Experiences and support needs of patients receiving home mechanical ventilation and their caregivers: a qualitative meta-synthesis.

BACKGROUND: Home mechanical ventilation (HMV) is an important form of support for the out-of-hospital management of patients with chronic respiratory failure and long-term ventilatory dependence. It plays an important role in prolonging life, relieving symptoms, and supporting home living. As the site of treatment extends from the hospital to the home, patients and caregivers must jointly face challenges related to treatment decision-making, technological adaptation, expansion of caregiving responsibilities, and insufficient support systems. Existing studies have often focused on a single dise

Multidisciplinary nutritional support for undernutrition in nursing home and home-care: A cluster randomized controlled trial.

OBJECTIVE: To assess the effect of multidisciplinary nutritional support for undernutrition in older adults in nursing home and home-care identified with the validated Eating Validation Scheme (EVS). METHODS: An 11 wk cluster randomized trial with a home-care (3 clusters) or nursing home (3 clusters) setting as the unit of randomization. Before starting the study, a train-the-trainer course was performed to educate the nutrition coordinators. In addition to the nutrition coordinator, the participants assigned to the intervention group strategy received multidisciplinary nutrition support. Foc

Experiences of care partners and residents with the Long-Term Care Palliative Toolkit during the COVID-19 pandemic: A multiple methods study.

BACKGROUND: With a large burden of suffering and death in 2020 due to COVID-19 in long-term care (LTC) homes resulting in restrictions of visitations, there is a need for a formal virtual intervention to support families/friends (i.e., care partners) and residents around palliative care, including planning for end-of-life when outbreaks like these occur. The LTC Palliative Toolkit includes informational resources for care partners, residents, and healthcare providers about the trajectory of life-limiting chronic illnesses (i.e., frailty, dementia, heart failure, kidney disease, lung disease) a

What Constitutes High-Quality Paediatric Palliative Care? A Qualitative Exploration of the Perspectives of Children, Young People, and Parents.

BACKGROUND: Globally, over 21 million children need palliative care each year. Although guidelines exist to support paediatric palliative care delivery, they are not informed by the experiences of children themselves. OBJECTIVE: We aimed to determine what constitutes good quality palliative care from the perspectives of children with life-limiting or life-threatening conditions and their parents. METHODS: We analysed semi-structured qualitative interviews using reflexive thematic analysis informed by the European Association for Palliative Care charter of palliative care for children and you

[Efficacy of the Paediatrics Palliative Care Team of Murcia according to the experience of the parents].

INTRODUCTION: The care at the end of children's lives must be sensitive to the needs of the child and their family. An understanding of the illness is required from the perspective of parents faced with the death of their child, in order to improve quality and guide the development of end-of-life care in Paediatrics. METHOD: A retrospective observational study was conducted between June 2014 and June 2017 using a questionnaire, to assess the needs, experiences, and satisfaction with the care received, from a sample of parents who lost a child due to a foreseeable cause. Three different study

Attitudes toward advance care planning among people with dementia at different stages of the disease: A qualitative study.

BACKGROUND: Although advance care planning improves end-of-life care, participation is low. For dementia, attitudes must be studied by disease stage to capture changes and better understand patient engagement. PURPOSE: To explore how people with mild-to-moderate dementia described their attitudes toward advance care planning following diagnosis, during disease progression, and when anticipating advanced dementia and end-of-life circumstances. METHODS: This qualitative study used maximum variation purposive sampling to recruit 18 people with dementia from the neurology department of a tertiar

Palliative Care Throughout the Entire Perioperative Pathway in Cardiovascular Surgery.

Cardiovascular surgery has substantially improved survival and quality of life for patients with cardiovascular disease, yet contemporary surgical populations are increasingly older, multimorbid, and affected by advanced disease, rendering procedures and perioperative care more complex. Patients often experience severe physical symptoms, psychological distress, and difficult decisions, which in cases of complications also include escalation, limitation, or withdrawal of life-sustaining treatment. In this article, we argue that palliative care offers a framework to address these complex needs t

Veno-venous extracorporeal life support for blastomycosis-associated acute respiratory distress syndrome.

BACKGROUND: Blastomyces is a dimorphic fungus endemic to regions of North America, which can lead to pneumonia and fatal severe acute respiratory diseases syndrome in up to 89% of patients. Extracorporeal life support can provide adequate oxygenation while allowing the lungs to rest and heal, which might be an ideal therapy in this patient group, although long-term clinical and radiological outcomes are not known. CLINICAL FEATURES: We report on five consecutive patients admitted to Toronto General Hospital intensive care unit between January 2012 and September 2016, with progressive respirat

Optimizing hospice care: A scoping review of staffing strategies to enhance practice.

BACKGROUND: Strategic hospice staffing is essential to optimize patient care and strengthen workforce performance and retention, particularly as aging populations and rising rates of life-limiting illnesses increase the demand for end-of-life care. Despite growing recognition of staffing challenges in hospice care, there is limited synthesis of evidence on effective models and practices that support high-quality, sustainable care delivery. OBJECTIVE: This scoping review aimed to synthesize evidence on workforce characteristics and strategies that support sustainable, high-quality hospice care

Integration of rehabilitation and palliation for people with severe lung disease or heart failure - a scoping review.

Traditionally, rehabilitation and palliative care are delivered as separate services, where rehabilitation focuses on regaining physical functions and palliative care on symptom relief and existential support. There is growing recognition of the potential benefit in integrating these approaches. This scoping review explores how integration or coordination of rehabilitation and palliative care is described in the literature, with a focus on heart failure, chronic obstructive pulmonary disease, and interstitial lung disease. A systematic search was conducted in MEDLINE, EMBASE, and CINAHL, ident

Providing End-of-Life Care in the Community: What Are the Challenges in Malaysia?

A 72-year-old woman with metastatic lung cancer to bone and brain and with left external iliac vein thrombosis was under the care of a community palliative care provider. She experienced an acute pain crisis due to acute limb ischemia of the left lower limb. Goals-of-care discussions were held with the patient and her family; she prioritized symptom control and end-of-life care at home. The family and patient were aware of her short prognosis. Her complex pain was managed by the community palliative team, and her family was empowered to give subcutaneous injections. We illustrate a case showin

'You constantly have to be switched on': A qualitative interview study of parents of children with STXBP1-related disorders in the Netherlands.

BACKGROUND: Disorder-related variants in the STXBP1 gene are increasingly detected in children with severe developmental disorders. It is commonly acknowledged that developmental disorders significantly impact family life, but little is known about the day-to-day experiences of caregivers living with children with STXBP1-related disorders (STXBP1-RD). This knowledge gap may hinder researchers and care professionals from aligning research activities, care, and support with the perspectives of parents. METHODS: We conducted a semi-structured interview study to gain a better understanding of the

Economic Evaluation of Exercise-Based Cardiac Rehabilitation Programs for Chronic Heart Failure Patients in Colombia.

PURPOSE: Cardiac rehabilitation (CR) in patients with chronic heart failure (CHF) has met with resistance from third-party payers in low- and middle-income countries because of lack of evidence regarding its cost-effectiveness. We aimed to provide information to help better inform this decision-making process. METHODS: Costs associated with a 12-week exercise-based rehabilitation program in Colombia for patients with CHF were estimated. We collected data on all medical resources used in ambulatory care and data on hospital costs incurred for treating patients with uncompensated CHF. A literat
