Advancing patient-centered care: moving from outcome-based to risk factor-based models using the big four risk factors.

This article reimagines the health care system to focus on risk factors rather than outcomes in order to improve patient-centered care and reduce health care expenditure. Patient-centered care has been a global priority since 2001 when the Institute of Medicine declared it an essential aim for health care systems. As part of this discussion and to help facilitate this change, the concept of the big four risk factors - diet and nutrition; physical activity; smoking and tobacco use; and excessive alcohol consumption - is introduced in the context of the Americas from which it originates. Reproducibility in biomedical research.

Reproducibility has long been considered a foundation of trustworthy science, yet practices that enable reproducible research remain inconsistently implemented, despite widespread awareness and establishment of policies and guidelines. Biomedical research that cannot be reproduced can lead to resource waste and adversely impact patient safety, public health outcomes, scientific progress and public trust. Rapid advances in technology, scientific dissemination and clinical translation have prompted renewed emphasis on validating research methods and findings to ensure transparency, rigor and cre

Strategies to Increase Equitable Access to Penicillin Allergy Delabeling Interactions: A Qualitative Study.

IMPORTANCE: Patients with unconfirmed and potentially false penicillin allergy labels receive unnecessarily broad-spectrum antibiotics that can lead to worse health outcomes and antimicrobial resistance. While ambulatory care is a favorable setting to engage in penicillin allergy delabeling, the process of removing an incorrect penicillin allergy label has not been widely implemented. OBJECTIVE: To contextualize barriers to and facilitators of penicillin allergy delabeling from a diverse range of perspectives and inform actionable strategies. DESIGN, SETTING, AND PARTICIPANTS: This qualitati

Using culturally responsive and sustaining course design to increase retention of underrepresented STEM undergraduates.

Undergraduate science courses are in urgent need of increased retention and recruitment of underrepresented minority (URM) college students. There is a significant gap in current approaches to curricular design and their efficacy for URM student retention. To address this, we propose increased incorporation of culturally responsive and sustaining education (CRSE) methodology in science, technology, engineering, and mathematics (STEM) curricula design. We identify well-documented and effective strategies, emphasizing practices that value students' prior knowledge and relate course content to st

Precision medicine for epilepsy: challenges and perspectives for an optimized clinical care pathway.

Epilepsies are a common, but heterogeneous group of brain disorders, characterized by an enduring predisposition to recurrent epileptic seizures. Recognizing epilepsies as a disease spectrum offers compelling opportunities to implement precision medicine in routine care. In this narrative review, we assess the status and development of precision epilepsy, compare its implementation with the advanced model of precision oncology, and discuss strategies to advance the implementation of precision medicine in epilepsy care. We aim to raise awareness about the current state-of-the-art approaches in

Designing a stigma reduction intervention in Zambia: perspectives of young people with HIV and key stakeholders using the consolidated framework for implementation research.

BACKGROUND: Young people with HIV (YPWH) in Zambia and other low-income settings in Africa experience the compounded effects of intersecting HIV and poverty stigma. Meanwhile, limited research using structured implementation science frameworks has informed the development of interventions to address this intersectional stigma. We sought to address this gap by applying the Consolidated Framework for Implementation Research (CFIR) to explore perspectives of YPWH and key stakeholders (service providers, local government representatives) on the design and implementation of interventions to reduce

Implementation strategies for evidence-based healthcare interventions in rural and remote settings: a scoping review.

Globally, people living in rural and remote areas experience poorer healthcare access and outcomes than urban populations. Applying implementation strategies that support the translation of evidence-based healthcare interventions may help reduce these inequities; however, real-world implementation is complex, and it remains unclear how strategies are applied and tailored to rural and remote contexts. This scoping review synthesised evidence on implementation strategies for healthcare interventions in rural and remote settings of high-income countries. Five databases (Ovid MEDLINE, Embase, Coch
