Maxillary distraction osteogenesis is a challenging technique to treat severe maxillary retrusion. Maxillary advancement by distraction has the advantage to provide new bone in combination with simultaneous expansion of the soft-tissue functional matrix. Cleft lip and palate patients can present with severe maxillary retrusion and Class III malocclusion. Two 13-year-old patients, born with non-syndromic cleft lip and palate, underwent maxillary distraction--one had a bilateral, the other a unilateral complete cleft lip and palate. Maxillary advancement was performed using an external distracti

Orthognathic surgery aims to improve both the function and facial appearance of the patient. Translation of the maxillomandibular complex for correction of malocclusion is always followed by changes to the covering soft tissues, especially the nose and lips. The purpose of this study was to evaluate the changes in the nasal region and upper lip due to orthognathic surgery using combined cone beam computed tomography (CBCT) and three-dimensional (3D) stereophotogrammetry datasets. Patients who underwent a Le Fort I osteotomy, with or without a bilateral sagittal split osteotomy, were included i

Malfunction and abnormal posture of the tongue has been thought to be one of etiological factors for skeletal open-bite at length. In this study, an adult female patient with anterior open-bite is reported, who has been treated by means of posterior maxillary osteotomy and has obtained a good occlusion and profile, which is stable throughout the retention period. Electromyography was obtained from the tongue muscles by surface electrodes and the tongue muscle activity during deglutition was measured before and after surgery. The tongue posture was also analyzed on the basis of the roentgen cep…

This study reports the incidence of complications and sinus membrane perforations when using sonic instruments to prepare the lateral window osteotomy for maxillary sinus augmentation. The charts of 33 consecutive patients (40 sinuses) were reviewed. Sinus membrane perforations were reported in 7 cases (17.5% of procedures), with 3 perforations occurring during preparation of the osteotomy window (7.5% of procedures). Postoperatively, 2 cases of sinus infection (5%) and 2 cases of flap dehiscence (5%) were recorded. This study demonstrates that elevation of the sinus floor with a combination o

The object of the study was to assess the effect of Le Fort I osteotomy and maxillary interdental osteotomy on the marginal bone level. Forty patients (25 female, 15 male, mean age 24 years, range 15-46) treated for dentofacial deformities comprised the subjects of the study and underwent Le Fort I osteotomy with or without simultaneous interdental osteotomy. Outcome was measured by marginal bone level measured in radiographs before and 1 year after operation. All patients had good oral hygiene. There was an overall significant mean marginal bone loss of 0.2 mm at surfaces without interdental

INTRODUCTION: The horizontal bony canal in the lateral maxillary wall is the site of anastomosis between the arterial branches from the posterior superior alveolar artery (PSAa) and the infraorbital artery. This anatomic structure is known as the 'alveolar antral artery'.
MATERIALS AND METHODS: We performed a literature review. The anatomic location of the alveolar antral artery in the lateral maxillary sinus wall was researched and its importance in surgical procedures routinely performed on this bony wall discussed.
RESULTS: This artery can be accidentally involved during surgical procedur

Our aim was to evaluate the feasibility of a minimally-invasive surgical technique for anterior maxillary distraction osteogenesis to correct maxillary hypoplasia in patients with clefts. A modified Y distractor was placed intraorally in 106 patients with cleft- associated maxillary deficiency to facilitate protraction of the maxilla. Subsequently the patients had an anterior maxillary osteotomy through a minimally invasive incision, followed by activation of the appliance at the rate of 0.8mm/day until positive overjet was achieved. The patient's lateral cephalograms were evaluated preoperati…

The inability of orthodontists to change the cant of the maxillary occlusal plane without surgical intervention is a limitation of orthodontic treatment. LeFort I osteotomy with asymmetric maxillary impaction is often used to correct this problem. However, canting caused by extruded teeth can be corrected easily with normal orthodontic appliances and skeletal anchorage. The correction occurs through intrusion of the extruded teeth on 1 side of the maxilla, thereby avoiding a more aggressive surgical approach. Patients with considerable maxillomandibular anteroposterior discrepancies and maxillary hypoplasia require corrective treatment through orthognathic surgery. However, in the treatment of severe maxillary retrognathism, it is necessary to reconstruct areas of bone deficiency through grafting techniques in addition to maxillary advancement using only the Le Fort I osteotomy. Treatment in these patients is more challenging and requires high surgical predictability. Alloplastic materials often have been used for the reconstruction of poor bone contours. Condylectomy is an indication of choice for unilateral hypercondylia, whatever the age of the patient, and should be conducted as early as possible and associated with intermaxillary elastic traction. Complementary one- or two-stage operations for mandibular correction are necessary in severe advanced cases. Complementary maxillary osteotomies are very rarely required.
