Babesiosis in immunosuppressed hosts: pathogenesis, diagnosis and management.

PURPOSE OF REVIEW: This review provides the most recent evidence of the challenges that occur in the management of babesiosis in immunocompromised hosts. RECENT FINDINGS: The epidemiology of babesiosis is affected by climate change leading to increasing numbers of cases as well as increasing areas of endemicity. Immunosuppressed hosts, especially with asplenia or B-cell defects, are at high risk of having severe disease as well as persistent and relapsed infection. Resistance to the primary therapies azithromycin and atovaquone can develop leading to further challenges in treating persistent …

Strongyloides hyperinfection in a patient with solid malignant tumor a case report.

Strongyloides stercoralis is unique among the nematodes, in which it completes its life cycle inside a single human host by causing autoinfection in the host, and it causes hyperinfection leading to persistent and fatal disseminated infections in immunocompromised hosts. The present case report is about strongyloidiasis fatal hyperinfection in a patient with malignant tumor of the tongue on radiotherapy treatment, to highlight the need for clinical suspicion of strongyloidiasis in an immunocompromised host. As per the Centers for Disease Control and Prevention, the mortality in strongyloides h…

Genetic and immunologic determinants of BCG disease: from mechanism to prevention.

BACKGROUND: Bacillus Calmette-Guérin (BCG) infection, a disseminated infection triggered in immunocompromised hosts by the administration of the attenuated BCG vaccine, is characterized by a pathogenic mechanism that involves the synergistic interaction between host immune deficiencies and strain-specific genetic mutations. METHODS: This review synthesizes published multi-omics and proteomics findings from the past decade, along with reported results from in vitro drug susceptibility assays and molecular diagnostics, to explore key pathogenic genes in BCG strains. We compare differences in pa…

Variation in Candida albicans EFG1 expression enables host-dependent changes in colonizing fungal populations.

UNLABELLED: To understand differences in host-Candida albicans interactions that occur during colonization of healthy or compromised hosts, … in gene expression in colonizing C. albicans cells. Altered gene expression in organisms colonizing immunocompromised hosts may begin the transition of C. albicans from a commensal to a pathogen. IMPORTANCE: In healthy people, the fungus Candida albicans colonizes the gastrointestinal tract and other sites without producing obvious pathology. In an immunocompromised pa …

Pathogenesis of acute respiratory illness caused by human parainfluenza viruses.

Human parainfluenza viruses (HPIVs) are a common cause of acute respiratory illness throughout life. Infants, children, and the immunocompromised are the most likely to develop severe disease. HPIV1 and HPIV2 are best known to cause croup while HPIV3 is a common cause of bronchiolitis and pneumonia. HPIVs replicate productively in respiratory epithelial cells and do not spread systemically unless the host is severely immunocompromised. Molecular studies have delineated how HPIVs evade and block cellular innate immune responses to permit efficient replication, local spread, and host-to-host tra

SARS-CoV-2 variants resistant to monoclonal antibodies in immunocompromised patients constitute a public health concern.

COVID-19 in immunocompromised hosts has emerged as a difficult therapeutic management problem. Immunocompromised hosts mount weak responses to SARS-CoV-2 and manifest infection outcomes ranging from severe disease to persistent infection. Weakened immune systems mean greater viral loads and increased opportunities for viral evolution. Gupta, Konnova, et al. report the emergence of resistant SARS-CoV-2 variants in immunocompromised patients after monoclonal antibody (mAb) therapy. mAbs target only a single determinant in the viral Spike protein, which is a weakness of such therapy when treating…

Cytomegalovirus induced hepatitis in an immunocompetent host.

Cytomegalovirus (CMV) infection is most commonly sub clinical. In the immunocompromised host, primary CMV infection, reactivation and re-infection are all associated with significant morbidity and mortality. The clinical presentation of CMV infection in immunocompromised host varies according to the degree of immunocompromisation and to host factors. Populations at greatest risk of CMV infection and disease include individuals with HIV infection, transplant recipients, cancer patients receiving chemotherapy and burn patients. In the immunocompetent adult, primary CMV infection is usually asymp…

Vaccination of immunosuppressed patients.

Chronically ill patients with a weak immune system are at increased risk of severe infections, often as a result of immunomodulatory therapy rather than the underlying disease. Many patients in this group exhibit reduced vaccine responses and require tailored vaccination regimens. Following the launch of the national Adult Immunisation Programme, all doctors must stay abreast of current guidance on vaccination in immunosuppressed patients. This clinical review article summarises interdisciplinary knowledge on patients with a reduced vaccine response.

An Atypical Case of Exophiala Bergeri Chromoblastomycosis in an Immunosuppromised Individual.

Chromoblastomycosis (CBM) is a rare chronic, granulomatous mycosis affecting the skin and subcutaneous tissue. It is a deep, slow growing, fungal infection that most often presents with epidermal changes that clinically appear as a hypertrophic and verrucous plaque. We describe an atypical presentation of an immunocompromised male patient, age 76 years, who presented with a 4-month history of painless subcutaneous firm nodules of the left fifth finger. The fungal culture grew Exophiala bergeri, and histopathology showed medlar bodies with lack of pseuodepitheliomatous hyperplasia or epidermal

[Aspergillus and the lung].

Aspergillus is a ubiquitary mold, with whom the human has often contact. Normally Aspergillus is not a pathogen for the immunocompetent subject, but it can be source of infections in the immunocompromised patient or in the persons with structural pneumological pathologies. In other subjects it can elicit allergic reactions or immunological response. These different pathologies are various and often difficult to diagnose. We propose here a view of these different pathologies.

Chagas disease in immunocompromised patients.

SUMMARYAs Chagas disease remains prevalent in the Americas, it is important that healthcare professionals and researchers are aware of the screening, diagnosis, monitoring, and treatment recommendations for the populations of patients they care for and study. Management of Trypanosoma cruzi infection in immunocompromised hosts is challenging, particularly because, regardless of antitrypanosomal treatment status, immunocompromised patients with Chagas disease are at risk for T. cruzi reactivation, which can be lethal. Evidence-based practices to prevent and manage T. cruzi reactivation vary dep…

Cryptococcosis.

Cryptococcosis is an invasive fungal infection caused by yeasts of the genus Cryptococcus that causes a significant global burden of disease in both immunocompromised and immunocompetent individuals. Over the past several decades, diagnosis and management of cryptococcal disease have moved to focus on rapid, reliable, and cost-effective care delivery, with the advent of new antigen detection assays and novel antifungal treatment strategies.

Dematiaceous Molds.

Dematiaceous molds are darkly pigmented environmental molds found worldwide, especially prevalent in tropical and subtropical regions. Common genera include Bipolaris, Cladophialophora, Exophiala, and Alternaria. They cause disease in both immunocompetent and immunocompromised individuals, presenting as cutaneous infections, allergic sinusitis, pneumonia, and, rarely, disseminated infections. Contaminated medical products have also led to fungal meningitis outbreaks. Treatment typically involves itraconazole, voriconazole, or posaconazole, along with source control and reduction in immunosuppr

[A case of disseminated nocardiosis in a non-immunosuppressed host].

Nocardia is an opportunistic pathogen that most commonly affects immunosuppressed hosts, while disseminated infections in healthy hosts are rare. In this paper, we described a case of disseminated Nocardia infection in a non-immunosuppressed host whose initial imaging was strikingly similar to systemic metastasis from a tumor. Since no tumor cells were found in any of the numerous biopsies taken, we considered infection by specific pathogens before metagenomic next-generation sequencing of a lumbar spine tissue biopsy finally confirmed the diagnosis of Nocardia infection. To help doctors bette

Leuconostoc, a masquerading pathogen in oral cancer patient: A rare case report.

Osteoradionecrosis is the most common cause of mandibular osteomyelitis. Here we are reporting a case of osteomyelitis of the mandible caused by Leuconostoc mesenteroides spp cremoris. The frequency of Leuconostoc infections are under reported mainly due to its difficult identification. Non pathogenic organism like leuconostoc can cause infection in the immunocompromised and should warrant proper microbial identification and adequate targeted treatment to manage the patients.

Harris' hawk (Parabuteo unicinctus) as a source of pathogenic human yeasts: a potential risk to human health.

Aim: Invasive human fungal infections have been a serious public health problem among immunocompromised patients. Wild bird species are related to the eco-epidemiology of some infectious diseases, mainly Cryptococcosis, Histoplasmosis, Aspergillosis, Chlamydiosis, Salmonellosis and allergic diseases. Falconry is the art of training predators for hunting. Nowadays, birds of prey are used as pets, which brings new sources of infections to humans. Materials & methods: We identified fungal pathogenic yeasts, Candida parapsilosis, Debaryomyces hansenii and Rhodotorula mucilaginosa. Conclusion: Stud
