In this article, we present the case of a patient with heart failure with biological aortic valve prosthesis and multiple vascular changes consistent with Takayasu arteritis (TA) who was seen in our department receiving corticosteroids and secondary prevention of rheumatic fever (RF); it was not possible to exclude the association between both diseases.

BACKGROUND: Patient prosthesis mismatch is associated with significant long-term morbidity and mortality after aortic valve replacement, but the role and outcomes of annular enlargement (AE) remain poorly defined. We hypothesized that increasing rates of AE may lead to improved outcomes for patients at risk for severe patient prosthesis mismatch.
METHODS: Patients over age 65 years undergoing surgical aortic valve replacement with or without coronary artery bypass grafting from 2008-2016 in The Society of Thoracic Surgeons Adult Cardiac Surgery Database with matching Centers for Medicare & Me

The main complication after implantation of a Björk-Shiley tilting disc valve in the tricuspid position is late thrombotic obstruction. Of 28 patients with tricuspid valve replacement (16 with mitral, aortic, and tricuspid valve replacement; 12 with mitral and tricuspid valve replacement), with a mean follow-up of 5.2 years, seven (25%) had thrombosis of the tricuspid prosthesis. Three patients had a recurrent thrombotic malfunction, for a total of ten thrombotic malfunctions in 146 patient-years, a rate of 6.8 per 100 patient-years. Thrombosis occurred late in all patients. Clinical deteriora

Changes of the central haemodynamics at rest and during exercise were evaluated in 45 patients who underwent tricuspid valve surgery. Tricuspid valve disease was associated with left heart valvular lesions in 42 patients, while isolated tricuspid valve lesions were present in 3 patients. The pre-operative evaluation showed marked impairment of cardiac function expressed by cardiomegaly, low physical working capacity, hypokinetic central circulation, elevated right atrial pressure and pulmonary hypertension in the majority of patients. Nineteen patients underwent tricuspid valve replacement and

During the period January 1981 to September 1986, 444 Medtronic-Hall heart valve prostheses were implanted in 351 patients (mean age, 45 +/- 10 years) mainly for rheumatic valve disease (63.2%). Most of the patients were in New York Heart Association functional class III. Concomitant surgical procedures, mainly conservative tricuspid or mitral procedures or coronary artery bypass grafting, were performed in 101 patients (28.7%). Single-valve replacement was performed in 262 patients (74.6%) (aortic in 117 patients, mitral in 143, and tricuspid in 2), double-valve replacement in 85 (24.2%) (mit

A 62-year-old woman with severe mitral stenosis and two large parietal thrombi inside the left atrium is described. The patient was admitted to the hospital because of heart failure. Transthoracic and transesophageal echocardiography showed a severe calcific mitral stenosis and two large (56.9 and 46.2 mm in diameter) parietal thrombi inside the left atrium attached to the interatrial septum and associated with severe spontaneous echo contrast. This severe spontaneous echo contrast was detected in the left atrium and in the left atrial appendage. The patient was referred for cardiac surgery. T

An in-depth statistical analysis of early and late results of aortic valve replacement using the Björk-Shiley tilting-disc prosthesis is presented. Our experience with this prosthesis indicates that replacement carries a low surgical risk, a low incidence of complications (embolization, infection, or hemorrhage due to long-term use of anticoagulants), and good long-term survival. Coexisting coronary artery disease increases surgical mortality significantly, and simultaneous, complete revascularization is essential. Patients undergoing isolated aortic valve replacement did significantly better

Pannus formation is a known complication of valve replacement surgery. However, few cases describe pannus formation in the mitral position, and they are mostly associated with prosthetic valves rather than only annuloplasty. We describe the case of a 62-year-old woman who first underwent reductive annuloplasty using a Carpentier-Edwards 28 mm ring prosthesis in 2009 that performed well on regular follow-up for 5 years. In 2014, the patient presented with significant weight gain, shortness of breath (New York Heart Association functional class III) and signs of severe mitral stenosis. Echocardi

Mitral valve repair is the ideal intervention for mitral valve disease with excellent long-term survival comparable to the age-matched general population. When the mitral valve is not repairable, mechanical prostheses may be associated with improved survival as compared with biological prostheses. Newer mechanical and biological valve prostheses have the potential to improve outcomes following mitral valve replacement in young patients. Patients presenting for mitral valve surgery after failed transcatheter mitral valve-in-valve have high rates of postoperative mortality and morbidity, exceedi

AIM: To study the prevalence and structure of postoperative cerebral dysfunction depending on the type and position of the implanted prosthetic heart valve in patients who underwent surgery for the acquired heart valve disease.
MATERIAL AND METHODS: The study included 115 patients (70 men and 45 women; 64 [56; 72] years old), who underwent elective replacement or repair surgery for the acquired heart valve disease.
RESULTS AND CONCLUSION: The postoperative cerebral dysfunction was diagnosed in 40.9% patients, including replacement in the aortic position (45.5%), in the mitral position (55%),
