BACKGROUND: Infection with the hepatitis C virus (HCV) is a leading cause of chronic liver disease worldwide. Epidemiologic and natural history studies have helped to define the clinical impact of HCV infection, and molecular diagnostic assays have established clinical endpoints against which therapeutic regimens are currently tested. The absence of definitive therapy has generated controversy regarding indications and optimal candidacy for currently approved treatment. This paper reviews the epidemiology, natural history, clinical manifestations, diagnostic modalities, and current treatment o…

Formerly the diagnosis of acute and chronic non-A, non-B hepatitis was made by the exclusion of other causes. However, in 1989 cloning of an antigenic component of the hepatitis C virus (HCV) was reported. This led to first- and second-generation tests for antibody to HCV (anti-HCV) in serum. HCV has been associated with acute and chronic posttransfusion and sporadic non-A, non-B hepatitis, and with hepatocellular carcinoma. Viral HCV RNA can be estimated with the polymerase chain reaction test, but this technically difficult test is not generally available. The entire viral genome has been se…

The viral hepatitis is a serious public health problem worldwide. Some problem is hepatitis B, particularly superinfection HBV-HDV and least hepatitis C (HCV), because they are transmitted via parenteral routes. About 20% of patients becomes a chronic carrier. Some chronic carriers are healthy: and they have no functional deficiencies. Others however, chronic active hepatitis develops and can lead to cirrhosis of the liver and finally to hepatocellular carcinoma, that is one of the major cancers of the world today. The immunocomplexes play a role in pathogenesis of several syndromes, such as: …

Carbamazepine is widely used in the treatment of epilepsy and various neuralgias. Its most common side-effects are leukopenia and skin rash. Hepatic side-effects are rare, but serious and occasionally even fatal. A 72-year-old woman with toxic hepatitis due to carbamazepine is presented. We recommend monitoring liver function tests in every patient receiving this drug.

Chronic viral hepatitis is a common clinical problem; it must be differentiated from other forms of chronic liver disease by history, laboratory data, and liver biopsy. This article reviews the treatment of chronic hepatitis B, delta hepatitis, and nonA and nonB viral hepatitis and emphasizes the controlled trials when available.

In a group of 1,025 cases of alcoholic liver the authors detected 28 cases (2.73%) of acute alcoholic hepatitis on the basis of the morphologic examination, 75% were males, mean age 45 +/- 18 years. The mean amount of alcohol consumed daily was 106.7 g for a mean duration of 19.78 years. The hepatic lesional substrate was hepatic steatosis and cirrhosis in 7 cases (25%) and steatofibrosis in the other 14 cases (50%). The triggering factors were the abuse of alcohol in 27 cases and a pneumonia in the last case. The disease was characterized by fever, encephalopathy, hepatomegaly, hepatocytolyti…

Four primarily healthy patients with granulomatous hepatitis during acute cytomegalovirus infection are described. Granulomatous hepatitis associated with cytomegalovirus infection has been described previously in only 7 patients. As the authors have observed 4 such cases within a period of one year, this disease pattern may occur more frequently than was formerly assumed.

During the last decade an increased incidence of the adverse reactions to drugs with liver involvement was found. The liver lesions produced … rm was the most frequently encountered, raising problems of differential diagnosis with acute viral hepatitis and obstructive jaundice. Clinical evolution of drug induced hepatic lesions is favourable as soon as the administration of drug is arrested. Chronic evolution, as reported in the literature when the administration of hepatotoxic drug is continued, …

Viral hepatitis has long been recognized as a hazard in the health care environment. Nosocomial hepatitis B initially emerged in the setting of transfusion-associated infection and later in patients in dialysis units and on oncology wards. Health care workers are also at risk of acquiring nosocomial hepatitis B and more likely to acquire the infection from their patients than vice versa. Rare instances of nosocomially-transmitted hepatitis A have been documented, but hepatitis A virus excretion patterns in relation to onset of disease generally preclude significant transmission in the hospital…

The evidence relating four clinically distinct rheumatologic syndromes to infection by the hepatitis B virus is reviewed. Acute hepatitis B is not infrequently heralded by a prodromal rash and rheumatoidlike polyarthritis. Chronic active hepatitis B more rarely is associated with transient arthritis or arthralgias. Polyarteritis nodosa may be a manifestation of hepatitis B infection in as many as 40 percent of cases, and recently the syndrome of "essential" mixed cryoglobulinemia has also been linked to infection with this virus. The finding of immune complexes of varying composition, sometime…
