Fluid resuscitation in circulatory shock.

Over the past century, the treatment of various forms of circulatory shock has included fluid resuscitation with either crystalloidal or colloidal solutions. Despite decades of investigation, there still is considerable controversy over the beneficial and adverse effects of each fluid type. Most authors agree that the initial resuscitation of any form of shock should be performed with crystalloid solutions. Trauma resuscitation uses crystalloid therapy almost exclusively. Much controversy exists when the shock state involves increased microvascular permeability, such as seen in sepsis, anaphyl

Relationship between the pH of enema solutions and intestinal damage in rabbits.

Mechanical enemas can lead to intestinal mucosal injuries and bowel barrier damage, presenting as electrolyte disturbances and functional intestinal disorders. Most researchers believe that the mechanism of injury is related to osmolality, volume and temperature of the solution, infusion pressure, and the composition of the enema tube. We hypothesized that the pH of the enema solution may also contribute to intestinal damage. We administered enema solutions--normal saline, soapsuds, or vinegar (neutral, alkaline, or acidic solutions, respectively)--to three groups of rabbits (n = 20 per group)

Management of severe dengue in children.

Dengue is a major global disease which, in its severe form, affects up to 500,000 people worldwide each year, most of whom are children. The development of a safe and effective vaccine is a clear priority, together with public health measures to prevent the spread of infection. However, while major epidemics continue to occur, clinicians must also focus on optimising management. Although no specific treatment is available at present, with good supportive care, mortality for children with DHF can be reduced to well below 1%. In patients without signs of shock, fluid replacement can be attempted

Effect of composition of gastric contents on resistance to emptying of liquids from stomach in humans.

Experiments were conducted in normal healthy volunteers to investigate whether factors other than the intragastric pressure induced by tonic contraction of the gastric fundus could regulate the gastric emptying of liquids. The emptying of solutions of different osmolality and composition from the stomach was measured, while maintaining the intragastric pressure constant with a barostat system that employed an external reservoir. Preliminary experiments showed a linear relationship between intragastric pressure and emptying rate, but indicated that a normal intragastric pressure of 7 cm water w

Central retinal artery occlusion associated with severe ovarian hyperstimulation syndrome.

PURPOSE: Ovarian hyperstimulation syndrome (OHSS) is a serious iatrogenic complication of ovulation induction. It is a potentially lethal condition, with severe complications which include ovarian enlargement, and massive fluid redistribution from the vascular system into free spaces resulting in ascites, pleural effusion, electrolyte imbalance, hemoconcentration, hypovolemia, oliguria, and adult respiratory distress syndrome. Thromboembolism is a rare but extremely serious complication. CASE REPORT: We report a case of severe OHSS, presenting with central retinal artery occlusion (CRAO). DI

Effect of hypervolemic therapy on cerebral blood flow after subarachnoid hemorrhage : a randomized controlled trial.

BACKGROUND AND PURPOSE: Cerebral blood flow (CBF) is reduced after subarachnoid hemorrhage (SAH), and symptomatic vasospasm is a major cause of morbidity and mortality. Volume expansion has been reported to increase CBF after SAH, but CBF values in hypervolemic (HV) and normovolemic (NV) subjects have never been directly compared. METHODS: On the day after aneurysm clipping, we randomly assigned 82 patients to receive HV or NV fluid management until SAH day 14. In addition to 80 mL/h of isotonic crystalloid, 250 mL of 5% albumin solution was given every 2 hours to maintain normal (NV group, n

[Analysis of type II pneumocyte protection by pneumoplegic solutions. An experimental in vitro study of lung preservation for transplantation].

With the aim of studying the physiopathological mechanism of lung preservation, we have valued the Pneumocytes Type II viability after six hours of incubation at 4 degrees C in extracellular (Ringer Lactate) and intracellular (Collins, Euro-Collins and Belzer) solutions. The cells have been cut off from adult rat's lung using the modified Dobbs' method. Alveolar Type II viability have been valued using two methods: the total protein content in each culture and the metabolic function of the cells using the rate of protein synthesis by means of 35 S methionine uptake assay. The results have demo

Adhesive bonding of various materials to hard tooth tissues--solubility of dentinal smear layer in dilute acid buffers.

While the acid etch technique has largely solved the problem of bonding restorative materials to enamel, attempts to improve adhesive bonding to dentin have so far had only limited success. One aspect to which insufficient attention has been paid is the presence of a smear layer on cut surfaces and of pellicle on surfaces exposed to saliva, both of which would reduce or prevent durable adhesive bonds. The present study attempts to evaluate the usefulness of various acidic solutions in removing these surface layers while causing minimal damage to the vital contents of the dentinal tubules. A se

Transplacental, amniotic, urinary, and fetal fluid dynamics during very-large-volume fetal intravenous infusions.

With rapid intravenous infusion of very large volumes of isotonic saline solutions into the fetus, the fluid could stay within the fetal body, thereby creating hydrops fetalis, be transferred into the amniotic fluid through the fetal kidneys, thereby creating polyhydramnios, or be transferred across the placenta into the maternal circulation. This study was designed to explore these possibilities. After a 1-hour control period, 10 near-term chronically catheterized ovine fetuses were infused intravenously with 4 L (greater than 100% of fetal weight) of either isotonic saline solution or lactat

[Whole bowel irrigation as preparation for colorectal surgery].

One hundred consecutive patients underwent a whole gut irrigation as preoperative care for surgery of the large intestine. The solution used was balanced for isotonicity with mineral salts and polyethylene glycol. The procedure was quite well tolerated by the patients, also by the elderly. Only slight changes in body weight or in serum ionic concentrations were noted. In 88% of the patients the preparation was considered to be good or excellent by the operating surgeon. There was a tendency for low rate postoperative infections.

Profuse diarrhea induced by Campylobacter.

A 31-year-old woman with a five-day prodrome of fever, chills, and diarrhea was hospitalized with severe dehydration and profuse diarrhea due to infection with Campylobacter jejuni. Stool volumes were as high as 8,800 ml per 24 hours. Treatment with intravenous fluids, trimethoprim-sulfamethoxazole, and an oral isotonic glucose-sodium solution produced rapid improvement, suggesting a possible role for oral fluid and electrolyte replacement in Campylobacter enteritis.

Dissociation of frog olfactory epithelium.

We report a method for producing cell suspensions from frog olfactory epithelium. The tissue is incubated for 45 min at room temperature in a solution which causes dissociation of the epithelium. The solution is an isotonic saline buffered to maintain a pH of 10.3 and a free Ca2+ ion concentration of 10(-6) M. The method uses no degradative enzymes, except for a brief DNase treatment. The resulting cell suspension contains single olfactory receptor neurons, sustentacular cells, glandular cells, and respiratory epithelial cells. The cells are viable as judged by vital staining, ciliary motility

[Treatment of acute diabetic metabolic crises in adults (Update 2026)].

Acute metabolic crises can be potentially life-threatening situations in adults depending on the extent. Therefore, rapid comprehensive diagnostic and therapeutic measures with close monitoring of vital and laboratory parameters are required. The treatment of diabetic ketoacidosis (DKA) and the hyperosmolar hyperglycemic state (HHS) is essentially the same and replacement of the mostly substantial fluid deficit with several liters of a physiological crystalloid solution is the first and most important step. Serum potassium concentrations need to be carefully monitored to guide its substitution

Cardiac ultrasound-guided crystalloid preloading before spinal anesthesia vs. standard coloading for scheduled cesarean delivery: a randomized controlled trial.

BACKGROUNDS: Spinal anesthesia for cesarean delivery commonly causes maternal hypotension (60-80%), leading to adverse maternal and fetal outcomes. Standard crystalloid coloading is preferred over preloading, but individualized fluid strategies based on preload dependence remain untested. METHODS: In this randomized controlled trial (NCT07108881) conducted at the Tunis Maternity Center, 96 ASA II women with singleton pregnancies scheduled for elective cesarean under spinal anesthesia and preload dependent (ΔLVOT VTI ≥ 12% after passive leg raising via cardiac ultrasound) were enrolled (n=96)

Mineralocorticoid deficiency in hemorrhagic shock.

BACKGROUND: In the critically ill, mineralocorticoid deficiency (MD) is associated with greater disease severity, the development of acute renal insufficiency, and increased mortality. We hypothesized that severely injured trauma patients presenting with hemorrhagic shock would demonstrate a high degree of MD. We also hypothesized that MD in these patients would be associated with increased length of stay, hypotension, fluid requirements, and acute kidney injury (AKI). MATERIALS AND METHODS: Thirty-two trauma patients in hemorrhagic shock on admission to the trauma bay (SBP <90 mm Hg × 2) wer
