Immune checkpoint inhibitors are increasingly associated with complex immune-related endocrine adverse events that may evolve dynamically over time. We report a rare case of pembrolizumab-induced hypophysitis complicated by central diabetes insipidus, which was unmasked following glucocorticoid replacement. A 68-year-old female patient receiving immunotherapy for lung adenocarcinoma presented with hyponatremia and secondary adrenal insufficiency, followed by abrupt severe hypernatremia after steroid initiation. Subsequent evaluation confirmed central diabetes insipidus, and treatment with desm

Immune checkpoint inhibitors (ICIs) have revolutionized cancer treatment, particularly in advanced esophageal squamous cell carcinoma (ESCC), where nivolumab has demonstrated significant survival benefits. However, these therapies may precipitate immune-related adverse events (irAEs), including endocrine disorders such as hypophysitis. While hypophysitis is more commonly associated with anti-cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) agents, its occurrence following anti-programmed cell death protein 1 (PD-1) inhibitors like nivolumab remains rare and diagnostically challenging due t

The immune landscape plays an important role in various pituitary diseases, ranging from hypophysitis to pituitary neuroendocrine tumors. Moreover, the use of immune checkpoint inhibitors (ICIs) has dramatically altered the landscape of cancer treatment by improving prognosis and overall survival in a multitude of advanced-staged malignancies, though their use in pituitary neuroendocrine tumors has remained limited. In this review, we will focus on selected topics to highlight the impact of the immune microenvironment on the function of the pituitary gland, namely, animal models of autoimmune

CONTEXT: The occurrence of multiple endocrinopathies due to immune checkpoint inhibitors (ICIs) is a relatively common adverse event. However, the occurrence of a combination of hypophysitis and type 1 diabetes mellitus (T1DM) is extremely rare, and its clinical features are unclear.
OBJECTIVE: We comparatively analyzed the clinical features of this combination and each individual ICI-induced endocrinopathy.
METHODS: We reported 3 cases that we encountered and reviewed previously reported cases of patients with combined hypophysitis and T1DM due to ICIs.
RESULTS: Anti-programmed cell death-

BACKGROUND: Immune checkpoint inhibitors (ICI) are clinically active across multiple tumor types but the associated immune-related adverse events (irAEs) lead to treatment delays or discontinuation and negatively impact quality-of-life. Hypophysitis is often a permanent irAE that may affect multiple pituitary hormonal axes. Here we comprehensively characterize our institution's clinical experience with ICI-induced hypophysitis and the associated patterns of pituitary function loss.
METHODS: Patients with solid tumors, mostly melanoma and renal cell carcinoma (RCC), treated with ICI at Yale Ca

Immune checkpoint inhibitors (ICIs) can cause pituitary dysfunction due to hypophysitis. We aimed to characterize ICI-induced hypophysitis and examine its association with overall survival in this single-center retrospective cohort study of adult patients with cancer who received an ICI from January 1, 2012 through December 31, 2016. A total of 896 patients were identified who received ipilimumab alone (n=120); ipilimumab and nivolumab (n=50); ipilimumab before or after pembrolizumab (n=70); pembrolizumab alone (n=406); and nivolumab alone (n=250). Twenty-six patients (2.9%) developed hypophys

Primary hypophysitis (PH) is a rare autoimmune inflammatory disease of the pituitary gland. The aim of the study was to evaluate clinical characteristics, disease management, and outcomes of cases with PH. Medical records of PH patients admitted to Hacettepe University Hospital between 1999 and 2017 were analyzed retrospectively. Paraffin-embedded pathology blocks were obtained for both re-examination and IgG4 immunostaining. Twenty PH patients (15 females, 5 males) were evaluated. Mean age at diagnosis was 41.5±13.4 years. Some form of hormonal disorder was present in 63.2% of cases, hypogona

BACKGROUND: Immunoglobulin G4-related hypophysitis (IgG4-RH) is a rare disease, diagnosis of which typically depends on histopathology following an invasive pituitary biopsy, possibly leading to permanent hypopituitarism. Herein, we report two cases of IgG4-RH with favorable responses to glucocorticoids. One of them was multiple organs involved and treated with glucocorticoids and methotrexate.
METHODS: We retrospectively review clinical features, radiological images, and treatment of two cases with IgG4-RH. In addition, literature on IgG4-RH was comprehensively reviewed and a new therapeutic

OBJECTIVE: Clinical data on primary hypophysitis are still scarce. Especially non-surgical cases are underreported. We sought to analyse clinical characteristics of primary hypophysitis, particularly in clinically diagnosed patients.
DESIGN: Retrospective single centre study in 60 patients with primary hypophysitis.
METHODS: Symptoms, MRI, histopathological findings, treatment and outcomes were analysed in 12 histopathologically and 48 clinically diagnosed patients. Diagnostic criteria for clinical diagnosis were: a) MRI findings compatible with primary hypophysitis; b) course of disease exc

BACKGROUND: Autoimmune hypophysitis (AH) is a primary autoimmune inflammatory disorder of the pituitary gland, which usually presents as a mass in the sella turcica. Systemic lupus erythematosus (SLE) is another inflammatory disorder in which the immune system attacks healthy cells and tissues throughout the body. Although both diseases are autoimmune disorders, they rarely coexist, and the relationship between them is unclear.
CASE REPORT: A 66-year-old man was evaluated at the endocrinology clinic because of worsening fatigue, anorexia, drowsiness, and leg oedema. Examination revealed alert

Rhesus macaques experimentally infected with Simian Immunodeficiency Virus (SIV) experience immunosuppression and often opportunistic infection. Among the most common opportunistic infections are rhesus cytomegalovirus (RhCMV), a ubiquitous betaherpesvirus that undergoes continuous low-level replication in immunocompetent monkeys. Upon SIV-mediated immunodeficiency, RhCMV reactivates and results in lesions in numerous organ systems including the nervous and reproductive systems. We report the first case of cytomegaloviral hypophysitis in a SIV-immunocompromised rhesus macaque.

A 70-year old man with a 14 year history of Sjögren syndrome, interstitial pneumonia, and autoimmune hepatitis (AIH) was admitted to our hospital due to hyponatremia with a one month history of fatigue, thirst, and nausea. Laboratory tests on admission revealed that this patient had a central adrenal insufficiency. Pituitary magnetic resonance imaging (MRI) further showed swelling of the stalk and posterior lobe of his pituitary, suggesting infundibulo-hypophysitis. Based on his past history of autoimmune disease, his serum IgG4 levels were measured and found to be remarkably high (924 mg/ dL)

Autoimmune polyendocrine syndromes (APS) are organ-specific autoimmune disorders affecting multiple endocrine glands; these are gradually destroyed by action of autoantibodies. Similarly to other autoimmune diseases, the presence of certain genetic predisposition is an essential prerequisite to the disease development; polymorphism of the main histocompatible system (HLA in humans) appears to play the most important role. APS are categorized into four types, based on what combination of endocrine glands is affected. APS type 1, characterised by hypoparathyreosis, mucocutaneous candidiasis and

We report a case of secondary hypophysitis caused by rupture of Rathke's cleft cyst. A 30-year-old woman was admitted to our hospital with complaints of polyuria and left visual acuity impairment. These symptoms were preceded by aseptic meningitis one month prior to admission. An MRI on admission showed a suprasellar cystic mass and a swelling pituitary stalk. The mass was partially resected via the transsphenoidal approach. Pathological diagnosis was adeno- and neuro-hypophysitis and ruptured Rathke's cleft cyst. These findings strongly suggested that the hypophysitis was caused by rupture of

6 years after surgery for epidermoid lung cancer, a 58-year-old man presented with atopic dermatitis and a high level of IgE, primary hypothyroidism of autoimmune origin, slight hyperprolactinemia and adrenal insufficiency of pituitary origin. ACTH levels were low and unresponsive to CRH. There was no deficiency of the other pituitary hormones and the MRI of the pituitary was normal. The other instances of autoimmune thyroiditis associated with ACTH deficiency are reviewed. The etiology of the deficiency is discussed in the context of a possible hypophysitis.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have emerged as immunotherapy treatment modalities for several advanced malignancies. Although effective in cancer treatment, inhibition of cellular checkpoints can result in the loss of immunologic tolerance with consequent autoimmune disorders. Clinicians must be aware of immune-related adverse events (irAEs), such as hypophysitis, to ensure prompt treatment.
CASE PRESENTATION: A 73-year-old male with a history of metastatic melanoma who had received 5 cycles of pembrolizumab (anti-PD-1) and 2 cycles of combination ICI therapy with ipilimumab

Granulomatous hypophysitis is a rare form of pituitary inflammation that closely mimics pituitary macroadenoma in clinical presentation as well as radiological appearance. Histopathology remains the gold standard diagnostic modality. Here, we present the case of a young female who presented with symptoms of anterior and posterior pituitary dysfunction, including secondary amenorrhea, cold intolerance, polyuria, and polydipsia. Magnetic resonance imaging revealed a sellar mass with suprasellar extension, suggesting a pituitary macroadenoma. Histopathological examination of the transsphenoidal b
