Treatment by primary arthrodesis of the knee joint after temporary intraluminal shunt placement in a 70-year-old man with open knee dislocation involving the popliteal vessels is reported. After temporary shunting, the definitive vascular reconstruction of both the popliteal artery and vein was established by end-to-end anastomoses because of the shortening effect of the arthrodesis. Two skin coverage procedures were performed on day 0 and day 18. The patient recovered activity to a level near his pretrauma status. Primary arthrodesis for open knee dislocation associated with vascular injury i…

PURPOSE: This study aimed to compare the incidence rates of concomitant injuries, including meniscal and cartilage injuries, between multiligamentous knee injuries (MLKI) with and without dislocation based on our 15-year experience of knee dislocation and MLKI at a level 1 trauma center.
METHODS: We retrospectively identified 100 patients (115 knees) with MLKIs and/or dislocations at our trauma center between 2007 and 2021. Magnetic resonance imaging was routinely performed to evaluate the injured structures and extent of injury. The anatomic structures of the knee were categorized into anter…

PURPOSE: This study compared mid-term outcomes of two anterior cruciate ligament (ACL) restoration techniques within an early total surgical care of acute knee dislocation: repair with additional internal bracing (ACLIB) and reconstruction with autograft (ACLR). Initial results at 12 months demonstrated that ACLR offered superior stability compared to ACLIB.
METHODS: Retrospective clinical study of patients with acute type III or IV KD. ACLIB or ACLR procedures were performed accompanied by simultaneous suture and internal bracing of the posterior cruciate ligament (PCL) and repair with later…

BACKGROUND: Acute tibiofemoral knee dislocations (KDs) with a single cruciate ligament remaining intact are rare and can be classified as Schenck KD I. The inclusion of multiligament knee injuries (MLKIs) has contributed to a recent surge in Schenck KD I prevalence and has convoluted the original definition of the classification.
PURPOSE: To (1) report on a series of true Schenck KD I injuries with radiologically confirmed tibiofemoral dislocation and (2) introduce suffix modifications to further subclassify these injuries based on the reported cases.
STUDY DESIGN: Case series; Level of evid…

Due to ligament laxity, bearing dislocation occurs in 1-6% of Oxford Domed Lateral (ODL) replacements with most dislocations occurring medially. Dislocations were studied using a previously built mechanical rig, however testing using the rig was inefficient. The aim of this study was to develop a better tool that was more reliable and efficient. An established robotics software package, the Open Motion Planning Library, was modified to accept the ODL components. Using a robotics path planning algorithm, the mobile bearing was allowed to find a way out from between the femoral and tibial compon

BACKGROUND: Lateral patellar dislocation (LPD) frequently causes damage to the knee. Injury patterns and risk factors contributing to such injuries have not yet been examined in detail.
METHODS: We retrospectively analyzed 50 consecutive patients with primary LPD. Two reviewers evaluated the MRI images regarding risk factors for LPD (Dejours classification; Caton-Deschamps Index, CDI; distance from the tibial tuberosity to trochlear groove, TT-TG; trochlear depth, TD) as well as joint damages according to the Whole-Organ Magnetic Resonance Imaging Score (WORMS).
RESULTS: 33 male and 17 femal

Congenital knee dislocation (CDK) is a rare condition, and its treatment is subject to debate. Here, we report on a new treatment for CDK (using a hexapod-type external fixator, HEF) in a three-year-old child with Larsen syndrome and grade III anterior dislocations of both knees. The left knee was treated with serial splints, whereas an HEF was used to treat an irreducible dislocation of the right knee. Two HEF aluminium rings were applied surgically. The procedure involved a distraction of the knee, then tibial posterior translation, and lastly progressive flexion. The clinical and radiologic…

The objective of this case report was to describe a patient who developed habitual dislocation of patella after knee disarticulation. Congenital deficiency of the tibia (tibial hemimelia, aplasia or dysplasia) is very rare, with an incidence of approximately 1 per 1 million live births. Knee disarticulation is preferred over transfemoral amputation for type I tibial deficiency. Most of the surgical techniques describe preservation of the patella and suturing of patellar tendon with cruciate ligaments. We present a case of a 14-year-old girl, who underwent disarticulation through the right knee

Congenital patellar dislocation is a relatively rare clinical deformity. In some patients, a delayed diagnosis or lack of early surgical intervention results in permanent patellar dislocation and subsequent knee osteoarthritis. Total knee arthroplasty (TKA) is currently the most effective treatment for end-stage osteoarthritis, but performing surgery in these patients presents notable technical challenges. The current report presents the case of a 60-year-old woman who had been suffering from knee extensor weakness and dysfunction for >50 years, resulting in an active knee extension deficit. T

Inferior dislocation of the patella is a rare cause of a blocked knee and occurs typically in elderly patients. In most cases the mechanism is an osteophyte on the superior pole of the patella that becomes entrapped into the femoral trochlea, keeping the patella horizontally rotated and inferior localized. Advanced age and osteoarthritic changes of the knee are the main risk factors. Reduction should be performed under general anesthesia. We describe a case of an elderly lady with a type II inferior patella dislocation after direct blunt trauma. The therapy entailed reduction, knee arthroscopy

PURPOSE: The literature shows no consensus regarding the best method and timing for the treatment of acute knee dislocations. Studies indicate that a one-stage procedure performed in the early stages of injury can lead to superior results. The aim of this study was to evaluate the results after performing early surgical repair with additional suture augmentation (ligament bracing) of all torn ligaments in acute knee dislocations.
METHODS: In this prospective multicentre study, 73 patients with an acute type III or IV knee dislocation were treated with one-stage ligament bracing within 10 days…

INTRODUCTION: An original classification of congenital dislocation of the knee (CDK) was drawn up, based on neonatal semiology. The objective of the present study was to assess impact on treatment decision-making and prognosis.
MATERIAL AND METHODS: Fifty-one CDKs in 40 patients were classified neonatally into 3 types: I, reducible (n=28); II, recalcitrant (n=16); and III, irreducible (n=7). Number of anterior skin grooves, range of motion (RoM), flexion deficit and reduction stability were recorded. Depending on reducibility, treatment comprised: physiotherapy with splints, traction with cas

BACKGROUND: Bearing dislocation is a unique complication of mobile-bearing unicompartmental knee arthroplasty (UKA), with diverse underlying mechanisms. This article reports a rare mechanism of bearing dislocation following mobile-bearing UKA, which was probably associated with the new-onset rheumatoid arthritis (RA) leading to anterior cruciate ligament (ACL) insufficiency—aiming to highlight this uncommon etiology and its implications for clinical practice. CASE PRESENTATION: A 53-year-old woman presented with acute right knee pain and restricted mobility one day prior to admission, who was

Explore the therapeutic effect of vastus medialis oblique plasty and the reliability and applicability of functional magnetic resonance imaging as a diagnostic method for early cartilage degeneration and injury diagnosis. From July 2020 to July 2022, there were 53 patients with recurrent patellar dislocation who met the inclusion criteria for surgery, including 34 women and 19 men, aged 11 to 53 years, with an average age of 24.4 years. After patient selection, functional magnetic resonance imaging was performed before surgery. According to the presence or absence of cartilage injury, they wer

BACKGROUND: Surgical treatment of young patients with recurrent lateral patella dislocation (RLDP) is often recommended because of loss of knee function that compromises their level of activity or even their daily life functioning. This situation is comparable to young patients with an anterior cruciate ligament (ACL) rupture. The purpose of this study was therefore to explore the time from injury to surgery and the pre-operative symptoms and knee function of young RLPD patients scheduled for stabilizing surgery and compare this group to age and sex-matched ACL-deficient patients.

Traumatic knee joint dislocations are relatively rare. Most of knee dislocations are reduced by closed manipulation. However occasionally, especially in the case of soft tissue incarceration, closed reduction may not be possible and open reduction is mandatory. This report introduces a case of irreducible posterolateral rotary knee dislocation with interposition of vastus medialis treated through two-staged operations. In this report, we included preoperative magnetic resonance images (MRI), detailed intraoperative descriptions with photographs and video illustration that show the status of an…
