Four of 18 patients with common variable immunodeficiency and one of eight patients with selective IgA deficiency had increased proportions of T epsilon cells. Three patients with common variable immunodeficiency and two with selective IgA deficiency had increased numbers of T epsilon cells. Three patients with common variable immunodeficiency had decreased numbers of T epsilon cells. Five of six patients with hyperimmunoglobulinaemia E had high proportions of T epsilon cells; this increase was not related to the elevated IgE levels.

Dedicator of cytokinesis 8 (DOCK8) mutations lead to a primary immunodeficiency associated with recurrent gastrointestinal infections and poor antibody responses but, paradoxically, heightened IgE to food antigens, suggesting that DOCK8 is central to immune homeostasis in the gut. The aim of this study was to screen a group of children with Down syndrome (DS) for celiac disease, and to define future strategies for screening the patients followed at our center. In three patients with juvenile rheumatoid arthritis, serum IgA concentrations were within the normal limit at the onset of disease and before aspirin administration. A 20-month-old Indian boy presented with recurrent pyogenic infections and failure to thrive. His IgG and IgA levels were low, but his IgM was elevated. He also had undetectable isohaemagglutinin titre and neutropenia, both parameters being poor prognostic indicators in this very rare primary immunodeficiency state--antibody deficiency with hyper IgM. Our patient subsequently succumbed to Pseudomonas aeruginosa septicaemia and meningitis inspite of aggressive antibiotic and intravenous gammaglobulin therapy.
