Advanced Neuroimaging of Tinnitus.

Although tinnitus may originate in damage to the peripheral auditory apparatus, its perception and distressing symptomatology are consequences of alterations to auditory, sensory, and limbic neural networks. This has been described in several studies, some using advanced structural MR imaging techniques such as diffusion tensor imaging. An understanding of these complex changes could enable development of targeted treatment. New MR imaging techniques enabling detailed depiction of the labyrinth may be useful when diagnosis of Meniere disease is equivocal. Advances in computed tomography and MR

[Deafness of infectious origin].

A large number of cases of deafness is due to infection. Both viral and bacterial agents can induce lesions of the middle ear, the inner ear and/or of the cochlear-vestibular nerve. The ear is reached through the Eustachian tube, the outer ear or the subarachnoid spaces. The characteristics of infection-induced hearing loss depend on the infectious agent and on the site of infection. We discuss successively the characteristics of deafness caused by infection of the middle ear, represented by acute and chronic otitis of common or mycobacterial origin, by infection of the inner ear with bacteria

Furosemide VOR test for the detection of endolymphatic hydrops.

In order to detect endolymphatic hydrops, we applied three neuro-otologic examination methods, including the furosemide VOR test, the glycerol dehydration test, and electrocochleography in 61 patients with unilateral Menière's disease, 21 patients with delayed endolymphatic hydrops, and 10 patients with syphilitic labyrinthitis. The positive rate of the furosemide VOR test in patients with endolymphatic hydrops was more than 50%. Inconsistencies were observed between the results of the furosemide VOR test, the glycerol test and/or electrocochleography. Therefore, combined test battery of these

Selective chemical vestibulectomy: preliminary results with human application.

The involved membranous labyrinth of patients with medically intractable Meniere's disease is being accessed by means of a fenestration of the bony horizontal semicircular canal in preparation for the application of a 125 or 250 mcg flake of solid streptomycin. Sensory information originating in the affected vestibular end organ is selectively blocked from reaching the central nervous system. This uniformly results in the rapid elimination of vertiginous symptoms, and at these calculated doses yields preservation of (or in some cases improvement in) preoperative cochlear thresholds. The ration

Micro-CT Analysis of Rodent Temporal Bones: Identifying Optimal Species for Otological Research.

OBJECTIVES: Rodents are used in most otological research studies and the choice of the most appropriate animal model may be crucial in studies of ear diseases and in the development of effective treatments. Here, we used micro-CT to compare temporal bone anatomy between four rodent model animals (guinea pigs, gerbils, rats, and mice) and humans, aiming to better characterize the anatomy of the inner and middle ear, and facial nerve to support informed animal model selection in otologic research. METHODS: We generated three-dimensional reconstructions and measured the various middle (tympanic

[Clinical diagnosis and treatment of chronic dizziness].

Chronic dizziness is defined as a complex of symptoms lasting months or years, including oscillopsia, nystagmus, and postural instability. Diagnostic search includes peripheral vestibulopathy - chronic unilateral vestibulopathy resulting from vestibular neuronitis, schwannoma of the 8th pair of cranial nerves, medical or surgical ablation of the labyrinth, and bilateral vestibulopathy; central vestibulopathy - syndromes accompanied by central vertical nystagmus, small vessel disease in patients with chronic cerebral ischemia; general somatic diseases, peripheral polyneuropathy, side-effects of

Hearing loss.

Nearly 30 million Americans have hearing loss. Audiograms are used to determine if hearing loss is sensorineural (SNHL), conductive (CHL), or mixed (MHL). SNHL indicates dysfunction of the cochlea, cochlear nerve, or brain. The study of choice for adults with SNHL is gadolinium-enhanced magnetic resonance imaging, which is used to look for acoustic neuromas, abnormal labyrinth signal intensity or enhancement, and brain disease. In children with SNHL, computed tomography (CT) performed with bone algorithms demonstrates inner ear dysplasias. SNHL from trauma is best evaluated with CT studies. CH

Maternal high-fat, high-sucrose diet-induced excess adiposity is linked to placental hypoxia and disruption of fetoplacental immune homeostasis in late gestation†.

Maternal excess adiposity during pregnancy is linked to placental malperfusion and inflammatory injury. Obesity-associated placental malperfusion may induce fetoplacental hypoxia, contributing to adverse health outcomes within and beyond the perinatal period. However, direct comparisons of tissue oxygen saturation at the uteroplacental interface in pregnancies complicated by excess adiposity are lacking. Using a mouse model of preconception high-fat, high-sucrose (HFHS) diet-induced excess adiposity, we found that both placental junctional and labyrinth zones at E17.5 were hypoxic compared to

Automated head motion system improves reliability and lessens operator dependence for head impulse testing of vestibular reflexes.

Deficiency of the eye-stabilizing vestibulo-ocular reflex (VOR) is a defining feature in multiple diseases of the vestibular labyrinth, which comprises the inner ear's sensors of head rotation, translation and orientation. Diagnosis of these disorders is facilitated by observation and measurement of eye movements during and after head motion. The video head impulse test has recently garnered interest as a clinical diagnostic assessment of vestibular dysfunction. In typical practice, it involves use of video-oculography goggles to measure eye movements while a clinician examiner grasps the subj

[Morphological features of experimental transient endolymphatic hydrops].

This paper describes a method for simulating transient endolymphatic hydrops based on the generation of vasomotor disorders in the labyrinthine artery area. The method is based on a deep short-term chloroethyl freezing of the lateral surface of the animal surface in the projection of the spinal artery. In histological terms, hydrops is characterized by two factors: vascular lesions (hyperemia of the vascular stripe, venous congestion) and changes in physico-chemical parameters of labyrinthine fluids (edema of cell elements of the labyrinth, membrane deformation, precipitation of the granular-r

Acute otomastoiditis and its complications: role of CT.

Acute bacterial (suppurative) otomastoiditis responds to antibiotic treatment; radiologic study is required only when there is clinical suggestion of coalescent mastoiditis, intracranial complications, or an underlying chronic disease. Computed tomography (CT) is the method of choice for evaluating otogenic intra- or extra-cranial complications. CT scans can show stages of disease progression when infection has spread by way of soft tissue, blood, and bone pathways into the dural venous sinuses, meninges, labyrinth, facial nerves, epidural and other intracranial spaces. When there is clinical

Recent and Frequent Vertigo Attacks Produce Negative Findings on Furosemide-Loading Vestibular Evoked Myogenic Potential Testing in Meniere's Disease.

Objective: The peak-to-peak amplitude of the p13-n23 wave in cervical vestibular evoked myogenic potential can increase after furosemide administration in patients with Meniere's disease [furosemide-loading VEMP (FVEMP) testing]. The examination is used to test for the presence of endolymphatic hydrops; we investigated factors that may influence the results. Methods: Forty-two subjects (23 males and 19 females, aged 24-70 years) with unilateral definite Meniere's disease who underwent FVEMP testing were retrospectively studied. Possible factors associated with the results of FVEMP testing were

Profound deafness. Associated sensory and neural degeneration.

The cases of deafness presented here have been selected primarily to illustrate the degree to which the acoustic ganglion has degenerated in profound or total deafness. Examples of seven types of acquired diseases of the inner ear and one type of probable congenital degeneration of uncertain cause are given. The number of surviving ganglion cells in each 5-mm section of the cochlea has been expressed as a percentage of the normal average count corresponding to that section of the organ of Corti. The state of the acoustic ganglion has been correlated with that of the organ of Corti and the peri

Menière's disease.

Menière's disease causes paroxysmal rotatory vertigo, due to endolymphatic hydrops, an accumulation of endolymph in the endolymphatic space of the labyrinth. Its major symptoms are attacks of rotatory vertigo lasting minutes to hours, with unilateral hearing loss, tinnitus and aural fullness. As the disease progresses, attacks happen less often, but hearing loss and tinnitus gradually become permanent. Neuro-otological complications may develop, such as benign paroxysmal positional vertigo, vestibular drop attacks and bilateral vestibulopathy. The diagnosis of Menière's disease is based on the
