Laminoplasty (LMP) is a widely accepted surgical procedure for ossification of the posterior longitudinal ligament (OPLL) of the cervical spine. Progression of OPLL can occur in the long term after LMP. The aim of the present study was to determine whether addition of the instrumented fusion, (posterior decompression with instrumented fusion [PDF]), can suppress progression of OPLL or not. The present study included 50 patients who underwent LMP (n=23) or PDF (n=27) for OPLL of the cervical spine. We performed open door laminoplasty. PDF surgery was performed by double-door laminoplasty follow

Objective Anterior column reconstruction using the lateral transpsoas approach requires sectioning of the anterior longitudinal ligament while protecting the great vessels. Our aim was to study the anatomical plane of separation between the retroperitoneal vessels and the anterior aspect of the lumbar spine as they relate to safety in the lateral transpsoas anterior column reconstruction procedure. Method A total of 100 T2-weighted magnetic resonance imaging (MRI) examinations were studied. Measurements were obtained for each vertebral body and for each intervertebral disk levels from L1-L2 to

BACKGROUND: Normalization of gait is expected 8-12 weeks after anterior cruciate ligament reconstruction and is a criterion for progression to running. Long-term persistence of sagittal knee loading deficits suggests that early goals are not met. Magnitude and progression of deficits in gait during this time and their relationship to deficits in running are not known.
METHODS: 12 individuals status-post reconstruction (5 males) underwent 3-dimensional motion analysis of gait after surgery: one (T1) and three (T2) months and at initiation of running (T3); and running T3. Repeated measures ANOV

This report describes a rare clinical entity, thoracic myelopathy due to ossification of the posterior longitudinal ligament (OPLL), and its management. A 40-year-old woman presented with thoracic myelopathy due to OPLL, extending from T2-T9. We performed a posterior laminectomy and instrumented fusion. However, postoperative paraplegia occurred within 36 h post-surgery. Emergent anterior decompression and interbody fusion was performed via the trans-thoracic approach. Neurological deterioration was reversed following this anterior procedure. Posterior decompression and instrumented fusion for

BACKGROUND: Anterior decompression with fusion (ADF) for patients with cervical ossification of the posterior longitudinal ligament (OPLL) is reportedly associated with a higher incidence of complications than is laminoplasty. However, the frequency of perioperative complications associated with ADF for cervical OPLL has not been fully established. The purpose of this study was to investigate the incidence of perioperative complications, especially neurological complications, following ADF performed to relieve compressive cervical myelopathy due to cervical OPLL.

Ossification of the posterior longitudinal ligament is a common cause of radiculopathy and myelopathy that often requires surgery to achieve decompression of the neural elements. With the evolution of surgical technique and a greater understanding of the biomechanics of cervical deformity, the criteria for selecting one approach over the other has been the subject of increased study and remains controversial. Ventral approaches typically consist of variations of the cervical corpectomy, whereas dorsal approaches include a wide range of techniques including laminoplasty, laminectomy, and lamine

To investigation of the outcomes of indirect posterior decompression with corrective fusion for myelopathy associated with thoracic ossification of the longitudinal ligament, and prognostic factors. Conservative treatment for myelopathy associated with thoracic ossification of the longitudinal ligament (OPLL) is mostly ineffective, and treatment is necessary. However, many authors have reported poor surgical outcomes, and no standard surgical procedure has been established. We have been performing indirect spinal cord decompression by posterior laminectomy and simultaneous corrective fusion of

OBJECTIVES: Fast field echo resembling a CT using restricted echo-spacing (FRACTURE) provides superior cortical and trabecular bone contrast. However, scan parameters may be suboptimal for imaging ossification of the posterior longitudinal ligament (OPLL) because of low contrast between cerebrospinal fluid and ossified ligaments. This study aimed to optimize FRACTURE parameters for improved visualization of cervical OPLL.
MATERIALS AND METHODS: Twenty-four patients with OPLL underwent 3.0-T MRI. Three FRACTURE-based sequences were acquired: (1) FRACTURE (ORG), using fast-field echo with a fli

Traumatic bone marrow lesions (BMLs) occur in ~80% of anterior cruciate ligament (ACL) injuries, typically in the lateral femoral condyle (LFC) and lateral tibial plateau (LTP). Associated with microfractures, vascular proliferation, inflammation, and bone density changes, BMLs may contribute to posttraumatic osteoarthritis. However, their relationship with knee pain is unclear. This study examined the prevalence, characteristics, and progression of BMLs after ACL injury, focusing on associations with pain, meniscal and ligament injuries, and fractures. Participants (N = 100, aged 14-55) with

Spinal coning is a rare complication in spinal anesthesia that results in acute neurological deterioration. It occurs when a pre-existing spinal stenosis alters the CSF flow, creating a high-pressure area beneath the stenosis. The introduction of a needle into this relatively high-pressure area would cause a decrease in CSF pressure, exacerbating the spinal cord compression. This article reports a 50-year-old female who presented with urinary frequency for six months and was diagnosed with urethra spasm. Following spinal anesthesia, a botulinum toxin injection over the urethra was performed by
