Demystifying pediculosis: school nurses taking the lead.

The treatment of Pediculosis capitis, or head lice, is fraught with misinformation, myths, and mismanagement. Common myths include the need to exclude children from school, the need to remove all visible nits ("no-nit" policies), the need for massive environmental cleaning, that head lice live for long periods of time, and that schools are a common location for lice transmission. Head lice are a common childhood nuisance, causing embarrassment and emotional trauma in both children and families. This article explores and challenges the commonly held beliefs about the identification, management,

Does orthodontic treatment lead to gingival recession?

DATA SOURCES: Medline, Embase, The Cochrane Central Register of Controlled Trials (CENTRAL) and the reference lists of identified studies. STUDY SELECTION: Human controlled or randomised clinical trials and animal studies reporting occurrence of gingival recession and crown length changes were included. Case reports, descriptive studies, review articles, opinion articles and studies concerning impacted teeth or injured anterior teeth, pre-orthodontic treatment for dental restoration and those limited to severe periodontal diseases or craniofacial anomalies were excluded. Articles in any langu

Early pregnancy blood lead and spontaneous abortion.

Although evidence tends to suggest that high levels of lead exposure increase the risk of spontaneous abortion, we do not yet know whether moderate- to low-level exposure elevates risk. Among 351 women (aged 16 to 35 years, with single pregnancies) who were registered for a longitudinal study, 15 (4.3%) women experienced spontaneous abortion after the 12th week of gestation and before the 20th week. We collected participants' blood samples during the first trimester of pregnancy (8-12 weeks) for lead measurement by inductively coupled plasma-mass spectrometry. Mean ± standard deviation of bloo…

Lead-induced peripheral neuropathy following Ayurvedic medication.

Lead poisoning following intake of Ayurvedic medication is one of the recent areas of concern. We report a case of a 58-year-old type II diabetic man who was stable with diet control and 30 mg pioglitazone per day. He took Ayurvedic medication for generalized weakness and developed peripheral neuropathy following its intake. He was found to have high blood and urinary lead levels and was diagnosed to have subacute lead poisoning. He was treated with d-Penicillamine for 8 weeks, following which his lead levels became normal. The use of d-Penicillamine was proved highly effective in treating a c…

Does areca nut use lead to dependence?

BACKGROUND: The areca nut is consumed by approximately 10% of the world's population, and its consumption is associated with long-term health risks, with or without tobacco additives. However, it is not known whether its use is associated with a dependence syndrome, as is seen with other psychoactive substances. OBJECTIVE: To examine whether areca nut usage (with or without tobacco additives) could lead to the development of a dependence syndrome. METHODS: Three groups: [a] persons using areca nut preparations without tobacco additives [n=98]; [b] persons using areca nut preparations with to…

Persistent radicals of trivalent tin and lead.

In this report we present synthetic, crystallographic, and new electron paramagnetic resonance (EPR) spectroscopic work that shows that the synthetic route leading to the recently reported, first persistent plumbyl radical *PbEbt3 (Ebt = ethylbis(trimethylsilyl)silyl), that is, the oxidation of the related PbEbt3-anion, was easily extended to the synthesis of other persistent molecular mononuclear radicals of lead and tin. At first, various novel solvates of homoleptic potassium metallates KSnHyp3 (4a), KPbHyp3 (3a), KSnEbt3 (4b), KPbIbt3 (3c), and KSnIbt3 (4c) (Hyp = tris(trimethylsilyl)silyl…

Hemodynamic collapse during left ventricular lead implantation.

Hypotension as a procedural complication during cardiac resynchronization therapy (CRT) implantation was reported in the initial randomized clinical trials. However, this phenomenon is not well characterized. We reviewed our CRT implantation experience to better understand this issue. There were 105 patients who underwent left ventricular lead implantation for CRT. Four patients had marked hypotension (systolic blood pressure < or = 50 mmHg) during the procedure. All had a history of hypertension and diabetes mellitus and were pacemaker dependent. Two had normal renal function, one had moderat…

Lead accumulation in tidemark of articular cartilage.

OBJECTIVE: Determination of the spatial distribution of the toxic element lead (Pb) and other trace elements in normal articular cartilage and subchondral bone from adult humans with no history of work-related exposure to Pb. METHODS: Four macroscopically normal femoral heads and three patellas were harvested from randomly selected forensic autopsies. All subjects died of acute illnesses, had no history of work-related exposure to Pb and had no metabolic bone disease. The elemental distribution of lead (Pb) together with zinc (Zn), strontium (Sr) and calcium (Ca) in the chondral and subchondr…

Can rock climbing lead to Dupuytren's disease?

OBJECTIVES: To determine if rock climbing is a significant factor in the development of Dupuytren's disease in men, and, if so, what is the most likely related factor. METHODS: Questionnaires were distributed to all 1100 members of the Climbers' Club of Great Britain. These detailed basic information about the climber, the presence of Dupuytren's disease, and any known risk factors for its development. RESULTS: About half (51%) of the questionnaires were returned. The respondents were almost entirely male. It was revealed that 19.5% of male climbers had developed Dupuytren's disease. There w

Lead and other metals in gestational hypertension.

OBJECTIVES: To identify any effects that lead, calcium, magnesium, and zinc may have on the development of gestational hypertension. METHODS: Third-trimester 110 normotensive and 33 gestational hypertension cases were assayed for various metal levels, using standard techniques, including atomic absorption spectrometry. RESULTS: Gestational hypertension cases showed significantly higher blood lead levels than normotensives. No differences were noticed regarding calcium (ionized and total), magnesium, and zinc levels. Blood lead was significantly related with blood pressure, even after correct…

Minimally invasive removal of infected pacemaker lead.

A 37-year-old woman with sick sinus syndrome suffered complications with recurring local infection at the generator pocket. Repeated debridement and antibiotic therapy was ineffective. Several attempts to remove leads via the implantation vein by direct traction were unsuccessful. We operated using cardiopulmonary bypass and applied a minimally invasive lower ministernotomy to obtain pleasing cosmetic results. After a right atriotomy, leads were removed. The minimally invasive approach gave satisfactory results, especially cosmetically.

Paternal Occupational Lead Exposure and Pregnancy Outcome.

Self-reported reproductive histories of male employees of a lead-zinc smelter were related to pre-conception measures of lead exposure to examine associations between paternal occupational lead exposure and adverse pregnancy outcome. The participants reported 2,021 pregnancies which resulted in 1,684 normal live births, 12 stillbirths, 30 birth defects, 203 spontaneous abortions, and 92 "other" outcomes. Birth defects and stillbirths were combined for the analysis. The risk of a stillbirth or birth defect was elevated for pre-conception employment in a high-lead-exposure compared with a low-le…

The neurobehavioural toxicology and teratology of lead.

When comparing neurobehavioural observations from occupational lead-exposure of adults on the one hand, and environmental lead exposure of children on the other, it appears that the developing relative to the mature brain is more at risk. Neurobehavioural toxicity in occupational lead-exposure has typically not been observed at blood lead-concentrations (PbBs) below 400 micrograms/l, whereas ih environmentally exposed children such deficit has been reported to occur down to PbB of 100-150 micrograms/l and, perhaps, even below this range. Both cross-sectional and prospective studies have arrive…
