Nodular transformation (nodular "regenerative" hyperplasia) of the liver. A clinicopathologic study of 30 cases.

Nodular transformation, a rare hyperplastic condition of the liver, has been reported in patients with rheumatoid arthritis, Felty's syndrome, the CRST syndrome, and myeloproliferative disorders. Associated disorders in the present clinicopathologic study of 30 cases included the foregoing as well as extrahepatic neoplasms, endocrine disorders, and other diseases producing immune dysfunction; some patients had received drug therapy for a prolonged time. Clinically nodular transformation may be confused with cirrhosis; histologic evaluation of liver biopsy material is essential for diagnosis. C

Invasive thymoma with intraorbital metastases.

Thymomas are epithelial neoplasm of thymus and most common primary neoplasm of anterior and superior mediastinum affecting males and females equally. It occurs usually in the fifth to seventh decade. Approximately one-third of thymomas are invasive. Metastases to distant extrathoracic sites such as the liver, lung, lymph node, kidneys, ovary and brain occur infrequently. This is more common with invasive thymomas. Although brain has been shown to be a site of infrequent metastases, intraorbital metastases has not yet been reported. Here we report one such case of invasive thymoma of anterior s

[Retroperitoneal tumors: a case of liposarcoma].

The Authors report a case of retroperitoneal sarcoma, in the histological subtype "liposarcoma", occasionally diagnosed in a 57 year old woman, who presented aspecific symptoms, characterized by dry cough, vomiting and chest pain. Chest TC showed a neoplasm of the retroperitoneum, close to liver, right colon, duodenum and right kidney. The arteriography of the right renal artery showed a neovascolarization inside the mass. A surgical resection was performed. The histological examination confirmed the diagnosis of liposarcoma. Liposarcomas account for 30% of the retroperitoneal soft tissue sarc

Mixed hepatoblastoma in an adult.

We report a case in an elderly adult of a highly malignant liver tumor with blastoid features that resembled hepatoblastoma. A liver tumor with a diameter of 23 cm was removed in a 78-year-old woman. The tumor showed highly differentiated epithelial hepatocellular and poorly differentiated epithelial and mesenchymal components. The blastoid nature and pluripotent differentiation potential were supported by immunohistologic analysis and suggest an origin of a poorly differentiated pluripotent hepatic cell with the potential to mature. We believe that this case of a mixed hepatoblastoma in an ad

Three cases of adrenocortical tumors mistaken for hepatocellular carcinomas/diagnostic pitfalls and differential diagnosis.

Adrenocortical adenomas and carcinomas in other parenchyma are extremely rare, with few cases reported and because of the rarity of these tumors, they occasionally cause problems during diagnosis. Adrenal cortical neoplasms in liver parenchyma can be present in 3 forms, including direct invasion or adhesion to liver parenchyma, tumors arising in adrenohepatic fusion tissue or in ectopic adrenal gland tissue. We report 3 cases of adrenal cortical tumors that were misdiagnosed as hepatocellular carcinoma in the preoperative state. The first case involved an adrenocortical adenoma arising in adre

[Hepatic diseases caused by drugs].

Drug-induced hepatotoxicity covers the clinical and pathological expressions of almost any acute or chronic liver disease. Liver damage is due to intrinsic toxicity of the drug (paracetamol) and/or immunoallergic mechanisms (halothane). Acute injury may be cytotoxic or cholestatic. Cytotoxic injury is characterized by necrosis (paracetamol, halothane) or steatosis (valproate). Cholestatic injury can be associated with immune-mediated portal inflammation (chlorpromazine) or solely attributed to inhibition of transport systems (cyclosporin A). Chronic drug-induced disorders include chronic activ

[Changes in the use of percutaneous liver biopsy over time].

AIM: To study, through the pathology diagnosis obtained, the current use of percutaneous liver biopsy and its change with time. MATERIALS AND METHODS: Pathology diagnosis of 759 liver biopsies performed during last 5 years are reviewed and compared with our previously published experience. RESULTS: Neoplasms have decreased from 16% of all pathologic diagnosis obtained in 1961-81 to less than 1% in the last 5 years. In 1985-87, liver cirrhosis was the final diagnosis in 60% of all biopsies, while currently it is only 18%, with a steady absolute number of cirrhosis each year. The most remarkab

[Hepatic and Peritoneal Histoplasmosis Simulating Another Pathology].

Histoplasmosis is the most frequent endemic systemic mycosis in Argentina. Most cases are asymptomatic or present mild respiratory manifestations, which resolve spontaneously; however, occasionally they present severe clinical forms when immunity is compromised. Although liver involvement in disseminated infection is well known, focal hepatic histoplasmosis is extremely rare. We present the case of a patient with focal granulomatous hepatitis and peritoneal involvement due to Histoplasma capsulatum simulating a primary liver neoplasm, who underwent surgery.

Rare case report: Liver metastasis from cervical adenocarcinoma presenting with cystic mass and obstructive jaundice.

Liver metastases can originate from primary tumors in various organs; however, metastasis from cervical cancer to the liver is rare. Cervical cancer patients with distant metastases have a poor prognosis and reduced survival rates. This report describes a case of a cystic liver mass with obstructive jaundice, observed four years after resection of cervical adenocarcinoma. The lesion lacked typical imaging characteristics of hepatic metastases and was initially suspected to be a biliary neoplasm. A contrast-enhanced, ultrasound-guided needle biopsy was performed to confirm the diagnosis. Histop

Selective Internal Radiation Therapy for Liver Metastasis of Papillary Thyroid Carcinoma.

Papillary carcinoma of thyroid (PTC) is the most common neoplasm of the thyroid gland that usually invades lymphatics rather than blood vessels. Hematogenous metastasis of PTC is rare, and distant metastasis to the liver is even rarer. Selective internal radiation therapy with Y microspheres is a well-recognized technique in the treatment of liver metastases. Herein, we present a case of PTC with radioactive iodine-negative hepatic metastasis detected by F-FDG PET/CT and treated with selective internal radiation therapy.

[Squamous cell carcinoma of gallbladder].

Adenocarcinoma is the most common malignant neoplasm of the gallbladder, but squamous cell carcinoma (SCC) is rare with an incidence of 1.4 approximately 3.3%. We present a recent case of a 63-year-old man complaining of abdominal distention. Preoperative US and CT revealed a large tumor of the gallbladder infiltrating the liver and transverse colon. Cholecystectomy, subsegmental resection of the liver, lymph node dissection, and partial resection of the transverse colon were performed. The resected specimen was histologically diagnosed as SCC without nodal metastases.

Primary multicystic undifferentiated embryonal sarcoma of the liver in an adult presenting with peripheral eosinophilia.

Primary undifferentiated embryonal sarcoma of liver is an extremely rare pathologic entity in the adult population. An unusual case is reported of a 44-year-old female who presented with multiple hepatic cysts and peripheral eosinophilia, initially thought to be hydatid disease of the liver. Angiogram revealed neovascularization. Surgical biopsy showed an anaplastic mesenchymal neoplasm. Pathologic findings are presented and the relevant literature reviewed. Hepatic undifferentiated embryonal sarcoma should be included in the differential diagnosis of eosinophilia accompanying hepatic cysts.

A rare cystic and papillary variant of acinar cell carcinoma with liver metastasis and molecular analysis.

Acinar cell carcinoma (ACC) is a pancreatic neoplasm with histologic variants that pose significant diagnostic challenges. We report a 75-year-old man with a previously unreported cystic and papillary ACC metastatic to the liver. Histologically, the tumor exhibited cystic and papillary proliferations of epithelial cells with eosinophilic cytoplasm, prominent nucleoli, and frequent mitoses. Immunohistochemical analysis showed positivity for trypsin and BCL-10, confirming ACC. The cyst lining displayed both acinar and ductal features, suggesting origin from acinar cystic transformation with acin

Pediatric Liver Tumors: Updates in Classification.

Malignant primary liver tumors are rare in children. Yet a wide histologic spectrum is seen, particularly in hepatoblastoma, the most common malignant liver tumor in children. Furthermore, there can be significant morphologic overlap with hepatocellular carcinoma, the second most common pediatric liver malignancy, and tumors with hybrid features of hepatoblastoma and hepatocellular carcinoma are also reported (currently placed in the provisional category of malignant hepatocellular neoplasm, not otherwise specified). This review provides detailed morphologic descriptions and updates in the evo

Carcinoid syndrome from a carcinoid tumor of the pancreas without liver metastases: A case report and literature review.

A carcinoid tumor of the pancreas (CTP) is a rare pancreatic neoplasm, and usually presents with carcinoid syndrome (CS). CS consists of the classic symptom triad of cutaneous flushing, diarrhea and valvular disease, and occurs in the majority of patients with liver metastases. In the present study, the patient presented with symptoms of CS. A diagnosis of CTP with CS was suspected due to high levels of urine 5-hydroxyindolacetic acid, and this was confirmed by a fine-needle aspiration biopsy. Computed tomography showed extended lymphadenopathy, but no liver metastases. The patient was managed

[Hydatoid cysts of liver--diagnostic and therapeutic problem].

In the Department of General Surgery of Medical University 11 patients with hydatid cysts of liver were operated. Ultrasonography and computer tomography allowed to define exact localization, size and contents of the cysts, what made possible planning of operating procedure. In 2 cases nonanatomic resection of liver parenchyma was performed, in 2 ones with coexisting stomach neoplasm just excision of the frontal wall of the cyst and emptying of the interior. In all the other cases the cysts were removed with surrounding liver tissue. Late results of surgical treatment with albendazol therapy a

[Solid pseudopapillary tumor of the pancreas. A report of 3 cases and a review of the literature].

Solid pseudopapillary tumours of the pancreas (SPTP) are a distinct clinico-pathological entity that differs from the other cystic pancreatic neoplasms in the young age of onset, the almost exclusive incidence in the female sex and the low degree of malignancy. SPTP is a rare neoplasm that has shown a progressive increase of incidence, passing from 0.17%-2.7% of all exocrine tumours of the pancreas in the 1980's, to 6% in recent reports in 2003. In addition, it accounts for about 5% of cystic neoplasms of the pancreas. With the present paper, in the world literature, updated to August 2005, 88

Uterine tumour resembling ovarian sex cord tumour: first report of a large series with follow-up.

AIMS: Uterine tumour resembling ovarian sex cord tumour (UTROSCT) is an uncommon mesenchymal neoplasm of uncertain histogenesis. While it is considered a neoplasm of uncertain but low malignant potential, there is limited evidence for this as there are no large studies with follow-up. We aimed to determine the clinical behaviour of this uncommon neoplasm and investigate clinicopathological parameters which predict behaviour. METHODS AND RESULTS: From a series of 34 cases of UTROSCT, mainly from consultation practice, we obtained follow-up information which was obtained by contacting referring
