Author presents original methodics of oral Mg deficiency testing, suitable for the detection of Mg deficiency, namely the latent states which could not have been easily evidenced till now. The test follows the retention of per os applied preparation of Mg on the basis of its discharge in urine. The realization of the oral Mg tolerance test is neither complicated nor embarrassing for the patient, it is thus applicable even in the out-patient practice.

Magnesium (Mg) is critical for the function of numerous enzyme systems. Mg deficiency thereby may result in many and varied clinical manifestations. Mg deficiency is common as approximately 10% of patients admitted to city hospitals are hypomagnesemic. Mg deficiency is usually due to losses from the gastrointestinal tract or from the kidney. Serum and erythrocyte magnesium levels were screened between attacks in patients with migraine without aura (n = 38) and with aura (n = 6), and for comparison in a group of patients suffering from chronic tension-type headache (n = 25) as well as a group of neurological, non-headache patients (n = 19). Hypomagnesemia is the commonest electrolyte abnormality in the ambulatory diabetic patient and is also a frequent finding in patients with diabetic ketoacidosis. Excessive urinary magnesium loss associated with glycosuria is probably the most important factor in the genesis of hypomagnesemia in the diabetic patient. The clinical consequences of magnesium deficiency include impairment of insulin secretion, insulin resistance and increased macrovascular risk. The role of magnesium deficiency in microvascular complications has yet to be clearly defined.

33 patients with senile, 18 patients with postmenopausal and 9 patients with medicamentous (corticosteroids) osteoporosis were treated with single therapy of Mg lactate (37 patients) and with the combined one of Mg lactate and Na fluoride (23 patients). They were evaluated in three periods of half a year, one year, two years. The better results were achieved with the single therapy than with the combined one and in the senile and postmenopausal osteoporosis than in the medicamentous one. Pain and restricted spine movement were influenced favourably. Kyphosis and x-ray findings were stabilized.

Serum and erythrocyte Mg and Ca were examined in 60 patients with osteoporosis; 33 patients suffered from senile osteoporosis, 18 patients from postmenopausal and 9 from medicamentous osteoporosis. The biochemical signs of chronic magnesium deficiency were present in the first two groups due to lowering erythrocyte magnesium. The tendency to blood calcium disturbance was demonstrated in the third group.

Mg deficiency is a frequent complication of inflammatory bowel disease (IBD) demonstrated in 13-88% of patients. Decreased oral intake, malabsorption and increased intestinal losses are the major causes of Mg deficiency. The complications of Mg deficiency include: cramps, bone pain, delirium, acute crises of tetany, fatigue, depression, cardiac abnormalities, urolithiasis, impaired healing and colonic motility disorders. Two cases of congestive heart failure with coexistent magnesium and potassium depletion are described.
