Ribavirin for treating Lassa fever: A systematic review of pre-clinical studies and implications for human dosing.

Ribavirin is currently the standard of care for treating Lassa fever. However, the human clinical trial data supporting its use suffer from … regimens in clinical use are unlikely to reliably achieve serum concentrations required to inhibit Lassa virus replication. The results of this review highlight serious issues with the evidence, which, by today standards, would be unlikely to support the transition of ribavirin from pre-clinical studies to human clinical trials. Additional pre-clinical studi …

Lassa Fever: Critical Review and Prospects for Control.

Lassa Fever is a deadly viral haemorrhagic disease, causing annually several hundreds of deaths in West Africa. This zoonotic disease is primarily transmitted to humans by rodents of the genus Mastomys, even though other rodents reportedly carry the Lassa virus, while secondary interhuman transmission accounts for approximately 20% of cases. Although this disease has been endemic in rural zones of Nigeria, Sierra Leone, Liberfia, and Guinea for hundreds of years, it is also characterised by epidemic outbreaks in the dry season, responsible for heavy death tolls. No licensed vaccine or satisfyi…

Positivity Rate, Predictors, and Outcome of Paediatric Lassa Fever Disease (LFD) in a Lassa Fever Endemic State, South-East Nigeria.

BACKGROUND: The pattern and case fatality rate of Paediatric Lassa fever disease (LFD) is not well documented even in Lassa fever endemic communities. AIM AND OBJECTIVE: This prospective observational study was aimed at determining the pattern and outcome of Paediatric LFD. METHODOLOGY: A total of 183 children that met the criteria for LFD suspects were subjected to the Lassa virus PCR test. The suspects that tested positive were recruited into the study and a structured questionnaire was used to collect information on socio-demographics. RESULTS: Of the 183 LFD suspects that were tested, 2…

[Serological study of the virus responsible for hemorrhagic fever in an urban population of Cameroon].

A sero-epidemiological study of Hemorrhagic Fever Viruses in a urban population of Cameroon. The authors report the results of a sero-epidemiological survey undertaken in a urban population of Cameroon and concerning Congo, Rift (RVF), Lassa, Ebola, Marburg and Yellow Fever Viruses. On 375 human sera tested, 1.06% show antibodies against RVF virus and 1.87% are positive for anti-Ebola antibodies thus yielding evidence that these two viruses are present in this area of Cameroon. 33.75% have antibodies against Yellow Fever Virus as determined with an Hemagglutination-inhibition test. This quite

Vaccine platforms for the prevention of Lassa fever.

Lassa fever is an acute viral haemorrhagic illness caused by Lassa virus (LASV), which is endemic throughout much of West Africa. The virus primarily circulates in the Mastomys natalensis reservoir and is transmitted to humans through contact with infectious rodents or their secretions; human-to-human transmission is documented as well. With the exception of Dengue fever, LASV has the highest human impact of any haemorrhagic fever virus. On-going outbreaks in Nigeria have resulted in unprecedented mortality. Consequently, the World Health Organization (WHO) has listed LASV as a high priority p…

Animal Models of Lassa Fever.

Lassa virus (LASV), the causative agent of Lassa fever, is estimated to be responsible for up to 300,000 new infections and 5000 deaths each year across Western Africa. The most recent 2018 and 2019 Nigerian outbreaks featured alarmingly high fatality rates of up to 25.4%. In addition to the severity and high fatality of the disease, a significant population of survivors suffer from long-term sequelae, such as sensorineural hearing loss, resulting in a huge socioeconomic burden in endemic regions. There are no Food and Drug Administration (FDA)-approved vaccines, and therapeutics remain extrem…

Housing equity for health equity: a rights-based approach to the control of Lassa fever in post-war Sierra Leone.

Poor quality housing is an infringement on the rights of all humans to a standard of living adequate for health. Among the many vulnerabilities of those without adequate shelter is the risk of disease spread by rodents and other pests. One such disease is Lassa fever, an acute and sometimes severe viral hemorrhagic illness endemic in West Africa. Lassa virus is maintained in the rodent Mastomys natalensis, commonly known as the "multimammate rat," which frequently invades the domestic environment, putting humans at risk of Lassa fever. The highest reported incidence of Lassa fever in the world…

Enhanced treatment of Lassa fever by immune plasma combined with ribavirin in cynomolgus monkeys.

Lassa virus-infected cynomolgus monkeys were treated with Lassa virus-immune monkey serum containing a high concentration of neutralizing antibody, the antiviral drug ribavirin, or a combination of ribavirin plus immune serum at various times after infection. When treatment was initiated early (day 0 or 4), either ribavirin (30 mg/kg of body weight per day) or immune serum alone protected monkeys. However, when the initial treatment was delayed until day 7, only four of eight ribavirin-treated and only one of six serum-treated monkeys survived. Treatment with ribavirin combined with immune ser…

Lassa Fever: Epidemiology, Clinical Features, Diagnosis, Management and Prevention.

Lassa fever outbreaks West Africa have caused up to 10,000 deaths annually. Primary infection occurs from contact with Lassa virus-infected rodents and exposure to their excreta, blood, or meat. Incubation takes 2 to 21 days. Symptoms are difficult to distinguish from malaria, typhoid, dengue, yellow fever, and other viral hemorrhagic fevers. Clinical manifestations range from asymptomatic, to mild, to severe fulminant disease. Ribavirin can improve outcomes. Overall mortality is between 1% and 15%. Lassa fever should be considered in the differential diagnosis with travel to West Africa. Ther…

[Experimental Lassa fever in hamadryas baboons].

Intramuscular or aerogenic inoculation of baboons with Lassa virus reproduced a severe form of the disease which clinically, pathoanatomically and virologically resembles the severe form of human Lassa fever reported in the literature. The severity of monkey infection is demonstrated by rapid development of symptoms of general toxicity in the presence of fever, manifestations of hemorrhagic diathesis, high level of viremia and isolation of the causative agent from nasopharyngeal washings.

Preventive practices by households against Lassa fever transmission in Ondo State, Southwest, Nigeria.

INTRODUCTION: continued transmission of Lassa fever has been recorded in 6 Local Government Areas (LGAs) of Ondo State in Southwest, Nigeria annually with high case fatality rates. Genomic analysis of the Lassa virus has indicated ongoing transmission from local rodents´ population to humans despite several public health interventions during the outbreak including risk communication on preventive practices against the disease. We assessed adherence to preventive practices by household against the spread of Lassa fever in these affected LGAs. METHODS: a descriptive cross-sectional study was co…

Attenuated and lethal variants of Pichindé virus induce differential patterns of NF-kappaB activation suggesting a potential target for novel therapeutics.

Lassa virus pathogenesis is believed to involve dysregulation of cytokines. We have previously shown nuclear factor-kappaB (NF-kappaB) inhibition using a BSL-2 model for Lassa fever. Here we further define the potential mechanism for NF-kappaB inhibition as involving increased levels of repressive p50/p50 homodimers, and suggest a novel therapeutic strategy that acts via modulation of host signaling.

Human-monoclonal-antibody therapy protects nonhuman primates against advanced Lassa fever.

There are no approved treatments for Lassa fever, which is endemic to the same regions of West Africa that were recently devastated by Ebola. Here we show that a combination of human monoclonal antibodies that cross-react with the glycoproteins of all four clades of Lassa virus is able to rescue 100% of cynomolgus macaques when treatment is initiated at advanced stages of disease, including up to 8 d after challenge.

Modelling the effects of climate and human factor on Lassa fever distribution in Ondo State Nigeria.

Lassa fever is an acute viral haemorrhagic disease caused by the Lassa virus. Transmission to humans primarily occurs through direct contact with Mastomys rats or via the ingestion of food or usage of household items contaminated with the urine or faeces of infected rats. The MaxEnt algorithm was used to estimate the distribution of Lassa fever based on data on the incidence of the disease, ecogeographic features, and human factors. Principal component analysis (PCA) was used to mitigate multicollinearity among the environmental variables. The model's accuracy was evaluated using the area unde…

Antibodies against haemorrhagic fever viruses in Kenya populations.

Human sera from Lodwar (77 sera), Nzoia (841 sera), Masinga (251 sera), Laisamis (174 sera) and the Malindi/Kilifi area (556 sera) in Kenya were tested by indirect immunofluorescence for antibodies against Marburg, Ebola (Zaire and Sudan strains), Congo haemorrhagic fever, Rift Valley fever and Lassa viruses. Antibodies against Ebola virus, particularly the Zaire strain, were detected in all regions and were, over-all, more abundant than antibodies against the other antigens. Ebola and Marburg antibody prevalence rates were highest in the samples from Lodwar and Laisamis, both semi-desert area…
