Global prevalence estimates of diffuse idiopathic skeletal hyperostosis: a systematic review and meta-analysis.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a whole-body disease characterized by ossification or calcification of joints and ligaments, which is present on all continents and in all ethnic groups. However, there is a lack of comprehensive information on the global prevalence and incidence of DISH. OBJECTIVE: To conduct a systematic review and meta-analysis to investigate the global prevalence of DISH. METHODS: Three electronic medical databases (Cochrane Database of Systematic Reviews, MEDLINE, and Embase) were used to conduct a systematic review of population-based and c…

An Innovative Minimally Invasive Delta Fixation for Thoracolumbar Fracture With Diffuse Idiopathic Skeletal Hyperostosis.

Diffuse idiopathic skeletal hyperostosis (DISH) is a relatively common disease for elderly people, characterized by a tendency of ossification of ligaments and tendons. Spinal fracture in patients with diffuse idiopathic skeletal hyperostosis is a very unstable fracture and usually needs posterior long spinal fixation. Conventional pedicle screw fixation is the standard method for these ankylosed spines. However, with the traditional technique, screw loosening, screw pullout, implant failure, and nonunion are often encountered due to the osteoporotic bone. Recently, transdiscal screw fixation …

Andersson-like lesion in diffuse idiopathic skeletal hyperostosis; a rare diagnostic and therapeutic challenge.

BACKGROUND: Andersson-like lesions (ALL) in diffuse idiopathic skeletal hyperostosis (DISH) are rare and often underrecognized entities that can mimic infective spondylodiscitis or inflammatory lesions such as Andersson lesion (AL) in ankylosing spondylitis (AS). Timely differentiation is crucial to avoid misdiagnosis and inappropriate treatment. CASE PRESENTATION: We report a 57-year-old diabetic male presenting with progressive cervical myelopathy and radiological findings suggestive of a discovertebral lesion at C6-C7 in the setting of DISH. Despite initial suspicion of infection, negative…

Clinical significance of laterality on diffuse idiopathic skeletal hyperostosis with unstable thoracolumbar vertebral fractures.

PURPOSE: This study was conducted to compare clinical characteristics and surgical outcomes based on the laterality of diffuse idiopathic skeletal hyperostosis (DISH) with vertebral fracture (VF). METHODS: We extracted 444 thoracolumbar VF patients in a single center, and 217 patients who underwent balloon kyphoplasty and 159 patients with conservative treatment were excluded. This retrospective study enrolled 68 adult patients who were diagnosed with thoracolumbar VFs that required posterior fusion surgery due to instability. We examined the distribution of DISH on the preoperative whole spi…

Multicompartmental Paranasal Sinus Osteoma Complicated by Frontal Bone Hyperostosis, Intracranial Mucocele, and Inflammatory Pseudotumor.

Parasinusal osteoma complicated by intracranial and orbit extension, cranial vault hyperostosis, intracranial mucocele, and inflammatory pseudotumor is exceptional. A 68-year-old man presented with a long history of progressive proptosis and recurrent episodes of keratoconjunctivitis in the left eye, with restriction in upward gaze. Contrast-enhanced magnetic resonance imaging revealed a frontal sinus lesion extending to the left anterior fossa and orbit, featuring an intracranial cystic component and heterogeneous contrast enhancement. Head computed tomography confirmed the double calcific-cy…

Prognostic factors of balloon kyphoplasty for osteoporotic vertebral fractures with diffuse idiopathic skeletal hyperostosis.

OBJECTIVE: The authors aimed to determine the poor prognostic factors of balloon kyphoplasty for the treatment of fractures of the most distal or distal-adjacent vertebrae in ankylosing spines with diffuse idiopathic skeletal hyperostosis (DISH). METHODS: Eighty-nine patients with fractures of the most distal or distal-adjacent vertebrae of ankylosing spines with DISH were included and divided into two groups: those with (n = 51) and without (n = 38) bone healing 6 months postoperatively. Clinical evaluation included age, sex, time from onset to surgery, the visual analog scale score for low-…

Synovitis, acne, pustulosis, hyperostosis and osteitis syndrome with mandibular involvement: Would surgical operation help?

BACKGROUND: Synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO) syndrome is a rare autoinflammatory disease; its primary manifestation includes osteoarthropathy with skin involvement. Janus kinase (JAK) inhibitors, such as tofacitinib, were used for rheumatoid arthritis; however, due to its downregulation of immune cytokines including interleukin (IL)-6 and IL-8, it might be effective for SAPHO patients. CASE SUMMARY: We report the 1st case of mandibular-related SAPHO syndrome treated with tofacitinib. The patient underwent mandibular resection surgery twice and postoperative patho…

Two Cases of Severe Type 2 Respiratory Failure Associated with Diffuse Idiopathic Skeletal Hyperostosis.

Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory process characterized by hyperostosis at tendon insertions and around joint capsules and ossification of the anterior longitudinal ligament of the spine. The flexibility of the spinal column is reduced in DISH and affects the movement of the thorax, leading to restrictive ventilatory function. In this report, we describe the first two cases of severe type 2 (hypercapnic) respiratory failure associated with DISH. Two older men presented with histories of shortness of breath. Radiography of the spine revealed DISH with coexist…

Surgical treatment of diffuse idiopathic skeletal hyperostosis of cervical spine with dysphagia - Case report.

INTRODUCTION: Diffuse idiopathic skeletal hyperostosis of cervical spine can lead to dysphagia. PRESENTATION OF CASE: A 73-year-old male weighing 110 kg and diagnosed with diffuse idiopathic skeletal hyperostosis in cervical spine with dysphagia. Patient manifested local pain of neck, a gradual limitation of spinal mobility. The surgery decision was based on swallowing problems, not pain in the spine. Before surgery radiographs, magnetic resonance images, computed tomography of the cervical spine and gastroscopy were obtained. Osteophytes were removed from the anterior approach with present o…

Synovitis, Acne, Pustulosis, Hyperostosis, and Osteitis (SAPHO) Syndrome with Destructive Spondylitis: A Case Report.

INTRODUCTION: Spinal lesions in synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome generally have a good prognosis and rarely cause structural destruction or neurological deterioration. We described a surgical case of posterior instrumented surgery without anterior reconstruction and bone graft in a patient with SAPHO syndrome with destructive spondylitis and reviewed the literature on surgical treatment for this entity. CASE REPORT: We describe the case of a 73-year-old male who presented with palmoplantar pustulosis. He experienced progressive low back and leg pain for…

Spinal fractures in patients with diffuse idiopathic skeletal hyperostosis: Clinical characteristics by fracture level.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) makes the spine prone to unstable fractures with neurological deterioration. This study was conducted to assess clinical and radiographic features of spinal fractures in DISH by the level of spinal injury, and to evaluate the optimal treatment for each level. METHODS: A multicenter retrospective study over a 5-year period, including 46 patients (35 males; 11 females) with a mean age of 77.2 ± 9.7 years at the time of injury. By fracture level, there were 7 cervical (15.2%), 25 thoracic (54.3%), and 14 lumbar (30.4%) fractures. We rec…

Diffuse Idiopathic Skeletal Hyperostosis: Persistent Sore Throat and Dysphagia in an Elderly Smoker Male.

Diffuse idiopathic skeletal hyperostosis (DISH) is rarely symptomatic. However, it can present with dyspnea, hoarseness, dysphagia, and stridor. An 80-year-old chronic smoker male presented with 6-month history of sore throat and progressive dysphagia. Computed tomography of the neck revealed bulky anterior bridging syndesmophytes along the anterior aspect of the cervical spine and facet effusion involving four contiguous vertebrae consistent with DISH. Dysphagia secondary to DISH was diagnosed. Fiberoptic laryngoscopy showed bilateral vocal cord paralysis. Patient's airway became compromised …
