[Treatment of floating knee injury in children with elastic intramedullary nail].

OBJECTIVE: To observe the clinical effect of elastic intramedullary nail in minimally invasive treatment of floating knee injury in children. METHODS: From January 2009 to September 2017, 11 children with floating knee injury were treated with one-off open reduction and elastic intramedullary nail or external fixator fixation, including 7 males and 4 females, aged 5.0 to 11.0 years, with an average age of 8.3 years. The treatment results were evaluated according to karlstrom's standard. RESULTS: Eleven patients were followed up for 8 to 48 months, with an average of 28 months. All the fractu

The Pediatric "Floating Knee" Injury: A State-of-the-Art Multicenter Study.

BACKGROUND: Isolated femoral and tibial fractures are 2 of the top 5 causes of pediatric orthopaedic hospital admission, yet their simultane … 66.6% of cases and good in 26.4% of cases. CONCLUSIONS: Previous literature on pediatric floating knee injuries consisted of small case series that were published prior to the introduction of flexible intramedullary nailing to North America. This multicenter study of a large cohort demonstrates a change in practice pattern from a largely nonoperative treatmen …

Iatrogenic popliteal artery injury in non arthroplasty knee surgery.

We have investigated iatrogenic popliteal artery injuries (PAI) during non arthroplasty knee surgery regarding mechanism of injury, treatment and outcomes, and to identify successful strategies when injury occurs. In all, 21 iatrogenic popliteal artery injuries in 21 patients during knee surgery other than knee arthroplasty were identified from the Swedish Vascular Registry (Swedvasc) between 1987 and 2011. Prospective registry data were supplemented with case-records, including long-term follow-up. In total, 13 patients suffered PAI during elective surgery and eight during urgent surgery such

Medial patellofemoral ligament: anatomy, injury and treatment in the adolescent knee.

PURPOSE OF REVIEW: Traumatic and nontraumatic patellofemoral instability (PFI) in children and adolescents is a complex problem. It is determined by a large number of mechanical and pathomorphologic conditions, mainly seen in nontraumatic dislocations. RECENT FINDINGS: Although conservative treatment with a short immobilization, followed by early passive motion and isometric quadriceps strengthening, can be considered in real traumatic dislocations without any cartilaginous injury, a surgical intervention should be considered in atraumatic cases. As 90% of PFI are nontraumatic and correlated

Risk factors for peroneal nerve injury and recovery in knee dislocation.

BACKGROUND: Acute knee dislocation is rare but has a high rate of associated neurovascular injuries and potentially limb-threatening complications. These include the substantial morbidity associated with peroneal nerve injury: neuropathic pain, decreased mobility, and considerably reduced function, which not only impairs patient function but complicates treatment. QUESTIONS/PURPOSES: We therefore identified and quantified the risks associated with specific factors for peroneal nerve injury and recovery in patients with knee dislocations. PATIENTS AND METHODS: We retrospectively reviewed the

Acute traumatic and sports-related osteochondral injury of the pediatric knee.

Adolescents are predisposed to osteochondral (OC) injuries in the knee. The medial facet of the patella, the femoral trochlea, and the lateral femoral condyle are the most common sites of injury. Most of these injuries are classically traumatic but noncontact injuries. Surgery is warranted in most cases of OC fracture. Depending on size, condition, and location of the lesion, options include OC fragment reduction and internal fixation or excision and cartilage resurfacing. Understanding of how to diagnose and treat OC fractures will help optimize outcomes.

Physical examination and imaging of the acute multiple ligament knee injury.

Examination and evaluation of the patient a with multiple ligament injured knee is a complicated process and best done in a methodical, comprehensive fashion with a particular emphasis placed on assessment of supporting soft tissues. Tissues that can be damaged during this devastating injury include bones, ligaments, meniscus, articular cartilage, and neurovascular structures. Uses of diagnostic imaging and the physical examination process will be described in this article.

The role of arteriography in assessing popliteal artery injury in knee dislocations.

BACKGROUND: This study aimed to review the need for angiography among patients with traumatic knee dislocations, and to evaluate any adverse consequences associated with the clinical decision to pursue or defer angiography. METHODS: A retrospective analysis was performed for 55 patients (57 knees) with traumatic knee dislocation during a 7-year period. The presence or absence of arterial injury was assessed via physical examination (to determine presence of foot pulses and ankle-brachial index > or = 0.80) and, in selected cases, via angiography. RESULTS: At the vascular examination, 32 knee

High-velocity knee dislocation with vascular injury. Treatment principles.

A high-velocity knee dislocation is a true orthopedic emergency. A high index of suspicion is necessary to identify reduced knee dislocations in multiple trauma patients. Coexisting injuries are common in patients with high-velocity knee dislocations. After an initial assessment, the knee joint should be reduced immediately and splinted. Popliteal artery injury is common in high-velocity dislocations. Arteriograms can be helpful but should not delay operative treatment of obvious arterial injuries. Early restoration of flow to the ischemic limb will avoid amputation in most patients. The ortho

Changes in joint cartilage aggrecan after knee injury and in osteoarthritis.

OBJECTIVE: To determine the concentrations of aggrecan fragments in synovial fluid from patients with knee joint injury, osteoarthritis (OA), or acute pyrophosphate arthritis (PPA; pseudogout), and to test their relative reactivity with the 846 epitope, a putative marker of cartilage aggrecan synthesis. METHODS: Samples of knee joint fluid from 385 patients and 9 healthy-knee volunteers were obtained in a cross-sectional study. Study groups were acute PPA/ pseudogout (n = 60), anterior cruciate ligament (ACL) rupture (n = 159), meniscus lesion (n = 129), and primary knee OA (n = 37). The 846

MR imaging of knees having isolated and combined ligament injuries.

OBJECTIVE: Although clinical evaluation and MR imaging both accurately reveal injuries in knees with isolated ligament tears, physical examination becomes progressively less reliable when multiple lesions exist. We investigated the accuracy of MR imaging of knees having varying degrees and numbers of ligament injuries. SUBJECTS AND METHODS: We prospectively interpreted the MR images of 340 consecutive injured knees and compared these interpretations with the results of subsequent arthroscopy or open surgery, which served as the gold standard. Our interpretations of MR images focused on five s

[A foreign body in the knee joint--a very rare injury in children].

Cause of uncertain knee-complaints in children can be foreign bodies which penetrated unnoticed the joint cavity. Here we report an unusual injury of this kind. A glass-foreign body intruded the knee joint in childhood. Complaints occurred for the first time in adult age. The diagnostic and therapeutic measures in such injuries are shown. At removal of the foreign body tetanus immunisation is necessary.

Effect of Vascular Injury on Functional Outcome in Knees with Multi-Ligament Injury: A Matched-Cohort Analysis.

BACKGROUND: Multi-ligament knee injury (MLKI) associated with knee dislocation can result in vascular injury. The purpose of this study was to compare knee function after MLKI between patients with a vascular injury requiring popliteal artery bypass grafting and patients without vascular involvement. Additionally, factors associated with poor knee function in patients who had MLKI with vascular injury were evaluated. METHODS: This retrospective study identified patients with an MLKI between 1992 and 2014. Each patient who had a concomitant vascular injury requiring bypass grafting (vascular c

Posterolateral tibial plateau fractures are associated with anterolateral complex injuries of the knee in primary ACL injury.

PURPOSE: Posterolateral tibial plateau fractures (PLTFs) have been increasingly studied due to their association with soft tissue injuries around the knee. This prospective study aimed to determine whether the presence of PLTFs is associated with soft tissue injuries around the knee and correlates with an increased preoperative quantitative pivot shift (PS). It was hypothesized that the presence of PLTFs is associated with greater clinical and quantitative PS. METHODS: A prospective registry study was conducted, and patients who underwent primary unilateral anterior cruciate ligament (ACL) re
