Value of triple H therapy in a patient with an ischemic penumbra following subarachnoid hemorrhage: a case study.

We report the case of a 43-year-old patient with delayed ischemic neurological deficit and an ischemic penumbra, reversed with triple H therapy (hypertension, hypervolemia, and hemodilution). The patient presented with subarachnoid hemorrhage caused by an aneurysm of the anterior communicating artery. He underwent surgical clipping and developed cerebral ischemia due to vasospasm. Permanent damage to the area of ischemic brain was prevented by institution of the triple H therapy. He recovered and was discharged with no subsequent neurological deficits.

Beating heart aortic valve replacement in a pregnant patient.

A 23-year-old woman with a history of previous bileaflet mechanical aortic valve replacement presented in acute heart failure at 27 weeks of gestation. Transthoracic echocardiography (TEE) demonstrated minimal leaflet excursion consistent with valve thrombosis. The patient underwent emergent surgery with normothermic cardiopulmonary bypass (CPB), and beating-heart technique for valve replacement. This novel strategy for aortic valve replacement is being presented as being particularly useful in pregnant patients whose fetuses are at risks of hypothermia, hemodilution, and hyperkalemia during C…

Clinical biological features of Ballantyne syndrome and the role of placental hydrops.

Ballantyne syndrome was first described in association with severe hydrops fetalis caused by rhesus isoimmunization, and lately, in association with diverse etiologies of nonimmunological severe fetal hydrops. This report is a case of typical Ballantyne syndrome in association with lethal hydrops fetalis caused by Ebstein's anomaly. It is likely that any severe fetal hydrops with massive placental hydrops may produce Ballantyne syndrome. Hemodilution could be the main biological feature, differentiating Ballantyne syndrome from usual preeclamptic syndromes. Pathophysiological hypotheses are di…

Amniodrainage-Induced Circulatory Dysfunction in Women Treated for Twin-To-Twin Transfusion Syndrome.

Twin-to-twin transfusion syndrome (TTTS) in monochorionic-diamniotic twin pregnancies usually requires fetoscopic laser ablation (FLA) followed by amniodrainage (AD). Perioperative maternal hemodynamic changes and hemodilution have been observed. Little is known about the underlying pathophysiology. We aimed to evaluate the impact of high volume amniodrainage on intrauterine pressure, placental thickness and maternal blood characteristics. A total of 18 cases of TTTS were included in this prospective pilot study. All patients were treated with FLA and subsequent AD. Intrauterine pressure and p…

Chronic evaluation of orthotopically implanted bileaflet mechanical aortic valves in adult domestic sheep.

This study was intended to develop a technically feasible and reproducible model for chronic hemodynamic and mechanical evaluation of orthotopically implanted bileaflet mechanical aortic valves in adult domestic sheep. Three adult sheep (mean age 22 weeks, mean weight 76 kg) underwent aortic valve replacement using 19-mm bileaflet aortic valves. Standard cardiopulmonary bypass techniques were followed, including mild hemodilution, systemic hypothermia, and cardioplegic arrest. After performing a left fourth intercostal thoracotomy, the valves were placed using interrupted 3-0 Ticron (Davis + G…

Cardiac surgery-associated acute kidney injury in cardiopulmonary bypass: a focus on sex differences and preventive strategies.

Cardiac surgery-associated acute kidney injury (CSA-AKI) is a high-risk complication with well-recognized increased morbidity and mortality after cardiac surgery attributable in large part to cardiopulmonary bypass (CPB)-associated factors contributing to AKI including hemodilution, hypothermia, hypotension, and exposure to artificial surfaces. These conditions disrupt the renal microcirculation and activate local and systemic inflammatory responses to nonpulsatile flow and low perfusion pressure. The underlying mechanisms of CSA-AKI in CPB are not fully understood, and the incidence of CSA-AK…

Estimation of the Lymph Flow Through Thoracic Duct in Human Subjects Using the Urine Osmolarity: Applicable for Evaluating the Effectiveness of Manual Lymph Drainage.

Currently, there are many methods to evaluate the effectiveness of manual lymph drainage in the treatment of lymphedema, that is, limb volum … ized rabbit significantly accelerated the lymph flow through the thoracic duct, resulting in marked hemodilution. Abdominal respiration in the supine position in human subjects produced similar hemodilution, with a marked decrease in the concentration of vasopressin in the blood. On this basis, we developed a new method to accurately measure the lymph flow thr …

Pathogen reduction of whole blood: Supplementing fibrinogen partly corrects clot formation in a massive transfusion model.

BACKGROUND: The use of whole blood (WB) to treat trauma patients is becoming more common. Similar to the treatment of individual components, pathogen inactivation (PI) technologies are available to treat WB. The impact of PI on WB function is not well understood. This study investigated the impact of PI of WB with riboflavin/ultraviolet (UV) light on its hemostatic function by modeling transfusion scenarios for trauma patients and assessing transfusion efficacy by rotational thromboelastometry (ROTEM). As fibrinogen is affected by PI of WB, the effect of fibrinogen supplementation commonly use

Management and monitoring of anticoagulation for children undergoing cardiopulmonary bypass in cardiac surgery.

Cardiopulmonary bypass (CPB) creates a pro-coagulant state by causing platelet activation and inflammation leading to thrombin generation and platelet dysfunction. It is associated with severe derangements in normal homeostasis resulting in both thrombotic and hemorrhagic complications. This derangement is greater in children with congenital heart disease than in adults because of the immaturity of the coagulation system, hemodilution of coagulation factors, hyperreactive platelets, and in some patients, physiologic changes associated with cyanosis. During CPB, an appropriate amount of heparin…

Conversion from nonischemic to ischemic retinal vein occlusion: prediction by venous velocity on color Doppler imaging.

PURPOSE: To identify color Doppler imaging (CDI) parameters and other prognostic factors of a conversion from nonischemic to ischemic retina … of the minimal central retinal venous velocity (CRV), measured by CDI before and after treatment by hemodilution, diminished the risk of conversion into an ischemic form (p=0.048). For BRVO, an elevated maximal CRV on diagnosis was a protector (p=0.004). Age was associated with a high risk of ischemic evolution for CRVO (p=0.023) but not BRVO. Initial visual a …

A case of delayed loss of facial nerve function after acoustic neuroma surgery.

We report on the uncommon delay of facial palsy in the course of a patient who underwent surgery for a left sided acoustic neuroma. Facial nerve function was preserved without clinical or electrophysiological deficit during the immediate postoperative phase and then lost almost totally over two days. This began on the seventh postoperative day. Recovery became visible under hypervolemic hemodilution and glucocorticoids a few days later and was complete after three months. Vasospasm in combination with neural edema might account for this uncommon delay of postoperative facial nerve deterioratio…

The effect of systemic hypotension during cardiopulmonary bypass on intraocular pressure and visual function in humans.

The effect of controlled systemic hypotension on intraocular pressure (IOP) and visual function was measured in a group of 12 patients undergoing extracorporeal perfusion during cardiovascular surgery. When venous pressure was controlled, intraocular pressure was noted to fall following a fall in the systemic blood pressure. There was a return of IOP toward normal levels with recovery of the systemic blood pressure. Systemic hypotension of 25 to 100 mm Hg for up to 201 minutes in the presence of hemodilution and hypothermia was not associated with any functional or morphologic change in ocular…

Intravascular volume expansion is prominently overestimated via hemoglobin concentration: a placebo-controlled and cross-over trial.

Blood volume (BV) expansion by means of intravenous infusion of colloids is an established procedure in clinical interventions. The essential questions of how much of the infused fluid stays in the circulation according to measured changes in hemoconcentration are uncertain. A placebo-controlled, single-blinded, and cross-over design was implemented to assess the effect of a definite increment in BV on hemodilution, as determined by hemoglobin concentration ([Hb]). Healthy women (n = 17) and men (n = 19) matched by sex, age (age = 26 ± 4 vs. 27 ± 3 yr, P = 0.522), and physical activity were su…

Continuous fluid infusion per rectum compared with intravenous fluid infusion in pigs.

BACKGROUND: Peripheral blood vessels in pigs are not easily accessible, making placement of intravenous catheters difficult. Alternative methods to intravenous administration of fluids, such as administering fluids via the rectum (proctoclysis), are warranted in pigs. HYPOTHESIS: Administration of polyionic crystalloid fluids via proctoclysis results in hemodilution changes similar to intravenous administration. The objectives of this study were to evaluate the tolerance for proctoclysis in pigs and compare analytes before and after intravenous or proctoclysis therapy. ANIMALS: Six healthy, …

Effect of nadir hematocrit during cardiopulmonary bypass on the early outcomes after surgical repair of acute type A aortic dissection.

OBJECTIVE: Although hemodilution during hypothermic cardiopulmonary bypass (CPB) had been thought to improve microcirculation and reduce blood viscosity, there has been no report investigating the effect of low nadir hematocrit (Hct) values caused by severe hemodilution on the surgical outcomes of patients with acute type A aortic dissection (ATAAD). METHODS: We retrospectively reviewed 112 consecutive patients who emergently underwent emergency surgical repair of ATAAD at our institution. The patients were classified into the high Hct (nadir Hct ≥ 21% during CPB; n = 51) and low Hct (nadir H…

Trending, Accuracy, and Precision of Noninvasive Hemoglobin Monitoring During Human Hemorrhage and Fixed Crystalloid Bolus.

Automated critical care systems for en route care will rely heavily on noninvasive continuous monitoring. Association between bipolar episodes and fluid and electrolyte homeostasis: a retrospective longitudinal study.

OBJECTIVES: Imbalance of fluid and electrolyte homeostasis has been suggested to be associated with the neuropathological processes underlying bipolar disorder. However, longitudinal data regarding the association of bipolar episodes with fluid balance are still lacking. We hypothesized that mania may be associated with a relative fluid retention and hemodilution, and depression with a relative hemoconcentration. METHODS: Patients with bipolar disorder (n = 43) admitted to a mental health center, both with depressive and manic episodes, were retrospectively followed between 2005 and 2013. Flu…

The effect of hematocrit during hypothermic cardiopulmonary bypass in infant heart surgery: results from the combined Boston hematocrit trials.

OBJECTIVE: Two randomized trials of hematocrit strategy during hypothermic cardiopulmonary bypass in infant heart surgery have been performe … with higher Psychomotor Development Index scores and reduced lactate levels. Because the effects of hemodilution may vary according to diagnosis, age at operation, bypass variables such as pH strategy and flow rate, and other perioperative factors, this study cannot ascertain a universally "safe" hemodilution level.

[Mirror syndrome: a case report in fetal medicine].

Mirror syndrome is a rare entity describing the association of foetal hydrops and maternal symptoms as general oedema and excessive weight gain mimicking preeclampsia. We report the case of a patient who developed symptoms of oedema, weight gain, headache and biological hemodilution associated with foetal hydrops due to a complex congenital heart defect. This symptomatology spontaneously resolved after foetal expulsion. Mirror or Ballantyne's syndrome needs to be identified on time and well differentiated from preeclampsia. Its consequences may involve the maternal and foetal prognosis.
