Primary tuberculosis of the fibular diaphysis: A rare case report.

INTRODUCTION: Primary diaphyseal tuberculosis has very low occurrence. With no systemic signs and specific radiographic features, there exists low index of suspicion, which may delay the diagnosis of tuberculosis. PRESENTATION OF CASE: A female aged 15 years presented with chronic leg pain and swelling for past 7 months. There was no significant history of tuberculosis present. On investigations ESR was 44 mm and positive mantoux test. Chest radiograph was normal. On x-ray (R) fibula intramedullary eccentric lytic lesion and on MRI (R) leg intramedullary lytic lesion was present suggestive of

Osseointegration of implants in irradiated bone with and without hyperbaric oxygen treatment: an experimental study in rat Tibiae.

PURPOSE: Hyperbaric oxygen (HBO) has been recommended to enhance implant osseointegration in irradiated bone. The aim of this study was to further investigate the effects of HBO on implant integration in irradiated bone tissue. MATERIALS AND METHODS: The present study was an experimental intraindividual study in 16 rats. A single fraction of 20 Gy external irradiation was applied to one rat hind leg, while the other served as a nonirradiated control. Three days after radiation, two implants were inserted in each tibial tuberosity. The rats were divided into two groups: non-HBO treated (group

A rare case of diplopodia and syndactyly: anatomical and surgical considerations.

Diplopodia is a rare congenital disorder that has not been extensively discussed in textbooks, and case reports appear to be the main source of information. Although the exact cause of diplopodia remains unknown, the presence of extra digits as well as metatarsals and tarsals allows it to be differentiated from pedal polydactyly. Syndactyly refers to the congenital fusion of the digits. Concomitant bilateral syndactyly and diplopodia is extremely unusual, and in this report we describe a case of right diplopodia and left polydactyly combined with bilateral manual syndactyly in a 15-year-old gi

Muscle and bone adaptations after treadmill training in incomplete Spinal Cord Injury: a case study using peripheral Quantitative Computed Tomography.

We describe the use of peripheral Quantitative Computed Tomography (pQCT) to identify musculoskeletal responses to partial body-weight supported treadmill training (BWSTT) in incomplete spinal cord injury (SCI). Long-term health consequences of SCI include extensive muscle atrophy, severe bone loss and an increased fracture risk in the affected limbs, mostly at both tibial epiphyses and the distal femoral epiphysis. Regular treadmill training may slow or reverse bone loss by recruiting available lower-limb musculature and loading the leg bones dynamically. The potential for detailed analysis o…

[Secondary hypertrophic osteoarthropathy. Differential diagnosis of post-traumatic ossification of the interosseous crural membrane].

The case of a 48-year-old patient is presented, suffering from persisting pain in the right lower leg for more than a year following a distortion of the ankle. The patient was known to have a history of severe asthmatic disease. Conventional X-rays showed a periostitis of the lower tibia and fibula. To exclude a malignant process, an open biopsy was performed. Neither tumor nor any histological signs of chronic inflammation were found. The patient claimed to have a similar pain in the contralateral lower leg 5 months after initial biopsy. X-rays again showed signs of a periostitis of the tibia

Patellar Tendon Load Progression during Rehabilitation Exercises: Implications for the Treatment of Patellar Tendon Injuries.

PURPOSE: This study aimed to evaluate patellar tendon loading profiles (loading index, based on loading peak, loading impulse, and loading rate) of rehabilitation exercises to develop clinical guidelines to incrementally increase the rate and magnitude of patellar tendon loading during rehabilitation. METHODS: Twenty healthy adults (10 females/10 males, 25.9 ± 5.7 yr) performed 35 rehabilitation exercises, including different variations of squats, lunge, jumps, hops, landings, running, and sports specific tasks. Kinematic and kinetic data were collected, and a patellar tendon loading index wa

[Multiple sites extrapodal actinomycetoma: Favorable outcome to treatment with a combination of cotrimoxazole and NSAI].

Mycetoma is a bacteriological or fungal infectious disease affecting the skin and/or soft tissues, which can be complicated by bone involvement. The most common feature is a tumor of the foot, but extrapodal localizations have been described. We report one case of a 47-year-old man who presented with tumefaction of a leg with multiple skin fistulae. Histopathological examination permitted to confirm the diagnosis of actinomycetoma and TDM showed the degree of bone and soft tissues involvement. Our case was characterized by the very inflammatory aspect of the tumor, its localization to the leg

The Efficacy of Denosumab in Treating Spinal Aneurysmal Bone Cyst: A Case Report.

The optimal treatment for aneurysmal bone cysts (ABCs) of the spine remains controversial. No treatment guidelines exist for the use of denosumab in aneurysmal bone cysts. In this report, we describe the results from a representative case and compare our experience with those of previously published reports. A 38-year-old male was referred for pain in the lower back and left leg. Radiographs and a needle biopsy specimen revealed a lumbar aneurysmal bone cyst, which was treated with denosumab chemotherapy. The pain in the lower back and left leg gradually improved, and at 16 weeks, the symptoms

Reconsidering uniform treatment: magnetic resonance imaging-based insights into the multiform pathophysiology of sprained ankles.

INTRODUCTION: Ankle sprains are among the most common musculoskeletal injuries, yet their clinical complexity is often underestimated. The relationship between ankle sprains and bone bruises in both the ankle joint and midfoot remains underexplored. At our institution, patients with moderate to severe ankle sprains routinely undergo magnetic resonance imaging (MRI) and receive conservative treatment with a short leg posterior cast splint. This study specifically focused on bone involvement assessed by MRI, rather than soft tissue or functional parameters. METHODS: We retrospectively reviewed

The effects of broiler chicken growth rate on valgus, varus, and tibial dyschondroplasia.

An experiment was conducted to test the hypothesis that the growth rate of broilers influences their susceptibilities to bone abnormalities, causing major leg problems. Leg angulations, described in the twisted legs syndrome as valgus and bilateral or unilateral varus, were investigated in 2 subpopulations of mixed-sex Arkansas randombred broilers. Valgus angulation was classified as mild (tibia-metatarsus angle between 10 and 25°), intermediate (25-45°), or severe (> 45°). Body weight was measured at hatch and weekly until 6 wk of age. There were 8 different settings of approximately 450 eggs

Quantitative Analysis of Lower Leg Adipose Tissue Distribution in Youth with Myelomeningocele.

Children with myelomeningocele have a high prevalence of obesity and excess fat accumulation in their lower extremities. However, it is not known if this is subcutaneous or intramuscular fat, the latter of which has been associated with insulin resistance and metabolic disorders. This study quantified lower leg bone, muscle, and adipose tissue volume in children with myelomeningocele, classifying adipose as subcutaneous or muscle-associated. Eighty-eight children with myelomeningocele and 113 children without myelomeningocele underwent lower leg computed tomographic scans. Subcutaneous and mus

Effects of long-term fixation on histological quality of undecalcified murine bones embedded in methylmethacrylate.

While long-term fixation and storage of specimens is common and useful for many research projects, it is particularly important for space flight investigations where samples may not be returned to Earth for several months (International Space Station) or years (manned mission to Mars). We examined two critical challenges of space flight experimentation: the effect of long-term fixation on the quality of mouse bone preservation and the preservation of antigens and enzymes for both histochemical and immunohistochemical analyses, and how the animal/sample processing affects the preservation. We s

Changes in composition and metabolic activity of the skeletal parts of the extremity of the adult rat following below-knee amputation.

Below-knee amputation in adult male rats resulted in characteristic quantitative and qualitative changes of the tibia stump and of the femur in the amputated leg. These changes were followed for periods of 1 to 32 weeks after the amputation. The femur of the amputated leg underwent a significant reduction of its total bone mass and a significant decrease of its percentage ash content, whereas its percentage water and organic contents increased significantly. The bone specific activity was initially decreased and began to increase after the 10th week post surgery with a positive linear regressi

Expanding occult intrasacral meningocele associated with diastematomyelia and multiple vertebral anomalies. Case report.

The authors report the case of a 7-year-old girl who presented with persistent low-back and left leg pain and was diagnosed with expanding occult intrasacral meningocele (OIM), diastematomyelia, tethered cord, and multiple vertebral anomalies. She was followed for 27 months. Removal of a bone spur and sectioning of the tight terminal filum did not relieve her left leg and back pain. Serial magnetic resonance (MR) imaging after initial detection of the OIM revealed gradual but significant enlargement of the lesion. Fifteen months after terminal filum release and bone spur resection, the OIM was

The Role of Ultrasound Imaging of Callus Formation in the Treatment of Long Bone Fractures in Children.

BACKGROUND: In the process of diagnosis and treatment of fractures, an X-ray study is typically performed. In modern medicine very important is the development of new diagnostic methods without adverse effects on the body. One of such techniques is ultrasound imaging. It has a high value in imaging most areas of the body, including the musculoskeletal system. Reports on the use of ultrasound in the evaluation of the callus are rare and this could be a method equivalent to or even better than standard radiographs. The aim of the study was to analyze the correlation of ultrasound with radiograph

Three cross leg flaps for lower leg reconstruction of Gustilo type III C open fracture.

A 60 year old male had Gustilo type III C open fracture of the right lower leg. After radical debridement, the large open defect including certain loss of the bone tissue was successfully augmented and covered, by consecutive three cross-leg flaps, which consisted of the free rectus abdominis musculocutaneous flap, the fibula osteocutaneous flap and the conventional sural flap. Although indication for amputation or preservation is decided with multiple factors in each case, a strategic combination of cross-leg flap, free flap, external fixation and vascular delay could increase the potential o
