Three hundred two low-risk obstetric patients with an unfavorable cervical examination and well-established gestational age of at least 287 days were randomly selected for management by either antepartum fetal testing or prostaglandin gel cervical ripening followed by aggressive induction of labor and delivery. The patients managed by induction of labor had a lower incidence of meconium-stained amniotic fluid, meconium aspiration, low Apgar scores, postmaturity syndrome, fetal distress, and cesarean delivery than did patients managed with antepartum fetal testing. Our data suggest that prostag

Obstetric data from 4403 nulliparas were analyzed in order to determine whether the duration of the second stage of labor influences perinatal outcome or maternal puerperal morbidity. No significant increase in the frequency of perinatal mortality, neonatal mortality, or low 5-minute Apgar scores was noted with long second stages. An increase in the incidence of low 1-minute Apgar scores was observed only in those infants who were not monitored. An increase in puerperal hemorrhage after more than 3 hours of second stage labor was attributable to those patients delivered by midforceps operation

Obstetric fistula formation is a catastrophic complication of prolonged obstructed labor. Obstetric fistulas are common in impoverished countries where access to maternal health care is poor. Although most fistulas can be closed successfully at the time of operation, a small number of women sustain such extensive pelvic injuries that their fistulas are irreparable. Some Western surgeons visiting African countries where fistulas are prevalent have become enthusiastic advocates of performing urinary diversions on these women, transplanting the ureters into the colon. We present a case study of o

BACKGROUND: Pelvic floor dysfunction refers to any combination of incontinence, overactive bladder, pelvic organ prolapse, and sexual dysfunction. Pelvic floor dysfunction affects approximately 25% to 30% of women and is linked to parity and age. Some obstetrical risk factors have been highlighted, though the second stage of labor has not been as thoroughly investigated. Allowing a longer second stage has been suggested as a method of reducing the rates of cesarean delivery in nulliparous women, though it has also been linked to pelvic floor injuries.
OBJECTIVE: This study aimed to determine

OBJECTIVE: To evaluate the outcome of active induction of labor for isolated oligohydramnios in low-risk term gestation.
METHODS: This retrospective study analyzed the obstetric and perinatal outcome of 412 singleton term pregnancies with cephalic presentation and no maternal risk factors or fetal abnormalities. Two groups were compared: 206 deliveries after induced labor for isolated oligohydramnios, and 206 deliveries matched for gestational age following spontaneous labor with normal amniotic fluid index.
RESULTS: The overall rate of cesarean deliveries and cesarean deliveries for nonreas

Childbirth is a dynamic process involving mutual adaptation between the maternal pelvis and the presenting fetal part. The ability of the pelvis to maintain optimal mobility during labor plays a crucial role in achieving favorable obstetric outcomes. The pubic arch angle (PAA) increases amplitude during pregnancy, showing pelvic tissue adjustment. The PAA evaluated with ultrasound in a single position predicts the risk of dystocia in labor and, consequently, anal sphincter trauma and incontinence after delivery. The hip flexion degree was found to reduce lumbar lordosis, shift the sacral promo

An obstetric fistula is a serious and debilitating complication of childbirth. It is an abnormal communication between the urinary tract or the gastrointestinal tract and the genital tract, produced by obstetric causes, usually prolonged and obstructed labor. The fetal head presses against the bony pelvis, creating a connection between the genital tract and the bladder or rectum, resulting in incontinence and, frequently, stillbirth. Factors such as early marital age, poverty, and limited access to obstetric care have perpetuated the high incidence of obstetric fistula in Ethiopia. Two Austral

OBJECTIVE: To describe the circumstances of occurrence and identify potential risk factors for obstetric fistula in northern Cameroon.
METHODS: A case series study of 42 obstetric fistula patients seeking services at the Provincial Hospital of Maroua, Cameroon, between May 2005 and August 2007. Structured interviews were conducted prior to surgical intervention.
RESULTS: Among obstetric fistula patients, 60% had lived with obstetric fistula for more than 5 years at the time of surgery. Eighty-one percent of patients had received no formal education and 86% were teenagers at their first deliv

OBJECTIVE: To study the association between bloody amniotic fluid (BAF) during labor and adverse pregnancy outcomes.
STUDY DESIGN: In the last 10 years we have implemented an institutional protocol that mandates obstetricians/midwives to report their subjective impression of the color of amniotic fluid (clear, meconium stained, bloody) during labor. The medical records, and neonatal charts of all singleton deliveries ≥ 370/7 weeks between 2008-2018 were reviewed. The cohort was divided into two groups: clear AF (Clear group) and BAF (BAF group). Cases with meconium stained AF were excluded. T

BACKGROUND: Friedman found in 1955 that most nulliparous women without epidural analgesia delivered within 2 hours in the second stage of labor. Estimating the probability of spontaneous delivery and assessing relevant predictors in a contemporary obstetrical population is essential for clinical decision-making.
OBJECTIVE: The aim of this study was to estimate the probability distribution of spontaneous delivery in occiput anterior and occiput posterior positions during the pushing phase, stratified by parity and epidural analgesia.
STUDY DESIGN: The study was a retrospective cohort study at

BACKGROUND: Epidural analgesia is a central nerve block technique achieved by injection of a local anaesthetic close to the nerves that transmit pain and is widely used as a form of pain relief in labour. However, there are concerns regarding unintended adverse effects on the mother and infant.
OBJECTIVES: To assess the effects of all modalities of epidural analgesia (including combined-spinal-epidural) on the mother and the baby, when compared with non-epidural or no pain relief during labour.
SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (31 Marc

OBJECTIVE: The purpose of this study was to determine whether introduction of Episcissors-60 into a labour and birth unit would decrease the incidence of obstetric anal sphincter injuries (OASIS).
METHODS: A before and after quality improvement study was conducted between April 1, 2020, and March 31, 2021. All patients who had a vaginal delivery were included in this study. All healthcare providers who perform vaginal deliveries were offered an educational session. Episcissors-60 were then introduced into a labour and birth unit. The primary outcome measure was the change in incidence of OASI

Persistant occiput posterior (OP) positions are the commonest malpresentations of the fetal head during labor and their diagnosis remains challenging. They are associated to prolonged second stage of labor, prolonged expulsive efforts, labor augmentation, cesarean sections and instrumental deliveries. On the maternal side, severe perineal tears, post-partum hemorrhage or chorioamnionitis are more frequent. Currently, prevention of persistent OP positions is based on the maintain of precise maternal positions. Several positions have been evaluated but only lateral position on the same side of t

BACKGROUND: Labor pain intensity is known to predict persistent postpartum pain, whereas acute postpartum pain may interfere with maternal postpartum physical, mental, and emotional well-being. Nevertheless, there is little research studying the association between labor pain intensity and acute postpartum pain. This study investigated the associations between labor pain intensity and psychological factors with acute postpartum pain.
METHODS: We included women with American Society of Anesthesiologists (ASA) physical status II, having ≥ 36 gestational weeks and a singleton pregnancy. We inves
