BACKGROUND/AIMS: To prevent the pancreatic fistulas, we designed a technique termed "no naked pancreatic surface in the cavity of jejunum" on pancreaticojejunostomy.
METHODOLOGY: We adopted pancreatic exocrine secretions following the pancreatic duct by drainage; there was no naked pancreatic surface in the cavity of jejunum, and entail 2-3 cm sheath of the jejunum to the pancreatic stump.

Esophageal replacement continues to be a challenging operation associated with significant morbidity. Continuous jejunal perfusion with 0.1, 1 and 10% lactose solutions was made in 9 patients with good milk tolerance and 25 patients with secondary hypolactasia. Small pieces of jejunum with an intact blood supply were explanted to the anterior abdominal wall in rats. Six weeks after explantation the mucosa appeared totally flat in many areas, both histologically and under the dissecting microscope. The structure of the flattened mucosa was shown to be identical to that in coeliac disease with hypertrophied intervillous ridges. A dynamic study with tritium-labelled thymidine demonstrated a considerably increased turnover in the flat mucosa with some disorganization of cell production and migration.
