[Reconstruction of metacarpophalangeal and interphalangeal joints with endoprostheses following injuries].

Implants have been used since 1969 by the authors for the replacement of damaged finger joints following trauma. In several cases reconstruction of other damaged tissues in the same finger was also performed. A method is described for replacement of the central slip of the extensor tendon over the PIP joint. Analysis of twenty-one MP and sixty-eight PIP endoprostheses placed in eighty-three patients until 1979 is given. Significant improvement in the range of movement is seen. Preoperative pain was relieved in all cases. There was a need to change occupation in ten percent of patients. Careful…

Fracture-dislocations of all five carpometacarpal joints: a case report.

Injuries to all five carpometacarpal joints are rare. Only four cases have been reported in the English literature. A case of fracture-dislocations of all five carpometacarpal joints is presented. An excellent result has been obtained 2 years after open reduction and internal fixation. This case is the only one in the English literature so treated and with prolonged follow-up.

HLA-B27 and involvement of sacroiliac joints in rheumatoid arthritis.

The frequency of radiographic signs of sacroiliac joint involvement (greater than or equal to 2 and greater than or equal to 3 according to the New York criteria) was significantly higher in 28 HLA-B27 positive patients with classical seropositive rheumatoid arthritis (RA), than in 28 B27 negative RA controls. The B27 positive RA patients had more subcutaneous nodules (p less than 0.01), worse functional class (p less than 0.05), and higher levels of erythrocyte sedimentation rate (ESR) (p less than 0.05) and haptoglobin (p less than 0.05). Sacroiliitis, independent of HLA-B27, was associated

Simultaneous closed dislocation of both interphalangeal joints in one finger.

A rare case of simultaneous dislocation of both interphalangeal joints in one finger in a table-tennis player is presented. The second dislocation took place when the first dislocated joint became the fixed part of the finger as it hit a wall. Treatment was, first, hyperextension to unlock the base of the phalanx, then traction along the phalanx: its base was then pushed into contact with the head of the proximal phalanx. Splinting was applied with the joint in slight flexion.

Fatigue response of lumbar intervertebral joints under axial cyclic loading.

A low-cycle fatigue of 11 lumbar intervertebral joints under axial compression is reported. The magnitude of the maximum compressive load ranged from 37 to 80% of the failure load. The maximum deformation, as a function of the number of cycles, showed two distinct results: one group showed a gradual, stable increase, and the other an abrupt, unstable increase. The before- and after-test radiographs showed a one-to-one correspondence between unstable specimens and generalized bony failure. The radiographs of 5-mm thick transverse endplate slices show crack propagation from the periphery of the …

The sacroiliac joints: anatomic, plain roentgenographic, and computed tomographic analysis.

Due to its unique bicompartmental anatomy and spatial configuration, the sacroiliac joint can be more accurately defined by computed tomography (CT) than conventional radiography. Using a tilted gantry and paraaxial scanning technique, the synovial portion of the joint is oriented vertically on the CT image, while the ligamentous portion is oriented oblique-horizontally. The tilted CT gantry technique allows full ventral-dorsal imaging of the synovial portion of the sacroiliac joint. We have found the accuracy of CT to be superior to conventional radiography in the detection of early erosive s
