Against Recommending History Indicated Cerclage.

Cerclage was originally devised by Shirodkar and McDonald for individuals who develop cervical changes in the second trimester. RCT data have confirmed the efficacy of ultrasound-indicated cerclage (UIC). After 70 years of research, the only evidence-based indications for history-indicated cerclage (HIC) are ≥3 early spontaneous preterm births (SPTBs) or early second-trimester losses, and UIC followed by preterm birth <32 weeks. All patients should undergo transvaginal ultrasound cervical length (TVU CL) screening, with UIC considered if the TVU CL shortens before 24 weeks to ≤20 mm in singlet…

History of vestibular schwannoma management.

Eduard Sandifort, was the first to describe a vestibular schwannoma (VS) (acoustic neuroma) at postmortem in 1777. Pirsig, in 1991, described the temporal bone findings in the skeletons of two children buried in an early bronze age cemetery at Franzenhausen in Austria dating back to 2500 BC which are consistent with the bilateral VS of NF2-related schwannomatosis. The first attempts at VS surgery began in the late 19th century with the first successful removal by Annandale in Edinburgh in 1895. Early operative mortality rates were as high as 86%. The suboccipital approach was first popularized

History of clinical intestinal transplantation.

The intestines have been considered the "forbidden organ" for years, and intestinal failure became the last organ failure recognized as such in the medical field. The impossibility of providing adequate nutritional support, turned these patients into recipients of just palliative comfort. In the 1960's, parenteral nutrition appeared as the most reasonable replacement therapy, but the initial success obtained with clinical kidney, heart, liver, lung and pancreas transplantation served as background to explore intestinal transplantation. The first clinical report of an isolated intestinal transp

Natural history of gastric leiomyoma.

BACKGROUND: Most gastric leiomyomas are asymptomatic and benign subepithelial tumors (SETs); however, some may increase in size or become symptomatic. Understanding their natural history is therefore important to their management. We investigated the natural history of histologically proven gastric leiomyomas. METHODS: We retrospectively reviewed histologically proven gastric leiomyoma cases at a tertiary center. The baseline characteristics of these cases were analyzed, and those with a follow-up period of at least 12 months without immediate resection were evaluated. The primary outcome was…

GIGANTISM AND ACROMEGALY THROUGH HISTORY.

Giants and acromegalists have long captured attention, and they were often almost mythical creatures of great strength and power. Due to their expressive appearance, they were recorded in myths and works of art much earlier than they were of interest to medical practitioners. The first recognition was made by a Dutch physician Johannes Wier in 1567, but the first description and appellation originated by Pierre Marie in 1886. Involvement and interest from neurosurgeons followed, and in 1907, Hermann Schloffer performed the first transsphenoidal surgery in a patient with pituitary adenoma. In 1

Progesterone hypersensitivity: a cyclical history.

Hypersensitivity reaction to progesterone is rare, with less than 200 cases reported. It occurs mainly in women in their third decade of life and can have a heterogeneous presentation (cutaneous and/or systemic), with temporal relation to serum levels of progesterone. Diagnosis is based on history and physical examination, supported by skin tests. We describe the case of a woman in her late 20s with pruritic erythema on the chest that manifested in the luteal phase of the menstrual cycle, with spontaneous resolution in less than 5 days, without secondary scarring. The histological diagnosis wa…

Monosexual/Plurisexual: A Concise History.

Monosexuality and bisexuality (attraction to one and more than one gender/sex, respectively) are historical constructs, as are monomodal (e.g., gender/sex-based) and multimodal concepts of erotic attraction. I provide a brief outline of distinctions between single-gender and multi-gender attractions as they emerged in continental Europe. Nineteenth-century conceptualizations of sexual orientation in terms of gender-exclusivity were animated by medical frames for socio-sexual disfavor and aversion. From the early 1880s bisexuality became framed as a stage of "sexual inversion," and, from 1891,

Patients, Disability, Syphilis, and History.

This paper explores the experiences of working-class patients treated for tertiary syphilis at the Neurology Dispensary of the Hospital of the University of Pennsylvania and the Infirmary for Nervous Disease of the Philadelphia Orthopedic Hospital from 1878 to 1917. Using the twin lenses of medical history and disability history, it foregrounds the struggles of individuals whose physical condition cannot be reversed.

Natural history of Myhre syndrome.

BACKGROUND: Myhre syndrome (MS) is a rare genetic disease characterized by skeletal disorders, facial features and joint limitation, caused by a gain of function mutation in SMAD4 gene. The natural history of MS remains incompletely understood. METHODS: We recruited in a longitudinal retrospective study patients with molecular confirmed MS from the French reference center for rare skeletal dysplasia. We described natural history by chaining data from medical reports, clinical data warehouse, medical imaging and photographies. RESULTS: We included 12 patients. The median age was 22 years old …
