Autologous Mesenchymal Stem Cells Increase Cortical Perfusion in Renovascular Disease.

Atherosclerotic renovascular disease (RVD) reduces renal blood flow (RBF) and GFR and accelerates poststenotic kidney (STK) tissue injury. Preclinical studies indicate that mesenchymal stem cells (MSCs) can stimulate angiogenesis and modify immune function in experimental RVD. We assessed the safety and efficacy of adding intra-arterial autologous adipose-derived MSCs into STK to standardized medical treatment in human subjects without revascularization. The intervention group (n=14) received a single infusion of MSC (1.0 × 105 or 2.5 × 105 cells/kg; n=7 each) plus standardized medical treatme

Sevoflurane with opioid or dexmedetomidine infusions in dogs undergoing intracranial surgery: a retrospective observational study.

This study reports the clinical use of two sevoflurane-based anesthetic techniques in dogs undergoing craniectomy. Twenty-one animals undergoing elective rostrotentorial or transfrontal craniectomy for brain tumor excision, anesthetized with sevoflurane, were enrolled in this retrospective, observational study. Anesthetic records were allocated to two groups: Sevo-Op (sevoflurane and short acting opioid infusion): 8 dogs and Sevo-Dex (sevoflurane and dexmedetomidine infusion): 13 dogs. Average mean arterial pressure (MAP), heart rate, end-tidal carbon dioxide, end-tidal sevoflurane and intraop

The use of streptokinase in replant salvage.

This is the first report of the clinical use of direct intra-arterial streptokinase infusion to promote the salvage of a failing thumb replant. Anastomotic revision was performed on two occasions following thrombosis in the vein grafts used to replant an avulsed thumb. Recurrent thrombosis was treated successfully with conservative thrombolytic therapy consisting of a direct infusion of streptokinase, 20,000 I.U. per hour into the radial artery for 32 hours, followed by intravenous heparin, 40,000 I.U. for 24 hours. The thumb survived, with no tissue loss. Thrombolytic therapy is the only non-

Free flap salvage via direct urokinase injection into thrombosed veins: a case report.

Intraoperative use of urokinase is a recognized method for salvaging compromised free flaps. However, protocols for dosage and administration vary, and no consensus exists regarding the optimal technique. Herein, we report a case of postoperative venous thrombosis in a free fibular flap. Despite the unsuccessful intra-arterial administration of urokinase owing to an extensive venous thrombus, we attempted to dissolve the thrombus through direct intravenous infusion using a 27 G needle at multiple sites in the vein where the thrombus had formed. Ten minutes after direct injection into the venou

Human placental lactogen decreases regional blood flow in anesthetized pigs.

In 22 pigs anesthetized with sodium pentobarbitone, changes in blood flow caused by infusion of human placental lactogen into the left renal, external iliac, and anterior descending coronary arteries were assessed using electromagnetic flowmeters. In 17 pigs, infusion of human placental lactogen whilst keeping the heart rate and arterial pressure constant decreased coronary, renal and iliac flow. In 5 additional pigs, increasing the dose of human placental lactogen produced a dose-related decrease in regional blood flow. The mechanisms of the above response were studied in 15 of the 17 pigs by

[Selective intra-arterial thrombolysis in acute limb ischemia].

Despite significant advances in modern treatment of patients with peripheral artery disease, acute limb ischemia remains highly prevalent, limb-threatening, and potentially fatal, continuing to pose a challenge to vascular surgeons. Currently, there are various approaches to managing patients with acute limb ischemia. Selective intra-arterial thrombolysis is an acceptable alternative to surgical treatment. To date, the main indications, advantages and complications of thrombolysis have been identified. Searching for prognostic risk factors for the development of hemorrhagic complications and t

Safety and Efficacy of Adjunctive Heparin Infusion During Catheter-Directed Thrombolysis for Acute Limb Ischemia.

Background Acute limb ischemia (ALI) requires urgent revascularization to prevent limb loss and mortality. Catheter-directed thrombolysis (CDT) is an established treatment; however, the role of concomitant intra-arterial heparin infusion during CDT remains unclear. Current guidelines discourage routine therapeutic heparinization, yet clinical practice varies. This study evaluated the efficacy and safety of adjunctive heparin infusion during CDT for ALI. Methodology A retrospective single-center study was conducted, including all patients who underwent CDT for ALI between November 2022 and May

Morphine is an arteriolar vasodilator in man.

AIM: The mechanisms of action of morphine on the arterial system are not well understood. The aim was to report forearm vascular responses, and their mediation, to intra-arterial morphine in healthy subjects. METHODS: Three separate protocols were performed: (i) dose ranging; (ii) acute tolerance; (iii) randomized crossover mechanistic study on forearm blood flow (FBF) responses to intrabrachial infusion of morphine using venous occlusion plethysmography. Morphine was infused either alone (study 1 and 2), or with an antagonist: naloxone, combined histamine-1 and histamine-2 receptor blockade

Regional nerve injury after intra-arterial chemotherapy.

Eleven patients at M.D. Anderson Hospital were referred for neurologic evaluation after having their internal or external iliac arteries catheterized for the treatment of localized pelvic or lower extremity tumors. Nine patients developed lumbosacral plexopathies and two patients, mononeuropathies. All symptoms occurred within 48 hours of the intra-arterial infusion. All patients received cis-dichlorodiammine-platinum (cisplatin; CDDP) intra-arterially, alone or in combination with other agents. Follow-up examinations revealed that only one patient had made partial recovery from the neurologic

Hemodynamic and Anatomic Variations Require an Adaptable Approach during Intra-Arterial Chemotherapy for Intraocular Retinoblastoma: Alternative Routes, Strategies, and Follow-Up.

BACKGROUND AND PURPOSE: Intra-arterial chemotherapy for retinoblastoma is not always a straightforward procedure, and it may require an adaptable approach. This study illustrates strategies used when the ophthalmic artery is difficult to catheterize or not visible, and it ascertains the effectiveness and safety of these strategies. MATERIALS AND METHODS: A retrospective study was performed on a series of 108 eyes affected by intraocular retinoblastoma and selected for intra-arterial chemotherapy (follow-up range, 6-82 months). We recognized 3 different patterns of drug delivery: a fixed patte

Repurposing a Dual-Head Programmable Power Injector for Controlled Intra-Arterial Vasodilator Delivery During Cerebral Vasospasm Treatment.

Cerebral vasospasm following aneurysmal subarachnoid hemorrhage remains a leading cause of morbidity and mortality. Endovascular treatment with intra-arterial vasodilators is established for medically refractory vasospasm, yet medication delivery relies on manual hand-syringe injection with limited control over infusion rate and volume. In this proof-of-concept technical note, we describe a workflow innovation repurposing a dual-head programmable power injector (Nemoto PRESS DUO elite, Nemoto Kyorindo) for simultaneous contrast administration and controlled vasodilator infusion during endovasc

[The study of continuous infusion chemotherapy with low-dose cisplatin and 5-fluorouracil for patients with primary liver cancer].

Twelve patients with unresectable primary liver cancer (hepatocellular carcinoma and cholangiocarcinoma) and postoperative recurrence of primary liver cancer received continuous arterial or systemic infusion of low-dose CDDP/5-FU. This infusion chemotherapy was continued for five days, discontinued for two days, and repeated four weeks as one course basally. The partial response rate in patients with HCC or CCC treated with intra-arterial infusion was 20% and 33%, respectively. The rate in patients with HCC or CCC treated with systemic infusion was 0% and 33%, respectively. The response rate i

Hepatic Arterial Buffer Response in Liver Radioembolization and Potential Use for Improved Cancer Therapy.

Liver radioembolization is a treatment option for unresectable liver cancers, performed by infusion of 90Y or 166Ho loaded spheres in the hepatic artery. As tumoral cells are mainly perfused via the liver artery unlike hepatic lobules, a twofold tumor to normal liver dose ratio is commonly obtained. To improve tumoral cell killing while preserving lobules, co-infusion of arterial vasoconstrictor has been proposed but with limited success: the hepatic arterial buffer response (HABR) and hepatic vascular escape mechanism hamper the arterioles vasoconstriction. The proposed project aims to take b

Excitation of carotid body chemoreceptors by neuropeptide-Y.

Neuropeptide-Y (NPY) is a peptide co-localised with noradrenaline in many sympathetic nerves. Recently, it has been found in postganglionic sympathetic nerves running to blood vessels in the carotid body. When NPY is administered to cats by infusion into the arterial blood close to the carotid bodies, breathing is stimulated. This effect is abolished when the carotid sinus nerves are cut. Similar intra-carotid infusions of NPY can be demonstrated to increase the frequency of firing of chemoreceptor afferent nerves. However, NPY introduced into the blood vessels of the carotid body immediately

Response to dopamine hydrochloride in the hepatorenal syndrome.

Functional renal failure accompanying advanced cirrhosis of the liver carries a grave prognosis. Seven patients with the hepatorenal syndrome and five patients with decompensated cirrhosis of the liver without renal failure were studied by the xenon Xe 133 washout technique. Mean renal blood flow and its cortical component were decreased in both groups compared to normal transplant donors, but to a significantly greater degree in hepatorenal syndrome. In hepatorenal patients, intra-arterial infusion of subpresor doses of dopamine hydrochloride improved the angiographic appearance of the renal

Massive bleeding due to arterial-enteric fistula from an ingested toothpick.

A 62-year-old man developed massive lower gastrointestinal tract bleeding. Upper endoscopy and superior mesenteric arteriography initially failed to disclose a cause of bleeding. On rebleeding, intra-arterial vasopressin infusion during repeated arteriography caused reflux of dye into the iliac vessels to allow visualization of an arterial-enteric fistula. Exploratory laparotomy subsequently revealed perforation of the small bowel and common iliac artery by a toothpick which had been swallowed.

Hepatic artery embolization in the treatment of hepatic neoplasms.

Seventy-two hepatic artery embolizations were performed in 47 patients to treat hepatic neoplasms. Hepatic artery embolization creates tumor devascularization, but the portal flow prevents infarction of liver parenchyma because of the single vascular supply from the hepatic artery to a neoplasm, in contrast to the dual vascular supply to the liver parenchyma. Indications for the use of hepatic artery embolization are failure of chemotherapy, either systemic or intra-arterial infusion, vascular anomalies requiring combined lobar embolization and lobar infusion, and lack of effective treatment.

[Arterial infusion chemotherapy in patients with gastric cancer in liver metastasis and long-term survival after treatment].

Fifty-five gastric cancer patients with liver metastasis received arterial infusion chemotherapy. In 14 patients who had lesions in the liver intra-hepatic artery infusion chemotherapy was performed, while in 41 patients who had lesions in the liver and other sites intra-aortic infusion chemotherapy was performed. Methods for inserting the catheter into the aorta or hepatic artery and treatment schedules were reported previously. Between 1975 to 1981, 33 gastric cancer patients with liver metastasis were treated with 5-FU only (4 cases). MMC.5-FU (18 cases) and ADM.5-FU (11 cases). No response

A System for Continuous Pre- to Post-reperfusion Intra-carotid Cold Infusion for Selective Brain Hypothermia in Rodent StrokeModels.

Intra-carotid cold infusion (ICCI) appears as a promising method for hypothermia-mediated brain protection from ischemic stroke. Recent clinical pilot studies indicate easy implementation of ICCI into endovascular acute ischemic stroke treatment. Current rodent ICCI-in-stroke models limit ICCI to the post-reperfusion phase. To establish a method for continuous ICCI over the duration of intra-ischemia to post-reperfusion in rodent stroke models, a novel system was developed. Eighteen male Sprague-Dawley rats were included. Intraluminal filament method was used for transient middle cerebral arte
