventricular pacing and may be advantageous in Cardiac Amyloidosis (CA), where conduction disease and septal infiltration are common.
METHODS: A systematic review of 9 published studies, including 7 case reports or small series and 2 retrospective cohorts, identified 59 patients with CA who underwent CSP.
RESULTS: Left bundle branch area pacing was the predominant technique. Implant success was high overall, ranging between 80 and 95%, with acute capture thresholds between 0.5 and 1.0 V at 0.5 ms and stable per

BACKGROUND: Cardiomyopathy and arrhythmias are under significant genetic influence. Here, we studied a family with dilated cardiomyopathy and associated conduction system disease in whom prior clinical cardiac gene panel testing was unrevealing.
METHODS: Whole-genome sequencing and induced pluripotent stem cells were used to examine a family with dilated cardiomyopathy and atrial and ventricular arrhythmias. We also characterized a mouse model with heterozygous and homozygous deletion of Mybphl.
RESULTS: Whole-genome sequencing identified a premature stop codon, R255X, in the MYBPHL gene enc

Involvement of the conduction system in the sudden death of obese young people has not been documented in the literature. We therefore studied the conduction system by serial section examination in 7 subjects, 5 obese and 2 mild to moderately obese, who died suddenly at ages 6, 11, 14, 16, 20, 30, and 32 years of age (5 males, 4 black and 1 white; two females, 1 black and 1 white). Three had a history of sleep apnea. The heart was hypertrophied and enlarged in 6; all 6 had a distinct ventricular septal bulge and epicardial coronary arteries were normal. All had focal mononuclear cells in and a

Heart disease in SSCL may be primary or secondary. Primary involvement includes pericarditis, myocardial fibrosis, and contraction band necrosis with congestive cardiomyopathy, conduction system fibrosis, intramural coronary artery lesions and, rarely, valvular disease. Symptoms include those of left and right ventricular failure, chest pain, palpitations, syncope, and sudden death. Pathogenesis may be related to functional Raynaud's phenomenon of the heart and/or structural small vessel disease. Therapy at present is symptomatic; however, new therapeutic approaches are warranted.

A 30-year-old woman with atrial flutter after surgical correction of tetralogy of Fallot, underwent gynecological procedure under general anesthesia. Because she had been noted to have atrial flutter and heart failure at 8 weeks' gestation, she was scheduled for dilatation and curettage. Chest X-ray film showed cardiomegaly and pulmonary congestive changes. ECG showed atrial flutter with 3:2 atrio-ventricular conduction rate and complete right branch block. She was anesthetized with propofol infused with target-controlled infusion system, fentanyl and 66% of nitrous oxide under close monitorin
