Data Resource Profile: The Enhanced Prescribing Database (EPD) in Northern Ireland.

INTRODUCTION: The Enhanced Prescribing Database (EPD) is a comprehensive electronic database capturing detailed information on all Health and Social Care Northern Ireland (HSCNI) prescriptions dispensed in community pharmacies across Northern Ireland (the publicly funded healthcare system in Northern Ireland, equivalent to the NHS elsewhere in the United Kingdom). Established in 2010 by the Health and Social Care Business Services Organisation (HSC BSO), the EPD provides a valuable resource for monitoring prescribing practices and supporting population-based research. METHODS: When pharmacies…

Ambient air pollution at home, work and school: Evidence from Ireland.

Most of the existing literature linking air pollution with health outcomes uses data on ambient (i.e., outdoor) air pollution concentrations … ople it is school/college). In this paper, we use micro-data from the 2022 Census of Population for Ireland, matched to air pollution data (PM2.5, PM10, NO2, O3) from the Irish Environmental Protection Agency (EPA), to assess differences in ambient air pollution concentrations at residential and work/school/college locations. The results show that average …

Characterisation of Giardia duodenalis assemblages from veterinary clinical cases in Ireland.

Giardia duodenalis is a flagellated intestinal protozoan and a common cause of diarrhoea in humans and animals worldwide. Infection typically occurs through waterborne transmission or the faecal-oral route. Genetic studies recognise eight assemblages (A-H) with varying host specificity and zoonotic potential, making accurate assemblage discrimination essential within a One-Health framework. This study aimed to identify assemblages present in clinical cases received at a veterinary diagnostic laboratory in Ireland using a multilocus genotyping (MLG) approach and critically evaluate the performa…

Geogenomic mapping of drug-resistant Mycobacterium tuberculosis from Ireland and overseas.

In this study, we performed an in-depth comparison of genome-sequenced Mycobacterium tuberculosis isolates from Ireland with isolates from other countries. The sequenced isolates from Ireland mostly belonged to Lineage 4 (64.15 %) with Lineages 2 (17.27 %), 1 (13.21 %), 3 (5.22 %), and 5 (0.15 %) also represented. Of these, Lineages 2 (47.57 %) and 4 (34.95 %) accounted for the majority of the isolates that were resistant to at least rifampicin. By performing hierarchical clustering of the genomes, we determined that many drug-resistant (DR) strains of Lineage 2 collected in Ireland belonged t…

Psychedelic-related deaths in England, Wales and Northern Ireland (1997-2022).

BACKGROUND: Psychedelic drugs are increasingly visible in society once more, but their risks and adverse effects have received less attention than perhaps they should. While fatalities associated with psychedelics appear rare, a systematic approach to characterising their aetiology is required to inform harm minimisation efforts. AIMS: This study aimed to analyse prevalence and characteristics of psychedelic-related deaths in England, Wales, and Northern Ireland, between 1997 and 2022. METHODS: We analysed coroner reports submitted to the National Programme on Substance Use Mortality where p…

Prescribing patterns of myopia control contact lenses among optometrists in Ireland.

PURPOSE: This retrospective analysis of electronic medical record (EMR) data investigated the prescribing patterns of soft myopia control contact lens (MCCL) treatments since their introduction in Ireland in 2017. METHODS: Anonymised EMR data were sourced from 33 optometry practices in Ireland from 2017 to 2021 to determine the number of practices prescribing MCCLs to myopic children 5-18 years old. In MCCL-prescribing practices, the proportion of contact lens wearing children fitted with MCCLs and the proportion of progressive (≤-0.25 D/year) myopic children fitted with MCCLs were determined…

Perinatal Testicular Torsion in Northern Ireland: A Nationwide 10-Year Review.

INTRODUCTION: Perinatal or neonatal testicular torsion (PTT) is defined as testicular torsion within the first 30 days of life. PTT is a rare event, and bilateral PTT is even rarer. However, recent articles have advocated emergency bilateral exploration to exclude the catastrophic possibility of asymptomatic contralateral testicular torsion leading to anorchia. The aim of this study is to evaluate PTT in Northern Ireland (NI) using a nationwide population-based retrospective observational study. METHODS: All boys in NI who underwent emergency scrotal exploration within their first 30 days of …

Establishing a COVID-19 pandemic severity assessment surveillance system in Ireland.

We developed a COVID-19 pandemic severity assessment (PSA) monitoring system in Ireland, in order to inform and improve public health preparedness, response and recovery. The system based on the World Health Organization (WHO) Pandemic Influenza Severity Assessment (PISA) project included a panel of surveillance parameters for the following indicators: transmissibility, impact and disease severity. Age-specific thresholds were established for each parameter and data visualised using heat maps. The findings from the first pandemic wave in Ireland have shown that the WHO PISA system can be adapt…

Epidemiology of Campylobacter infections in Ireland 2004-2016: What has changed?

Campylobacter is the most common notifiable cause of bacterial gastroenteritis in humans in Ireland. However, epidemiological information is limited. We aimed to describe Campylobacter epidemiology in Ireland and trends over time, to inform future surveillance and research. We reviewed data completeness and described notified cases of campylobacteriosis (2004-2016) by age, sex, geographical area, season and trends over time. We used negative binomial regression to estimate incidence rate ratios (IRR) and adjusted IRR (aIRR) by age group, sex, geographical area and season. We undertook interrup…

Dental school admissions in Ireland: can current selection criteria predict success?

INTRODUCTION: Entry into university education in Ireland, including dental school, is based solely on academic performance in the Leaving Certificate Examination, held at the end of formal school education. The aim of this investigation was to examine the suitability of this process for the selection of dental students in Ireland. MATERIALS AND METHODS: Information for all dental students who entered the dental degree programme immediately following completion of the Leaving Certificate Examination at the National University of Ireland, Cork, during the years 1997-1999 was retrieved. Tuberous sclerosis complex: clinical features, diagnosis, and prevalence within Northern Ireland.

Tuberous sclerosis complex (TSC) is an autosomal dominant disorder caused by mutations in the TSC1 and TSC2 genes on chromosomes 9 and 16 respectively. Diagnosis is based on clinical features but can be difficult as a result of variable phenotypic expression. With the advantage of mutation analysis in making a diagnosis of TSC, and improved identification of the associated clinical features, there have been few new data on its prevalence and on the proportion of cases due to new mutations. We have performed a retrospective epidemiological study on the prevalence of TSC, the clinical features a

Genetic study of congenital heart defects in Northern Ireland (1974-1978).

Congenital heart defects are a major congenital abnormality and are assuming increasing importance. A study was undertaken to estimate the incidence of congenital heart defects in Northern Ireland over a five year period (1974-1978), to determine the age at diagnosis and to assess the risk of recurrence in sibs. An incidence rate of 7.3 per 1000 total births was found. This reduced to 3.1 per 1000 total births if only invasive methods of diagnosis (catheter studies, surgery, or necropsy) were considered. The overall risk of recurrence for sibs (excluding index patients with chromosomal abnorma…

Using an experience based design approach to advance health literacy in Ireland.

Health literacy supports people, communities, and organisations in addressing noncommunicable diseases and their determinants, taking meaningful action to reduce risk factors, and support health and wellbeing for all. Addressing the issue of low health literacy has the potential to increase health, health equity and health system effectiveness by building citizens' capacities for health. The Sláintecare Healthy Communities programme was launched in 2021 in Ireland focussing specifically on promoting health and wellbeing at the community level, and improving health literacy is a key area of foc…

The perceived value of clinical reflective work undertaken by radiography students in Ireland.

INTRODUCTION: Self-reflection is promoted by the regulatory body CORU to support professional development and improve clinical practice. However, heavy workloads and the use of reflection as a formal assessment tool may reduce engagement. Limited research has explored students' perspectives on self-reflection. This study aimed to assess students' perceptions of its value and factors influencing engagement during clinical placement. METHODS: A mixed-methods study was conducted with undergraduate and graduate-entry radiography students at a single institution. Data was collected using a Likert-

The landscape of hereditary haemochromatosis risk and diagnosis across the British Isles and Ireland.

Hereditary haemochromatosis is caused by pathogenic variants in the homoeostatic iron regulator gene HFE. Outcomes include liver cancer, cirrhosis and arthropathy, but penetrance is incomplete. Here, we use genetic data from >400,000 subjects to determine the genetic risk across 29 regions of the British Isles and Ireland. Northwest Irish and Outer Hebrideans are at the highest risk (1/54 - 1/62 carry the major risk genotype), Mainland Scots are also at increased risk (1/117), declining to 1/212 in Southern England. We also assessed the prevalence of clinically diagnosed haemochromatosis in >6…
