Heparin-induced thrombocytopenia (HIT) is an under-recognized cause of early microvascular thrombosis and free flap loss after head and neck reconstruction. We report a 45-year-old man who underwent fibula free flap mandibular reconstruction and, within 72 hours of perioperative heparin exposure, developed recurrent venous thrombosis and persistent flap congestion despite multiple re-explorations. The platelet count fell markedly, the 4Ts score indicated a high pretest probability of HIT, and anti-platelet factor 4/heparin testing subsequently returned a positive result. Despite critical care …

UNLABELLED: Mandibular reconstruction is essential for restoring both function and aesthetics after segmental resection due to tumoral pathology. This study aimed to conduct a comparative analysis of three reconstruction strategies for defects resulting from segmental mandibular resection, utilizing finite element analysis (FEA).
METHODS: A digital model of the mandible was created from CBCT data and optimized for FEA. Three reconstruction scenarios were simulated: fixation with a titanium plate, reconstruction with an autogenous fibular graft stabilized with the same titanium plate, and fixa…

BACKGROUND: Oral and maxillofacial tumors, particularly those requiring mandibular reconstruction, present significant clinical challenges due to the complexities involved in achieving precise surgical outcomes and ensuring post-operative stability. Traditional methods for mandibular reconstruction, such as freehand bending of titanium plates, often result in errors in osteotomy and reconstruction, compromising both the precision and stability of the procedure.
METHODS: This study initially developed the Robot-assisted Augmented Reality Osteotomy Navigation System (RARONS) and constructed a v…

INTRODUCTION: The surgical gold standard following mandibular resection is reconstruction with a vascular osteocutaneous free flap. However, not all patients are suitable candidates for this type of procedure due to local or systemic contraindications or the critical anatomy of the remaining healthy bone. This study aimed to evaluate the outcomes of reconstruction with an anterolateral thigh (ALT) free flap in combination with a reconstruction plate as an alternative in such situations.
METHODS: This retrospective case series included 34 oncological patients treated with ALT and reconstructio

OBJECTIVES: Fibula free flaps (FFF) are the standard approach to mandibular reconstruction after partial resection, with the goal of restoring aesthetics and masticatory function. The graft position affects both and must be carefully selected. Correlations between sagittal positioning and upper airway anatomy are known from orthognathic surgery. This study aims to evaluate changes in mandibular position and upper airway anatomy after reconstructive surgery with FFF and corresponding correlations.
MATERIALS AND METHODS: Mandibular position after reconstruction was evaluated using three-dimensi…

The objective of this study was to analyze the outcomes of the free vascularized fibular flap in the reconstruction of mandibular defects, and to assess the oral health impact profile of these patients before surgery and after oral rehabilitation. Patients requiring reconstruction of defects greater than 6 cm were selected for this study. The defect size and type, the size of the required skin paddle, the need for second flaps, the intraoperative complications, and the type of closure were documented. Patients were evaluated postoperatively for function, aesthetics, and donor- or reconstructio

The aim of this paper is to introduce an innovative workflow for staged reconstruction of the mandible, including the temporomandibular joint (TMJ), using a temporary, patient-specific spacer. In cases of partial mandibular resection including disarticulation, sometimes needed to treat inflammatory bone disease, the spacer is intended to retain symmetry of the hard tissues, to preserve the soft tissues, and to act as a bactericidal agent. When complete healing of the affected surrounding tissues has occurred, final reconstruction using a patient-matched total TMJ endoprosthesis, in combination

PURPOSE: Osseointegrated dental implants in fibular free flaps can be used to reconstruct segmental mandibular defects resulting from head and neck resections. The double-barrel fibular flap (DBFF) has been applied as a modified method to overcome insufficient fibular width. This article describes the use of the DBFF with simultaneous dental implant placement as a superior method for esthetic mandibular reconstruction.
MATERIALS AND METHODS: From 2012 to 2015, 26 patients underwent mandibular reconstruction with a free fibular flap and immediate dental implant placement after segmental mandib…

BACKGROUND: Long-segment congenital tracheal stenosis (CTS) is characterized by segmental tracheal stenosis, complete tracheal rings, and absent posterior pars membranosa for >50% of tracheal length. Slide tracheoplasty on cardiopulmonary bypass (CPB) has traditionally been the procedure of choice for airway reconstruction. Pierre Robin sequence (PRS) is characterized by the triad of micrognathia, glossoptosis, and airway obstruction. The authors and others, have demonstrated the efficacy of mandibular distraction osteogenesis (MDO) to avoid tracheostomy in severe cases of PRS.

BACKGROUND: Many reconstructive methods for facial nerve defects have been described previously, such as the greater auricular nerve graft, the sural nerve graft, or hypoglossal-facial nerve anastomosis. Herein, we want to instruct a new technique of repairing facial nerve defects of zygomatic or marginal mandibular branches using upper buccal or cervical branches when we have to face segment defects of facial nerve with wide gaps between facial nerve stumps.
METHODS: The distal part of the upper buccal or cervical branches with peripheral tissue was removed to repair the defects of zygomatic

BACKGROUND: This study examined whether a combination of autologous platelet-rich fibrin glue (PRFG) with mesenchymal stem cells (MSCs) and MEDPOR as guided tissue regeneration (GTR) could act as an osteogenic substitute and whether this treatment yields faster new bone formation than MEDPOR alone or PRFG plus MSC.
MATERIAL: MSCs were harvested and isolated from the bone marrow of dog ilium. Full-thickness bony defects (1.5×1.5 cm) were created in the bilateral mandible angles of the dog. Treatments for bone defect in each group were as follows: group I (n=4), MEDPOR sheet as GTR and autologo

Introducing bone bioengineering concepts in craniofacial surgery demands development of novel imaging strategies, which overcome the shortcomings of radiography such as exposure to ionizing radiation. This study is aimed to investigate the usefulness of ultrasonography (US) in monitoring reconstruction of continuity osteoperiosteal mandibular defects in sheep using rhOP-1. The study was conducted on six adult sheep in which a critical size defect was created at the body of the mandible and was reconstructed using rhOP-1 with type-I collagen as a carrier. Ultrasound images were used to assess o

Reconstruction of segmental mandibular defects with a mandibular reconstruction plate and separate soft tissue flap can be indicated in certain patients. Whilst this technique can provide acceptable results, its exact longevity and timing of associated complications is poorly understood. This study was designed to quantify and plot in time the complication profile associated with this technique of mandibular reconstruction.A search of the Medline and PubMed Central databases was conducted using the terms "Mandibulectomy" and "Reconstruction Plate." Articles reporting total number of plate extr…

Vascularization remains the rate-limiting factor in biomaterial-based reconstruction of massive mandibular defects. Two distinct strategies have emerged: in situ vascularization through a local blood supply and ectopic prefabrication using the body as an in vivo bioreactor before transfer. This first systematic review to directly compare these paradigms, conducted according to a pre-registered protocol (PROSPERO CRD420261368560) and PRISMA 2020. Five databases were searched; risk of bias was assessed with SYRCLE and MINORS, and findings synthesized following the SWiM guideline. Twenty-five stu
