Biomechanical properties of keratoconus suspect eyes.

PURPOSE: Measuring corneal biomechanical properties may help detect keratoconus suspect corneas and eliminate the risk of ectasia after LASIK. METHODS: Data of 504 eyes separated into three groups were retrospectively reviewed: normal (n = 252), keratoconus suspect (n = 80), and keratoconus (n = 172). Corneal hysteresis (CH) and corneal resistance factor (CRF) were measured with an ocular biomechanics analyzer. RESULTS: Mean corneal hysteresis was 10.6 +/- 1.4 (SD) mm Hg in the normal group, compared with 10.0 +/- 1.6 mm Hg in the keratoconus suspect group and 8.1 +/- 1.4 mm Hg in the kerato…

[Keratoconus: epidemiology, risk factors and diagnosis].

Keratoconus is a bilateral, non-inflammatory and progredient corneal ectasia with an incidence of approximately 1 per 2,000 in the general population. Within the second decade of life the cornea develops a conical shape, due to thinning of the corneal stroma with subsequent irregular astigmatism and myopia leading to marked impairment of vision. The most common presentation of the keratoconus is as a sporadic disorder, but it has long been recognized that a significant minority of patients exhibit a family history as an autosomal dominant mode of inheritance. Most investigators suggest complet…

Keratoconus associated with hyperimmunoglobulin E syndrome.

PURPOSE: To report a case of keratoconus in a patient with hyper-IgE syndrome. METHODS: A case report of a 28-year-old man with hyperimmunoglobulin E syndrome (HIES) who presented with chronic eczematous dermatitis of the eyelids and severe eye itching. RESULTS: Best corrected visual acuity was 20/25 in the right eye and 20/30 in the left eye. There was a scissoring reflex in both eyes with retinoscopy. Biomicroscopy of the cornea was normal, but corneal topography showed bilateral keratoconus. CONCLUSIONS: It is difficult to establish a direct correlation between keratoconus and HIES due t…

Asymmetric keratoconus attributed to eye rubbing.

PURPOSE: To report a series of cases with asymmetric keratoconus attributed to eye rubbing, discuss its pathogenesis, and review the literature. METHODS: Case reports and literature review. RESULTS: A careful history obtained from 5 patients with asymmetric keratoconus revealed habitual eye rubbing of the more severely affected eye. CONCLUSIONS: Trauma, such as occurs with habitual eye rubbing, plays a role in the pathogenesis of keratoconus.

[Keratoconus: what happens with older patients?].

INTRODUCTION: Keratoconus is a degenerative corneal disease that is most often seen in young patients. The object of the present study was to analyze the demography of keratoconus patients. PATIENTS AND METHODS: We conducted a prospective study at Bordeaux University Hospital from January 2001 to September 2001. The study included all the consecutive patients with a diagnosis of keratoconus, detected previously or during the current examination. Inclusion criteria included evidence of unilateral or bilateral abnormal corneal steepening with videotopography. RESULTS: We included 121 patients.…

Recurrent keratoconus 40 years after keratoplasty.

Recurrent keratoconus is a corneal dystrophy that degrades the optical function of the corneal graft. Recurrence of keratoconus after keratoplasty has been reported previously in some scattered case reports, but the aetiology is still unclear. This report describes the case of a 74-year-old white woman, who presented recurrent keratoconus 40 years after keratoplasty. Ophthalmological and videokeratographic data are presented. The patient obtained satisfactory vision with soft contact lenses, and recurrent keratoconus remained stable during a 2-year period. Recurrent keratoconus generally has a…

Matrix metalloproteinase 2: involvement in keratoconus.

PURPOSE: The activation of matrix metalloproteinase-2 (MMP-2) is postulated to be a crucial pathogenic factor behind progressive and chronic diseases in which basement membranes are disrupted. An ocular example is keratoconus. The purpose of the present enquiry was therefore to investigate and compare the activities of the MMP-2 secreted by keratocytes of normal and keratoconic corneas. METHODS: The spectrum of MMP-2 activities secreted by cultures of keratocytes derived from normal and keratoconic corneas was analysed by zymography. Subsequently, selected preparations were assayed for peptid…

[Topography-based calculation of keratoconus dimensions].

BACKGROUND: Keratoconus is a corneal dystrophy which usually develops in the second or third decade of life and shows various speed of progression. This disease may degrade the image-forming properties of the eye even in its early stage. The purpose of this study was to support the conventional clinical qualitative diagnostic methods of keratoconus handling with a topography-based algorithm to quantify the "ballooning" of the anterior corneal surface. PATIENTS AND METHODS: Eighty-eight patients with keratoconus (46 with mild and 42 with severe clinical signs) and a control group of 40 normal …

Association between anterior and posterior keratoconus.

BACKGROUND/METHODS: We studied a 53-year-old male patient, presenting with a history of reduced vision since childhood in the right eye and since birth in the left eye. RESULTS: Ocular examination revealed the presence of anterior keratoconus in the right eye and posterior keratoconus in the left eye. CONCLUSION: To our knowledge, co-occurrence of both varieties of keratoconus in the same patient has not been previously reported.

Corneal topography to help detect keratoconus.

1. The incidence of keratoconus is about 1 in 2,000 in the general population, but is believed to be higher if subclinical forms are taken into account. 2. Corneal topography helps to detect keratoconus when other testing methods may have missed early signs of the disease. 3. At a time when refractive surgery is common, it is essential to identify corneal abnormalities prior to surgery.

Keratohematoma leading to acquired posterior keratoconus.

We describe a patient with congenital syphilis associated with corneal neovascularization and subsequent intracorneal hemorrhage. Over time, this condition resulted in acquired posterior keratoconus. A review of our clinical case, the literature, and histologic findings is presented. To our knowledge, this is the first reported case of acquired posterior keratoconus associated with corneal vascularization and hemorrhage secondary to congenital syphilis. We also have provided histopathologic evidence for the prescence of an iron ring.

Floppy eyelid syndrome associated with keratoconus.

Keratoconus has been associated with various ocular and systemic disorders. It has been linked, in a few cases, with floppy eyelid syndrome (a disorder characterized by a soft rubbery upper tarsus, papillary conjunctivitis and obesity). A 40-year-old male presented with a complaint of gradually decreasing vision in the right eye. An intermittent right exotropia had become constant and he was unable to maintain fixation voluntarily. Corneal findings were consistent with keratoconus. Floppy eyelid syndrome was also diagnosed. An underlying connective tissue disorder may link these two disorders.

Contact lens failure in keratoconus management.

PURPOSE: Surgery is indicated for keratoconus when management with contact lenses fails. The authors sought to determine the relative contribution of various preoperative patient and ocular factors to the ultimate causes of contact lens failure. METHODS: The records of unoperated eyes of keratoconus patients whose contact lenses were managed intensively before undergoing penetrating keratoplasty (PK) at the authors' institution between 1981 and 1990 were selected for study. Univariate and multivariate analyses were performed to identify risk factors for early contact lens failure. RESULTS: T…
