The main cause of anaerobic pulmonary infections is aspiration of saliva, upper airway secretions of gastric content. Predisposing conditions include prominent dental disease, chronic upper respiratory tract infections and reduced consciousness. Fusobacterium nucleatum, Bacteroides melaninogenicus and anaerobic Gram-positive cocci are the most frequently encountered organisms. The clinical presentations are lung abscess, lung gangrene and empyema, which all tend to take a slow and indolent course. Preferred localization are dependent lung segments, most often of the right side. For bacteriolog…

The most important viral organisms involving the eye are the DNA viruses herpes simplex, varicella-zoster, cytomegalovirus, adenovirus, and … mune reactions in the eyes, but patients on these drugs should be monitored carefully both for superinfections and for interference with tissue healing that might ultimately threaten the integrity of the globe.

Severe necrotizing anaerobic infections carry a high mortality. From a retrospective review of 17 cases of gas gangrene, necrotizing fasciitis and nonclostridial myonecrosis seen at the Victoria and University hospitals in London, Ontario, between Jan. 1, 1975 and Dec. 31, 1977, the most common antecedent was found to be minor trauma. Location of the infection varied; the scrotum and perineum were most commonly involved. Local findings were most frequently edema, skin tenderness and induration. Mixed infections predominated with a pure growth of Clostridium sp. occurring in only three cases. T…

Since corticosteroids were introduced, a steadily increasing number of opportunistic pathogens have been causing major disease in both systemically and locally compromised hosts. In ocular disease, the most common cause of compromise (and of infection with such opportunistic pathogens as the herpeviruses, many gram-negative bacteria, numerous fungi and Toxoplasma) is the topical use of corticosteroids and antibiotic-corticosteroid preparations. To avoid the damaging and sometimes blinding results of opportunistic infection, the use of these preparations should be carefully restricted.

Most serious neonatal streptococcal infections are caused by group-B streptococci. The pattern of serious group-B neonatal disease in Britain resembles that described in other countries; both "early-onset" and "late-onset" forms are seen, but reliable incidence rates have not yet been determined. Serological-type III strains predominate in neonatal meningitis in Britain, but not so markedly as in some parts of the U.S.A. A deficiency of group-II strains in meningitis is, however, apparent in both countries. Present information about the carriage of group-B streptococci suggests that antibiotic…
