Ultrasound biomicroscopy (UBM) is an invaluable investigation for imaging anterior segment structures. Although it is operator-dependent and time consuming, unlike optical-based imaging techniques, it is able to image structures posterior to the iris, such as the zonules, ciliary body and part of the pars plana. OBJECTIVE: This study aims to examine the characteristics and influencing factors of crystalline lens tilt and decentration in ultra-high myopic cataract patients, as measured by the CASIA2.
METHODS AND ANALYSIS: 60 eyes scheduled for cataract surgery with an axial length (AL) ≥ 28 mm were included. The IOLMaster700 was utilized to measure AL and the white-to-white (WTW) distance. The CASIA2 was employed to measure front curvature radius (FCR), crystalline lens tilt, and crystalline lens decentration. A 30-year-old male with Marfan syndrome underwent pars plana lensectomy and vitrectomy combined with silicone oil injection for the treatment of retinal detachment with proliferative vitreoretinopathy (PVR). Four months after vitrectomy, the silicone oil was removed using an anterior chamber maintainer (AC) and a self-sealing clear corneal incision at the upper temporal quadrant. Suprachoroidal hemorrhage (SCH) developed at the end of the operation. It was limited to the nasal side of the retina. Suprachoroidal hemorrhage remained to be limited and did not show progression.

By transscleral suture fixation, posterior chamber IOLs were implanted in 29 eyes (27 patients), including secondary implantation in 13 eyes 6 months to 10 years after intracapsular cataract extraction, fixation of dislocated IOL in 2 eyes, and primary implantation in 14 eyes with ruptured capsules due to complicated extracapsular extraction or traumatic cataract.
