We suggest that many severe hypersensitivity reactions during dialysis are IgE mediated (classical anaphylaxis). Besides ETO there must be other substances involved which are not extractable from the dialyzer (e.g. heparin). Some severe reactions are not explained by high IgE levels in the serum. However, in vitro complement activation gives no indication of an anaphylatoxin mediated reaction mechanism, at least in the severe cases of hypersensitivity.

Lymphocyte transformation in response to a saline extract of human pituitary was used to evaluate anti-pituitary immunity in patients with idiopathic hypopituitarism. Nine normal subjects and three children with idiopathic hypopituitarism were studied. Lymphocytes of one patient were significantly reactive to pituitary extract (beyond the 95% tolerance limits of the normal subjects) at five and eleven months after the diagnosis of hypopituitarism. Milk hypersensitivity is a problem that can have substantial health, financial and social implications. The condition is underdiagnosed by some physicians and overdiagnosed by others probably because its symptoms are varied and shared with many other diseases. The gap in our knowledge on the pathogenesis of various manifestations of CMA has been only partly bridged by the information accumulated in recent years. It has become clear that milk-sensitive patients are heterogeneous, both regarding symptomatology and underlying pathogenic mechanisms.

During an 11-year period (1967 through 1977) CDC monitored reactions of hypersensitivity to botulinal antitoxin of equine origin. Of 268 persons given botulinal antitoxin, 24 (9.0 percent) had nonfatal acute (5.3 percent) or delayed (3.7 percent) hypersensitivity reactions to a skin test or therapeutic dose. One hundred eighty-two patients undergoing initial surgical therapy for primary malignant melanoma were evaluated for delayed hypersensitivity using a battery of recall antigens prior to surgery. HLA antigen status was assessed in non-related individuals and related family members with bird breeder's hypersensitivity pneumonitis (HP). 35% of 20 non-related patients carried HLABW40 compared to 10% of 861 normal controls (p less than 0.001, Pc less than 0.05, respectively). A consistent association of an HLA haplotype with disease was demonstrated in three of four families. Selected HLA antisera contained antibodies which blocked the blastogenic response to bird antigen. This study suggests the existence of an HLA associated Ir gene in bird breeder's HP.

Guinea pigs primed for cutaneous basophil hypersensitivity (CBH) with several soluble proteins or with sheep erythrocytes developed a systemic, delayed-onset, maculopapular rash when challenged parenterally with specific antigen. Delayed hypersensitivity reactions were investigated in 5 patients with febrile acne conglobata. Each of them had febrile periods, large abscesses and leukocytosis. They reacted negatively to tuberculin 10 TU/ml, to Schick toxin, to oidiomycin, to trichophytin as well as to 25 common contact allergens. The capacity of peritoneal macrophages to spread was studied with cells of mice infected with Listeria monocytogenes and with cells of guinea pigs sensitized with BCG (bacille Calmette Guérin) vaccine or immunized with ovalbumin. In macrophages taken from animals having delayed hypersensitivity, this ability was markedly decreased by the presence of specific antigen for less than 1 hour. Such an effect was not observed in guinea pigs having only circulating antibodies.
