The dissociative bond.

Dissociation leaves a psychic void and a lingering sense of psychic absence. How do 2 people bond while they are both suffering from dissociation? The author explores the notion of a dissociative bond that occurs in the aftermath of trauma--a bond that holds at its core an understanding and shared detachment from the self. Such a bond is confined to unspoken terms that are established in the relational unconscious. The author proposes understanding the dissociative bond as a transitional space that may not lead to full integration of dissociated knowledge yet offers some healing. This is exemp

Expanding stress generation theory: test of a transdiagnostic model.

Originally formulated to understand the recurrence of depressive disorders, the stress generation hypothesis has recently been applied in research on anxiety and externalizing disorders. Results from these investigations, in combination with findings of extensive comorbidity between depression and other mental disorders, suggest the need for an expansion of stress generation models to include the stress generating effects of transdiagnostic pathology as well as those of specific syndromes. Employing latent variable modeling techniques to parse the general and specific elements of commonly co-o

Triggering role of stressful life events in patients with alopecia areata.

Recent clinical studies examined the role of stress as a potential trigger for alopecia areata. However, it is still questioned whether and to what extent stress plays a role in its pathogenesis. We determined whether stressful life events are risk factors in the onset and course of alopecia areata among our population. The study included 61 consecutive patients with alopecia areata who were all outpatients at dermatology clinics of Semnan city. Sixty age- and sex-matched individuals selected among healthy subjects with respect to the diagnosis of alopecia areata served as controls. Stressful

Stress and the risk of multiple sclerosis.

OBJECTIVE: Several studies have shown that stressful life events are associated with a subsequent significant increase in risk of multiple sclerosis (MS) exacerbations. We wanted to study prospectively whether stress can increase the risk of developing the disease itself. METHODS: We studied 2 cohorts of female nurses: the Nurses' Health Study (NHS) (n = 121,700) followed from 1976 and the Nurses' Health Study II (NHS II) (n = 116,671) followed from 1989. The risk of MS after self-report on general stress at home and at work in the NHS in 1982 was studied prospectively using Cox regression. L

Child maltreatment and memory.

Exposure to childhood trauma, especially child maltreatment, has important implications for memory of emotionally distressing experiences. These implications stem from cognitive, socio-emotional, mental health, and neurobiological consequences of maltreatment and can be at least partially explained by current theories concerning the effects of childhood trauma. In this review, two main hypotheses are advanced: (a) Maltreatment in childhood is associated with especially robust memory for emotionally distressing material in many individuals, but (b) maltreatment can impair memory for such materi

Hommage to reactive psychosis.

When looking back in the history of psychiatry, the concept of reactivity had very variable impact on ethiological considerations. If classification of mental disorders is taken as a proxy, it seems that this impact is presently at the lowest that it has been in the last hundred years. The author gives a short historical overview of concept of reactive psychosis, followed by a description of the current state of the art. He pleads for a rethinking of the current position as stated in ICD-10. Reintroduction of a certain degree of the reactivity concept into the classification seems to be requir

Young women with type 1 diabetes' management of turning points and transitions.

The authors used grounded theory to explore and develop a substantive theory to explain how 20 young women with type 1 diabetes managed their lives when facing turning points and undergoing transitions. The women experienced a basic social problem: being in the grip of blood glucose levels (BGLs), which consisted of three categories: (a) the impact of being susceptible to fluctuating BGLs, (b) the responses of other people to the individual woman's diabetes, and (c) the impact of the individual women's diabetes on other people's lives. The women used a basic social process to overcome the basi

A goal-striving life event and the onset of hypomanic and depressive episodes and symptoms: perspective from the behavioral approach system (BAS) dysregulation theory.

On the basis of the behavioral approach system (BAS) dysregulation theory of bipolar disorder, this study examined the relation between occurrence of a BAS activation-relevant life event--goal striving--and onset of hypomanic and depressive episodes and symptoms. In particular, the authors examined the relation between preparing for and completing final exams (a goal-striving event) and onset of bipolar spectrum episodes and symptoms in college students with bipolar II disorder or cyclothymia (i.e., "soft" bipolar spectrum conditions). One hundred fifty-nine individuals with either a bipolar s

Domains of childhood teasing and psychopathological symptoms in Dutch adolescents.

This study examined relations between memories for childhood teasing and symptoms of social anxiety, depression, and eating disorders in 130 Dutch adolescents. Participants completed the Teasing Questionnaire-Revised which measures how much people recall having been teased during childhood, as well as the Social Anxiety Scale for Children, the Child Depression Inventory, and the Children's Eating Attitude Test. Analysis yielded significant positive correlations between self-reported recollections of childhood teasing and various symptoms. Furthermore, some evidence indicated specific domains o

Karojisatsu in Japan: characteristics of 22 cases of work-related suicide.

With the rapidly increasing number of work-related suicides in Japan (Karojisatsu, in Japanese), both applications for worker's compensation insurance and civil suits are proliferating. The phenomenon of work-related suicide is examined along with the process and related factors. With informed consent from bereaved families, two certified psychiatrists independently reviewed and summarized 22 insurance and legal reports filed by psychiatrists on employee suicides that were related to heavy workloads. A clinical epidemiologist participated in discussions with psychiatrists to reach a consensus

The nature and expression of social phobia: toward a new classification.

Social phobia (social anxiety disorder) was officially recognized as a distinct clinical entity only with the publication of the DSM-III in 1980. Research on the psychopathology of this disorder has revealed a great degree of heterogeneity in its symptomatology. In order to acknowledge this heterogeneity, later versions of the nosological system introduced the generalized subtype of social phobia to describe individuals who fear most or all social situations. The empirical literature on the differences between the diagnostic subtypes has yielded inconsistent findings. Based on the recent emoti

Stress and affective disorders in humans.

A consistent body of findings published over 30 years shows raised rates of life events before onset of clinical unipolar depression. A range of threatening events is implicated, with only limited specificity, although a somewhat closer relationship to interpersonal losses. Social support both buffers effects of life events and its absence also has some independent stressful effects. Life stress also affects outcome. Effects extend across the age range, but are weaker or absent in recurrent disorder, particularly when this is severe or melancholic. They are also weaker in bipolar disorder, whe

A correlational test of the relationship between posttraumatic growth, religion, and cognitive processing.

The present study examined the degree to which event related rumination, a quest orientation to religion, and religious involvement is related to posttraumatic growth. Fifty-four young adults, selected based on prescreening for experience of a traumatic event, completed a measure of event related ruminations, the Quest Scale, an index of religious participation, and the Posttraumatic Growth Inventory. The three subscales of the Quest Scale, the two groups of rumination items (soon after event/within past two weeks), and the index of religious participation were entered in a standard multiple r

Beliefs about overcoming psychological problems among British and Japanese students.

This study was part of a series investigating lay attributions for the cure of psychological problems. Three groups of students--Japanese students in Japan, Japanese students studying in England, and British students--completed a questionnaire and rated the perceived efficacy of 24 different strategies for overcoming each of 5 psychological problems: agoraphobia, depression, smoking cessation, lack of confidence, and obesity. Factor analysis of the curative strategies revealed 5 interpretable factors, the first 3 of which were Professional Help, Inner Control, and Understanding. There were num

The relationship between external events and affect states in older people.

Long-term and moderately short-term effects of bereavement and marriage on psychological well-being (PWB) among older people were investigated. The aspect of PWB that was examined was the prevalence of six affects, rated in terms of their frequency during the past year. Affect frequency of four groups was tested: Recently widowed, recently married, and widowed and married elders unselected for length of time in those marital statuses. As predicted, both length of time in the marital status and congruence between the positive event (marriage) and positive affect and between congruence of the ne

Illness narratives of persons with post-polio syndrome.

This qualitative study investigated the lifetime illness experience of individuals with the 'late effects' of polio or post-polio syndrome. Fifteen individuals were interviewed twice about their illness experience and the interviews were transcribed verbatim. The empirical material first underwent a categorization process. The preliminary categories generated through this analysis were then condensed into broader categories which in the final analysis gave rise to the following temporal pattern or stages of the illness experience: (1) the acute phase of polio and subsequent treatment and care;

Response of hostile individuals to stressful changes in their working lives: test of a psychosocial vulnerability model.

BACKGROUND: According to the psychosocial vulnerability model, the health of hostile individuals is at greater risk than that of non-hostile individuals, due partly to lesser ability of the former to cope with psychosocial stress situations. We examined whether hostile individuals were more vulnerable than others to health problems when faced with stressful changes in their working lives. METHODS: Hostility, sickness absence and psychosocial stressors in 866 local government employees over a 5-year period, which included severe economic decline, were investigated. Hostility was measured by a

Impact of life events on the onset of vitiligo in adults: preliminary evidence for a psychological dimension in aetiology.

The present study retrospectively examines the role of stressful life events in the onset of vitiligo in adults. A matched clinical sample of patients with other forms of disfigurement or skin disease (dystrophic epidermolysis bullosa and naevi, not thought to be associated with stress) served as a control group. Newly diagnosed patients and matched controls were thus asked to complete the 12-month version of the Schedule of Recent Experience, a questionnaire which measures the frequency and number of stressful life events occurring over a specified period. The results suggest that such patien

A case-control study was conducted on 90 cases of "probable" Alzheimer's disease and 97 controls at the same age as patients with dementia of the Alzheimer type. The patients were diagnosed according to the DSM IIIR criteria. Information was obtained on birth and childhood, medical and surgical history, exposure to various social, psychological and biological factors, family history of dementia. A structured questionnaire was based on AMDP-system. Some of the variables studied reached statistical significance: first of all unfavourable factors (especially poor livelihood) taking place during t
