BACKGROUND: Performing successful total hip replacement (THR) in dysplastic, subluxed, and dislocated hip is a challenging task. Here, we assessed midterm clinical and radiological outcomes of uncemented total hip arthroplasty in osteoarthritis (OA) of hip secondary to Hartofilakidis low and high-dislocated hips with a mean follow-up of 8.8 years.
MATERIALS AND METHODS: A retrospective study of prospectively collected data was designed involving all consecutive patients who underwent uncemented THR for OA of hip secondary to developmental dysplasia of the hip and Grade II or Grade III Hartofi…

BACKGROUND: Side-to-side discordance in hip bone mineral density (BMD) is common. We evaluated the associations of discordance in hip BMD with hip osteoarthritis (OA) severity and the risk of progression to total hip replacement (THR) in middle-aged adults.
METHODOLOGY: In 568 participants (aged 47-69 years, 40.3% females) from the Busselton Healthy Ageing Study, including 145 who progressed to THR during a median follow-up of 7.9 years, bilateral hip BMD was measured using dual-energy x-ray absorptiometry (DXA), and Kellgren-Lawrence hip OA grading (0-normal to 4-severe OA) was performed on …

Sports-related injuries of the hip joint and surrounding structures are frequently encountered in athletes, encompassing a wide spectrum of osseous, intra-articular, and extra-articular pathologies. Early and accurate detection of typical injury patterns across various sports can expedite recovery. Delayed diagnosis often leads to prolonged return to play and progression of low grade to more severe injuries. These injuries, caused by repetitive stress or acute trauma, include fractures, hip dislocations, and intra-articular abnormalities, such as femoroacetabular impingement, labral tears, car…

Osteoarthritis is a complex, degenerative disease that can affect all the anatomical structures of the synovial joints. Most frequently, the joints of the hand, hip, and knee are affected, especially in the elderly patients. In our study, we evaluated 27 femoral heads, examining the histopathological changes that occurred in the articular cartilage, subchondral bone, and perisynovial soft tissues. At the level of the articular cartilage, there were observed a reduction in thickness, deformation of the articular surface, degradation of the cartilaginous matrix, the occurrence of fissures or fra…

Postoperative imaging of the hip used to be dominated by radiographs, computed tomography, and occasionally nuclear medicine studies, given that most surgeries were arthroplasties or, less commonly, core decompressions. The indications and procedures performed have expanded well beyond arthroplasties and now include labral procedures (resections, repairs, and reconstructions), osteochondroplasties, acetabuloplasties, and removal of loose bodies, among others. As a result, postoperative evaluation of the hip now often includes MR imaging and MR arthrography. This article discusses normal postop…

During hip replacement, acetabular and femur bone preparation may cause bone marrow extravasation. Paradoxical fat embolism occurs when emboli pass from the pulmonary to the systemic circulation by either a patent foramen ovale or pulmonary capillaries. Impaired consciousness in the immediate postoperative period of orthopedic surgery frequently results from hypovolemia, anemia, or residual anesthesia. A high index of suspicion is required to consider the possibility of paradoxical fat embolism.

In the last 20 years, femoroacetabular impingement has been at the forefront of clinical practice as a cause of hip pain in young adults. As arthroscopic techniques for the hip continue to evolve, the possible presence of a new group of conditions creating mechanical conflict in and around the hip joint (ischiofemoral, subspine and iliopsoas impingement) has recently been elucidated whilst interest in already known 'impingement' syndromes (pelvic-trochanteric and pectineofoveal impingement) is now revived. This article attempts to increase awareness of these relatively uncommon clinical entiti…

We describe the use of intraoperative ultrasound for the safe development of arthroscopic portals during hip arthroscopy without the requirement for fluoroscopy. We find this technique consistently accurate, allowing the safe introduction of arthroscopic instruments into the hip with a very low rate of iatrogenic injury. We have further developed the technique for application to both central- and peripheral-compartment procedures. We now have a total experience of more than 700 procedures to date. With the described technique of ultrasound guidance for portal placement, fluoroscopy is required…

Dancers require extreme ranges of motion in their hips. They require this for many styles and performances. Hip pain and hip injury in dancers can lead to lost work and performance time. There are many potential causes for hip pain in the dancer, including dysplasia, hyperlaxity, both intra- and extra-articular impingement, and soft tissue injuries. This article will review the current literature on these topics in dancers and how they can be applied to the dancer patient.

BACKGROUND: In the literature, the dislocation rate for hip prostheses is stated to be 2-3%. Dislocation may have several causes, such as aspects of the patient, the surgeon, the type of prosthesis or the operation. The objective of this study was to identify the frequency, causes and treatment of dislocation in our department.
MATERIAL AND METHODS: The material encompasses all patients operated on with a hip prosthesis at the Department of Orthopaedics, Haugesund Hospital, from 1987 through 2011. Information has been collected retrospectively from databases and patient records after an avera…

Imaging studies are central to the evaluation of persistent or recurrent symptoms after hip arthroplasty. The evaluation starts with radiographs and may be followed by arthrography, aspiration, scintigraphy, sonography, computed tomography, and MR imaging. Common etiologies of a painful or dysfunctional hip arthroplasty are mechanical loosening, polyethylene wear-induced osteolysis, adverse local tissue reaction to metal wear products, infection, fractures, heterotopic ossification, tendinopathy, and nerve injury. MR imaging with optimized protocols and dedicated techniques for metal artifact …

The imaging evaluation of the prosthetic hip begins with radiography, but arthrography, aspiration, scintigraphy, sonography, CT and MR imaging all have roles in the evaluation of the painful prosthesis. This article will review the appearance of normal hip arthroplasty including hemiarthroplasty, total arthroplasty, and hip resurfacing, as well as the appearances of potential complications such as aseptic loosening and osteolysis, dislocation, infection, periprosthetic fracture, hardware failure, and soft tissue abnormalities.

BACKGROUND: The renewed popularity of resurfacing hip arthroplasty in the last 10 years has generated a remarkable quantity of scientific contributions based on mid- and short-term follow-up. More than one paper has reported a consistent early revision rate as a consequence of biological or biomechanical failure. Two major complications are commonly described with resurfacing implants: avascular necrosis and femoral-neck fracture. A close relationship between these two events has been suggested, but not firmly demonstrated, whereas cementing technique seems to be better understood as potential…
