AEROBIC MICROFLORA IN THE PATHOGENESIS OF MAXILLARY SINUSITIS AFTER THE TREATMENT OF CARIES COMPLICATIONS.

Most sinusitis is the result of a periapical infection caused by caries or periodontal disease. The role of bacteria in the initiation of inflammation of the pulp and periapical zones has been well demonstrated. It is well known that colonization of microorganisms in the oral cavity can contribute to the development of pathology of the pulp and periapical organs. Purpose - to study the pathogenesis of maxillary sinusitis, which developed during the treatment of complications of caries of the upper jaw. The study involved 17 patients (mean 40.5±12.7 years) with maxillary sinusitis after treatme…

Utility of the morphometry of the maxillary sinuses for gender determination by using computed tomography.

BACKGROUND: It is impossible to use the routine skeletal parts for gender identification if the skeleton of unknown human remains is obtained in a fragmented and incomplete state. The alternative is to use other parts of the skeleton for gender identification. OBJECTIVES: The objective of this study was to assess the utility of the morphometry of the maxillary sinuses using computed tomography (CT) for gender determination. MATERIAL AND METHODS: The study, approved by the institutional ethics committee, analyzed CT scans of 200 patients (100 males, 100 females). The measurement of the mediol

Decreased mucosal oxygen tension in the maxillary sinuses in patients with cystic fibrosis.

BACKGROUND: Pseudomonas aeruginosa in the sinuses plays a role in the lungs in cystic fibrosis (CF) patients, but little is known about the sinus environment where the bacteria adapt. Anoxic areas are found in the lower respiratory airways but it is unknown if the same conditions exist in the sinuses. METHODS: The oxygen tension (pO(2)) was measured, using a novel in vivo method, in the maxillary sinus in a group of 20 CF patients. RESULTS: The CF patients had a significant lower pO(2) on the mucosa but not in the sinus lumen as compared with a control group of non-CF patients. Anoxic condit

[Microcirculation in nasal mucosa of healthy subjects and patients with rhinitis and maxillary sinusitis].

Laser Doppler flowmetry was used to determine normal parameters of microcirculation in the nasal cavity and their changes in different forms of rhinitis and maxillary sinusitis. The latter condition was shown to be associated with altered frequency and amplitude characteristics of microcirculation in nasal mucosa at the affected side. These changes can be used to evaluate efficacy of the on-going treatment. Stimulation of sympathetic innervation in patients with vasomotor rhinitis induced local pathological reactions of the microcirculatory bed in the mucous membrane of inferior nasal conchae.…

A study of the septa in the maxillary sinuses and the subantral alveolar processes in 30 patients.

The objective of this paper is to study the incidence of septa in the maxillary sinus and measure the height of the underlying alveolar process using panoramic radiography and computerized tomography (CT scans). Thirty patients who were going to be treated with dental implants were submitted to panoramic radiographs and CT scans. Sixty maxillary sinuses were analysed and divided into 2 groups: group 1 (totally edentulous) and group 2 (partially edentulous). The sinuses were divided into 3 regions (anterior, middle, and posterior), the septa were analysed, and the heights of the alveolar proces

Efficacy and safety of azithromycin versus phenoxymethylpenicillin in the treatment of acute maxillary sinusitis.

In the treatment of acute maxillary sinusitis, azithromycin offers an advantage over phenoxymethylpenicillin in that a complete course of treatment requires drug administration once daily for only three days. In this double-blind, parallel-group, multicenter study, 438 patients with radiographically verified maxillary sinusitis were randomly assigned to receive either 500 mg azithromycin once daily for three days (221 patients) or 1.3 g phenoxymethylpenicillin three times daily for ten days (217 patients). Nasal secretion, maxillary tenderness and pain, nasal obstruction, general malaise, and …

Application of polylactic acid polymer in the treatment of acute maxillary sinusitis in rabbits.

To evaluate the therapeutic effects of topical antibiotic delivered by polylactic acid (PLA) polymer on paranasal sinusitis, we induced maxillary sinusitis in 32 New Zealand white rabbits by obstructing the maxillary sinus ostium and inoculating the sinus cavity with Streptococcus pneumoniae. The rabbits were divided into three groups: a control group (group 1) treated only by reopening the ostium, a group (group 2) treated by both reopening the ostium and injecting ampicillin (40 mg/kg/day), and a group (group 3) in which ampicillin in the PLA carrier (0.326 mg) was placed within the sinus af…

A critical look at the treatment of maxillary sinusitis with long-term draining tubes.

Long-term draining tubes (LTD) have become a common treatment in complicated and prolonged forms of maxillary sinus empyema. Since not all patients show good recovery with this treatment we used sinus-manometry, mathematical calculations, and scanning electron microscopy (SEM) to critically analyze 6 cases with a prolonged history of the disease. Five out of the 6 patients recovered quickly after removal of the LTDs, normally performed sinus punctures, and an appropriate antibacterial treatment. One patient underwent functional endoscopic sinus surgery. Two of the 6 patients had uncommon bacte

[Use of polymer-based drugs in the treatment of acute and chronic maxillary sinusitis].

The 5-year experience gained in the ENT hospital of the Kursk Medical Institute and the ENT department of the Central Hospital of the USSR Ministry of Health proved effective the use of polymer-based drugs in chemotherapy of acute and chronic maxillary sinusitis. The drugs carried by polymers were introduced into the maxillary sinus by puncture or drainage. As a rule, these are antibiotics or the latter in combination with hormones and enzymes. Drugs with addition of polymers can be produced in any pharmacy.

A new approach to the surgical treatment of chronic maxillary sinusitis. (a preliminary report).

To overcome the numerous disadvantages of the Caldwell-Luc operation, a new surgical approach for the treatment of chronic antritis has been devised. A middle meatal antrostomy is made, and this respects the anatomy, physiology and patho-physiology of the sinus, with minimal trauma to the area. The technique is described in detail and its advantages are discussed.

Classifying Maxillary Sinuses of Polish Patients for Sinus Lift: A Pilot Study.

To date, there is no systematic anatomical classification available that could help clinicians in choosing between the lateral and palatal approach in sinus lift procedures. The aim was to provide a simple-to-use and memorable classification of the maxillary sinus concerning the thickness of lateral and palatal walls to facilitate the most adequate choice for the window location during direct sinus floor elevation. Cone beam computed tomography scans were consecutively obtained for 200 maxillary sinuses of patients needing dental implant placement with potential maxillary sinus augmentation. T

Sinus irrigation as an adjunctive therapy for odontogenic maxillary sinusitis - an in-depth analysis.

BACKGROUND: Odontogenic maxillary sinusitis (OMS) is widely acknowledged in both the dentistry and otolaryngology fields. Recently, iatrogenic odontogenic maxillary sinusitis cases can be encountered frequently. The purpose of this study was to evaluate the effect of intraoral sinus irrigation using the small lateral window approach in patients with odontogenic maxillary sinusitis by comparing pre- and postoperative volumetric measurement of CBCT and symptoms. We surveyed 21 patients who visited the Oral and Maxillofacial Surgery Department at PNUDH from 2016 to 2022. All the patients' informa…

A calcified epithelial odontogenic tumor with extension to the maxillary, ethmoid and sphenoid sinuses.

Calcified epithelial odontogenic tumor (CEOT) with extension to the maxillary, ethmoid and sphenoid sinuses is currently unreported. This extension poses a challenge for effective treatment, as CEOT is a benign neoplasm with contreversial potential aggressiveness. We report a case of CEOT that was previously misdiagnosed as an inverted papilloma. A segmental resection of the part of the tumor responsible for the symptoms was performed. Histology revealed polyhedral epithelial cells within an amorphous eosinophilic hyaline material containing round calcified structures and confirmed the diagnos

May Staphylococcus lugdunensis Be an Etiological Factor of Chronic Maxillary Sinuses Infection?

Staphylococcus lugdunensis is an opportunistic pathogen found in the healthy human skin microbiome bacterial community that is able to cause infections of diverse localization, manifestation, and course, including laryngological infections, such as necrotizing sinusitis. Chronic maxillary sinusitis is a disease present in up to one third of European and American populations, and its etiology is not fully described. Within this study, we aimed to characterize 18 S. lugdunensis strains recovered from maxillary sinuses and evaluate them as etiological agents of chronic disease. We performed MLST …

The impact of geometrical parameters on acoustically driven drug delivery to maxillary sinuses.

Acoustically driven nebulized drug delivery (acoustic aerosol delivery) is the most efficient noninvasive technique for drug delivery to maxillary sinuses (MS). This method is based on the oscillation of the air plug inside the ostium to transport drug particles from the nasal cavity (NC) to the MS. The larger the wavelength of the air plug oscillation in the ostium, the greater the penetration of drug particles to the MS. However, using this technique, the maximum drug delivery efficiency achieved to date is 5%, which means 95% of the aerosolized drugs do not enter the MS and are wasted. Sinc

Maxillary sinus pain with radiolucent sinuses due to agenesis of the membranous ostium.

INTRODUCTION: The absence of opacities on CT scan usually eliminates paranasal sinus disease as a cause of facial pain. The authors report a case, which constitutes an exception to this general rule, corresponding to a new aetiology of sinus pain. CASE REPORT: A 16-year-old boy presented with very painful "recurrent acute sinusitis" triggered by pressure changes (altitude, diving, surfing), with no sinus opacity on CT scan. Surgical exploration demonstrated absence of a primary or accessory maxillary ostium. Middle meatus antrostomy relieved the patient's pain. DISCUSSION: The pathophysiolog

ANATOMICAL CORRELATIONS OF THE PALATO-ALVEOLAR COMPLEXES AND MAXILLARY SINUSES IN ADULT PERSONS.

To detect the anatomical correlations between the form of the palato-alveolar complex and the shape and degree of pneumatization of the maxillary sinuses, skull frontal saw cuts obtained from 60 adult persons were studied at the level of the second molars. The form of the palato-alveolar complex was found to change significantly with the loss of teeth. Thus the palato-alveolar complex with well expressed alveolar process of the upper jaw and high palatal vault more often corresponded to the maxillarysinus of a trihedral or uncertain form with the varying degrees of pneumatization. The palato-a
