The skin microbiome: impact of modern environments on skin ecology, barrier integrity, and systemic immune programming.

Skin barrier structure and function is essential to human health. Hitherto unrecognized functions of epidermal keratinocytes show that the skin plays an important role in adapting whole-body physiology to changing environments, including the capacity to produce a wide variety of hormones, neurotransmitters and cytokine that can potentially influence whole-body states, and quite possibly, even emotions. Skin microbiota play an integral role in the maturation and homeostatic regulation of keratinocytes and host immune networks with systemic implications. As our primary interface with the externa

Integrating quality of life instruments into clinical care for benign gynecological conditions to enhance patient-centered care.

The quality-of-life (QoL) impacts of benign gynecological conditions, such as polycystic ovarian syndrome (PCOS), endometriosis, and uterine fibroids on patients' lives are underestimated. Upon analysis of the 36-Item Short Form Health Survey questionnaire (SF-36), which assesses physical functioning, role physical, bodily pain, general health, vitality, social functioning, role emotional, and mental health, benign gynecological conditions reveal comparable or worse QoL scores compared to QoL scores in other chronic conditions such as diabetes mellitus, breast cancer, and heart disease. The cu

Examining Practices for Youth Illegal Sexual Behaviors which May Warrant De-Implementation: Overview and Recommendations.

Youth problem sexual behaviors (PSBs), and particularly youth illegal sexual behaviors, have deleterious consequences for youth who engage in them, including removal from the home, perpetration of non-sexual offenses, and chronic involvement with the child welfare and juvenile legal systems (Lussier, Juvenile sex offending through a developmental life course criminology perspective: An agenda for policy and research). Further, individuals who are victims of youth PSBs also experience poor mental and physical health outcomes, resulting in a significant public health expenditure (Hailes et al.,

Systematic Development of National Guidelines for Obesity Care: The Swedish Approach.

INTRODUCTION: With the rapid development of treatment modalities for obesity management, there is an increasing demand for guidance to facilitate the prioritization of interventions. In 2020, the Swedish National Board of Health and Welfare started the process of producing the first national guidelines for obesity care directed to decision makers who allocate resources to the best knowledge-based care. The main aim of this paper was to describe the systematic development of these guidelines, designed to guarantee uniformly high standards of care throughout the whole country. METHODS: The stan

German Cardiac Arrest Registry: rationale and design of G-CAR.

BACKGROUND: In Germany, 70,000-100,000 persons per year suffer from out-of-hospital cardiac arrest (OHCA). Despite medical progress, survival rates with good neurological outcome remain low. For many important clinical issues, no or only insufficient evidence from randomised trials is available. Therefore, a systemic and standardised acquisition of the treatment course and of the outcome of OHCA patients is warranted. STUDY DESIGN: The German Cardiac Arrest Registry (G-CAR) is an observational, prospective, multicentre registry. It will determine the characteristics, initial treatment strateg

Comparison between radiography and magnetic resonance imaging for the detection of sacroiliitis in the initial diagnosis of axial spondyloarthritis: a cost-effectiveness study.

OBJECTIVE: The purpose of our study was to determine the cost-effectiveness of radiography and MRI-based imaging strategies for the initial diagnosis of sacroiliitis in a hypothetical population with suspected axial spondyloarthritis. MATERIALS AND METHODS: A decision analytic model from the health care system perspective for patients with inflammatory back pain suggestive of axial spondyloarthritis was used to evaluate the incremental cost-effectiveness of 3 imaging strategies for the sacroiliac joints over a 3-year horizon: radiography, MRI, and radiography followed by MRI. Personality as a Social Process: where Peter Giordano Meets Boris Parygin.

In this paper I comment on the "Individual personality is best understood as process, not structure: A Confucian-inspired perspective" article by Peter Giordano (Culture & Psychology, 23(4), 502-518 (2017)), which addresses the question of how to comprehend a personality which is continuously changing and varying with changes in social contexts and situations. The issue which Giordano turns up I believe to be of great importance and topicality in the quickly changing globalizing contemporary world. Giordano's paper highlights an important problem in the development of contemporary personality

Adverse Childhood Experiences and Higher-Level Functional Limitations Among Older Japanese People: Results From the JAGES Study.

BACKGROUND: A life-course perspective is essential in understanding the determinants of higher-level functional limitations. We examine the impact of adverse childhood experiences (ACEs) on higher-level functional limitations in older people. METHODS: Data were from the Japan Gerontological Evaluation Study 2013, a population-based cohort of independent people aged 65 years or older across Japan (n = 19,220). ACEs before the age of 18 were assessed in terms of seven adversities: parental death, parental divorce, parental mental illness, family violence, physical abuse, psychological neglect,

Economic impact of using clodronate in the management of patients with multiple myeloma.

The economic impact of using prophylactic clodronate as an adjunct to chemotherapy in the management of multiple myeloma for the first 4 years following diagnosis was established from the perspective of the National Health Service (NHS). A state-transition model of the course of multiple myeloma was constructed using the MRC VI myelomatosis trial results and information on patient management obtained retrospectively from clinical trialists. Data were collected on resource use and corresponding costs for standard management and managing severe hypercalcaemia, vertebral and non-vertebral fractur

Autism as a Predictor of Deviant Peer Association: Testing for Dual Systems Model Mediation Effects.

PURPOSE: Deviant peer association is a robust risk factor for offending, that is, any behavior that violates a criminal law. This can include behaviors like drug use, violence, and theft; among others. Autism is a neurodivergent condition characterized by both atypical cognitive development and challenges with social interaction and communication. Despite these impairments, there is a dearth of research focused on the relationship between autism and deviant peer association. Relatedly, there exists no research examining the role that dual systems model constructs (sensation-seeking and impulse

From Metabolic Syndrome to Cardiovascular-Kidney-Metabolic Syndrome (CKM): A Clinical and Pathophysiological Continuum.

Cardiovascular-kidney-metabolic (CKM) syndrome has emerged as a broader clinical and pathophysiological framework than traditional metabolic syndrome, addressing the limitations of a purely factor-clustering approach by integrating dysfunctional adiposity, metabolic dysfunction, chronic kidney disease, and cardiovascular disease within a dynamic multiorgan continuum. This narrative review critically examines the transition from metabolic syndrome to CKM syndrome, emphasizing why the newer framework may better reflect real-world cardiorenometabolic multimorbidity and provide more clinically mea

Stayers, movers and the in-between: life course patterns of labour market attachment and mobility among rural Swedish youth.

Structural disparities between rural and urban labour markets delay labour market entry for rural youth and risk long-term scarring related to both labour market integration and earnings. Migration is often viewed as a strategy to avoid these disadvantages. Using a life course perspective, this study examines how mobility and labour market attachment co-evolve across two decades of adulthood. Based on Swedish register data for cohorts born between 1977 and 1982 (n = 42,913) and sequence analysis with fuzzy clustering, we identify four long-term trajectories: stayers with strong attachment; sta…

Dilemma of Thrombolysis in a patient with high-risk Pulmonary Embolism with severe Thrombocytopenia: A case report.

BACKGROUND: Managing a high-risk pulmonary embolism (PE) in a critically ill patient with severe thrombocytopenia can present a challenging dilemma. There is a high risk of fatal bleeding due to anticoagulation in high-risk PE with thrombocytopenia; therefore, risks and benefits are balanced while dealing with such a critical scenario. CASE REPORT: We present a case of a female patient with thrombocytopenia who was admitted for management of lymphoma. Her hospital course was complicated by high-risk PE, leading to acute respiratory failure and hypotension, necessitating urgent transfer to the

Parental and peer influence on STEM career persistence: From higher education to first job.

This article investigates social influences that drive gender-specific differences in the degree of persistence individuals exhibit in regard to pursuing science, mathematics, engineering and technology (STEM), both as a field of study and as an occupation. It covers individuals' careers from entry into higher education to entry into the labor market. Following a life course perspective, I ask the following questions: (1) How stable are preferences regarding STEM subjects and occupations throughout young adulthood? (2) Are significant social ties, such as relations with friends and family memb…

Participant-Generated Timelines: A Participatory Tool to Explore Young People With Chronic Pain and Parents' Narratives of Their Healthcare Experiences.

Visual methods are becoming more evident in health research. Timeline drawings have been used as a participatory tool alongside interviews in life course research. In this article, we describe how a method involving timeline generation can explore patient experiences along a treatment continuum. Grounded in previously published evidence and using specific examples from two studies exploring the experiences of young people treated for chronic pain, we outline the key components of this method. Moreover, we highlight the flexibility of its application and the importance of using a person-centere
