We report two cases of a T cell lymphoproliferative disease not previously described, with cytologic and clinical features similar to those associated with Galton's "prolymphocytic" leukemia (PL). Our patients, like those with Galton's PL, had massive splenomegaly and minimal or absent hepatomegaly and lymphadenopathy. PURPOSE: Chronic lymphocytic leukemia (CLL) is the most common lymphoproliferative disorder in the western world. The involvement of the central nervous system (CNS) or the optic nerve in CLL, however, is rare. We report a case of a previously untreated patient with CLL whose first manifestation of the disease was a progressive visual loss caused by optic neuropathy.

Human T-cell lymphotrophic virus Type I (HTLV-I)-associated adult T-cell leukemia/lymphoma (ATL) is a relatively new clinical entity. The disease is endemic in southwestern Japan, the Caribbean basin, the southeastern United States and Africa. We report the identification of this disease in Israel and review previous cases of HTLV-I infection and ATL in this region. The disease was initially indolent and later clinically aggressive, characterized by hypercalcemia, osteolytic bone lesions, leukemic skin and organ infiltration and opportunistic infection.

We report here a case of adult T-cell leukemia (ATL) who presented acute renal failure and skin eruption. Renal and skin biopsies showed diffuse invasion of ATL cells. Furthermore, the surface phenotype of tumor cells taken from the bone marrow (BM) or peripheral blood (PB)(CD4-CD8-) differed from that of cells taken from the kidney or skin (CD4+CD8-). These findings suggested that CD4-CD8-ATL cells in the BM and PB had differentiated to CD4+CD8- cells in the kidney and skin.

A rare case of a 31-year-old woman is reported who had massive intraperitoneal bleeding caused by ovarian hemorrhage as the first manifestation of acute leukemia. Preoperative laboratory findings revealed severe anemia (Hb 6.6 g/dl) and thrombocytopenia (1.5 x 10(4)/mm(3)) but normal leukocyte count (3.9 x 10(3)/mm(3)). After surgery, blast cells were found in her peripheral blood and she was diagnosed with M0 type acute myeloid leukemia. In addition, histopathology revealed infiltration of leukemic cells in the resected ovary.

The authors describe a 6-year-old child, with a history of acute lymphoblastic leukemia, who presented with leukemic infiltration of the anterior segment of one eye mimicking anterior uveitis. After tissue diagnosis, administration of radiation therapy to the affected eye resulted in dramatic improvement of the condition. Involvement of the iris and anterior chamber is a recognized but unusual manifestation of leukemia. In the absence of systemic relapse, local radiation therapy without chemotherapy is highly effective, although the prognosis in these patients may be guarded.

BACKGROUND: The outcome of extramedullary infiltration (EMI) in pediatric acute myeloid leukemia (AML) is controversial, and little is known about the implications of stem cell transplantation (SCT) and gemtuzumab ozogamicin (GO) treatment on patients with EMI.
