Periprosthetic fractures around total knee arthroplasty are an increasingly common and complex problem driven by rising arthroplasty volumes, aging populations, and longer implant survivorship. Successful management requires integration of trauma principles with arthroplasty-specific considerations, particularly implant stability, bone stock, and patient frailty. Implant stability and bone quality are the primary determinants of treatment strategy, outweighing fracture morphology alone. Classification systems are useful frameworks but must be interpreted in the context of component fixation an…

BACKGROUND: Knee osteoarthritis (OA) is a major public health issue causing chronic pain, impaired physical function, and reduced quality of life. As there is no cure, self-management of symptoms via exercise is recommended by all current international clinical guidelines. This review updates one published in 2015.
OBJECTIVES: We aimed to assess the effects of land-based exercise for people with knee osteoarthritis (OA) by comparing: 1) exercise versus attention control or placebo; 2) exercise versus no treatment, usual care, or limited education; 3) exercise added to another co-intervention …

Knee pain encompasses a wide array of differential diagnoses and can often pose a diagnostic challenge, as it can have traumatic or non-traumatic causes. A good clinical history, assessment and anatomical knowledge gives a strong foundation to narrow down the diagnosis, and understanding the correct imaging modality and findings further informs correct and timely management. This article reviews various disease processes including fractures, inflammatory, infective and neoplastic causes, and discusses the assessment and various imaging modalities to aid diagnosis in both primary and secondary …

Introduction The coronavirus disease 2019 (COVID-19) pandemic has resulted in rapid healthcare system adaptations, including the acceptance of telemedicine in primary care. In the case of knee ailments, among the most common problems encountered in primary care, telemedicine provides a literal window to observe the patient performing functional activities. Despite its potential, there is a lack of standardized protocols for data collection. The purpose of this article is to provide a step-by-step protocol to aid in performing a telemedicine examination of the knee. Methods This article provide…

Subchondroplasty is a relatively new minimally invasive procedure that has been used to treat bone marrow edema associated with osteoarthritis. Subchondroplasty as treatment for early stage osteonecrosis of the knee has not been extensively studied. The authors hypothesized that subchondroplasty may be an effective treatment for relieving pain, improving function, and preventing collapse in osteonecrosis. In this study, a retrospective review of 11 cases of subchondroplasty of the distal femur was conducted. There were no surgical complications with the procedure, and patients reported statist…

Chondroblastoma is an uncommon osseous neoplasm that accounts for less than 1% of all bone tumors. Characteristically it arises in the epiphysis or apophysis of long bones in young patients and may extend into the metaphysis. A sixteen-year-old male presents with a 1-year history of left knee pain associated with joint stiffness and interfering with performing daily activities. Radiographic and contrast enhanced magnetic resonance imaging favored the diagnosis of chondroblastoma. This was then confirmed histologically. The clinical signs and symptoms of Chondroblastoma are usually nonspecific,…

Hidradenitis suppurativa is a poorly understood, destructive disease centered on pilosebaceous units and characterized by inflammatory nodules that progress to abscesses, sinus tracts, and scars. The typical patient is a young woman with involvement of the axillae, breasts, and groin. Presented is a 60-year-old man with poorly controlled diabetes, who initially developed pink-to-yellowish plaques with punched out ulcers on his knees that evolved to scars and draining sinus tracts. Draining sinus tracts were also noted under the left axilla. Histopathology revealed suppurative granulomatous inf…

A 69-year-old man was referred to physical therapy by his primary care physician for an insidious onset of right medial knee pain that had been present for over 4 weeks. Due to the disproportionate response following initial management, he was referred to an orthopaedist for imaging. Magnetic resonance imaging revealed medial femoral condyle osteopenia, bone marrow edema, and subchondral bone collapse, and subsequently the patient was diagnosed with spontaneous osteonecrosis of the knee. J Orthop Sports Phys Ther 2018;48(10):824. doi:10.2519/jospt.2018.7923.

A 14-year-old boy presented with a painful knee following a distorsion. He was unable to actively extend his lower leg. Conventional radiography showed no signs of a fracture. Both ultrasound imaging and MRI, however, demonstrated haemarthrosis and an osteochondral defect of the lateral femoral condyle. Apparently, an osteochondral lesion had developed due to patellar luxation. The osteochondral fragment was fixated using absorbable compression screws.

Osteochondritis dissecans (OCD) of the knee is an idiopathic, focal, subchondral-bone abnormality that can cause instability or detachment of a bone fragment and overlying articular cartilage, with subsequent progression to osteoarthritis. The diagnosis is usually made during adolescence. Mechanical factors play a major role in the pathophysiology of OCD. When the radiographic diagnosis is made early in a patient with open physes, healing can often be obtained simply by restricting sports activities. The degree of lesion instability can be assessed by magnetic resonance imaging. When the lesio…

Arthrogryposis is defined as limited range of motion in three or more joints in two or more body parts. This article will describe treatment options for the arthrogrypotic knee. In all types of arthrogryposis, and in both extension and flexion deformities, very early treatment is favorable. Just after birth, traction and mobilization followed by serial casting could often greatly improve the range of motion. In the hyperextended knee, surgical lengthening of the extensor apparatus may be needed. Flexion deformities could be improved with temporary physeal arrest of the anterior distal femur by…

Disorders related to the knee extensor mechanism are common and rarely require imaging. Non specific anterior knee pain, fracture, dislocation, overuse tendinopathy and chronic patellofemoral instability are the commonest conditions encountered. Imaging is used in acute trauma, and for the assessment of cases of anterior knee pain resistant to conservative measures. The role of the radiograph is now largely restricted to cases of suspected fracture. Ultrasound is the optimum technique for suspected tendon and bursal pathology and MRI is widely used for the assessment of dysplasia and instabili…

A patient presented with an isolated anterolateral dislocation of the proximal tibiofibular joint. Closed reduction under general anaesthesia was performed by external rotation of the lower leg with the knee in 90 degrees of flexion and the ankle dorsiflexed with an audible pop.

The authors report a case of a 7-year-old boy who sustained a penetrating injury of a splinter of wood to the knee. Arthroscopic examination, removal of visualised foreign material and washout did not alleviate the symptoms of pain and swelling in its entirety. Microbiology cultures also failed to determine the cause of the on-going symptoms. Five days later, the patient underwent a mini arthrotomy through a lateral incision, which demonstrated synovitis, and removal of the remaining embedded foreign body from the lateral condyle. Although the authors advocate arthroscopy as the surgeon's firs…
