The Medicaid Drug Rebate Program and the 340B Drug Pricing Program are generally understood as separate efforts to promote drug affordability-one by directly subsidizing low-income patients through insurance coverage (Medicaid) and the other by indirectly subsidizing safety-net clinics and hospitals (340B). Yet they interact in ways that can unintentionally raise costs for Medicaid. This Policy Insight examines how this interaction occurs, introduces two policy strategies to mitigate the interaction, and summarizes the use of these strategies across states as of 2024. We conclude with recommen…

Smaller quasi-experimental studies and short-term controlled trials of medically tailored meals (MTMs) have shown health benefits for several conditions, but MTMs have not been evaluated in large-scale policy initiatives. Multiple US states are now implementing MTMs in Medicaid, the health insurance program for low-income individuals, yet their impact has not been evaluated. We investigated changes in hospitalizations, emergency department (ED) visits and healthcare costs among 1,866 MTM recipients and 1,372 comparators from 2020 to 2023 across 11 health systems in Massachusetts. Both groups m…

Poor health and unstable housing are closely linked. Most research has focused on how housing shapes health, with little empirical study of whether and how health events can lead to future residential mobility or housing instability. This paper uses high-frequency administrative data on residential location and health among Medicaid enrollees in New York City to test whether adverse health events trigger housing mobility or insecurity, independent of the financial toll of medical bills. Using an event study design, I find that health shocks - or, sudden hospitalizations after two hospital-free…

BACKGROUND: Many U.S. states have enacted opioid-limiting laws to curb the opioid epidemic, often capping opioid prescriptions at seven days. How these laws affect opioid prescribing among long-term survivors of childhood cancer remains unclear.
METHODS: Using Medicaid claims linked to the Childhood Cancer Survivor Study, we identified childhood cancer survivors aged 18-64 years continuously enrolled in Medicaid ≥1 year during 2009-2019. Primary outcomes were any opioid prescription fill and opioid prescription with >7 days' supply. Secondary outcomes included total number of opioid prescript…

PURPOSE: To evaluate changes in buprenorphine dispensing for treatment of opioid use disorder (OUD) among Kentucky Medicaid enrollees following the removal of prior authorization (PA) requirements for the transmucosal buprenorphine combination product.
METHODS: We analyzed Kentucky Medicaid claims from 1/1/2016 through 6/30/2023 to identify enrollees with OUD during each quarter (Q1/2017-Q2/2023), using a 12-month lookback window. Segmented regression analysis was used for statistical modeling of an interrupted time series (ITS) design to assess changes in the quarterly proportion of enrollee…

Despite funding the most births in the United States (US), we know little about basic patterns and trends of mental health conditions among Medicaid-insured women during pregnancy and after delivery. We sought to characterize the prevalence of diagnosis and treatment for perinatal mood and anxiety disorders (PMAD) among women with Medicaid health insurance coverage. We conducted serial, cross-sectional analyses of women with live-birth deliveries during 2017-2021 using Transformed Medicaid Statistical Information System (T-MSIS) claims. To construct state-specific PMAD diagnosis and treatment …

BACKGROUND: The potential of methadone maintenance treatment (MMT) to address the opioid crisis has been limited by restrictive federal and local policies that require frequent clinic visits for medication. Unprecedented federal regulatory changes started in early COVID-19 and were made permanent in 2024, allowing clinics increased flexibility in granting take-home medication to patients. Medicaid payment policies can profoundly shape provider behaviors but may not align with regulatory changes.
METHODS: We conducted a systematic synthesis of Medicaid MMT payment and billing policies among 40…

OBJECTIVE: Eating disorders are often underdiagnosed and untreated. Among Medicaid beneficiaries, standard outpatient care is often the only service received for eating disorders. More severe cases, however, may require more intensive services (e.g., intensive outpatient, partial hospitalization, residential, or inpatient care). Medicaid is the largest public health insurance payer, but whether Medicaid beneficiaries have access to these services and what barriers to access they face remain unclear. This study aimed to investigate the availability of intensive eating disorder services across t…

IMPORTANCE: There are large racial and ethnic differences in the use of medications for opioid use disorder (MOUD). Whether differences in geographic availability of MOUD providers (defined in this study as buprenorphine prescribers, methadone dispensing opioid treatment programs, and naltrexone prescribers) contribute to these differences in Medicaid is unknown.
OBJECTIVE: To examine differential geographic availability of MOUD in Medicaid and whether it is associated with MOUD use.
DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study analyzed the geographic availability of Medicai…

Objectives. To examine the association of state Medicaid expansion status with mortality changes during the COVID-19 pandemic in the United States. Methods. Deaths among individuals 20 to 64 years of age in 3142 counties were identified from 2018 to 2022 mortality surveillance data. Age-adjusted mortality rates were calculated for each county and by cause of death. Changes in mortality rates before and after the onset of the COVID-19 pandemic (2018-2019 vs 2021-2022) in Medicaid expansion states relative to nonexpansion states were calculated after adjustment for county sociodemographic factor…

IMPORTANCE: Evidence-based, patient-centered treatment for alcohol use disorder (AUD) can include pharmacotherapy with naltrexone, acamprosate, or disulfiram; however, these medications are rarely used. Medicaid managed care plans (MCPs) manage health services for nearly 80% of Medicaid enrollees and are the largest payer for addiction treatment services. Little is known about Medicaid MCP policies for AUD medications.
OBJECTIVES: To describe Medicaid MCPs' coverage and management of acamprosate, naltrexone, and disulfiram for AUD and examine associations of plan characteristics and state pol…

IMPORTANCE: In 2023, the Massachusetts Medicaid and Children's Health Insurance Program (MassHealth) required accountable care organizations (ACOs) to increase payments to primary care practices and shift to monthly payments, currently calibrated to historical revenues and enhanced practice capabilities, such as being staffed to address behavioral health needs. To prevent rewarding practices for avoiding difficult patients, future payments to primary care practices should reflect their patients' apparent need.
OBJECTIVE: To describe MassHealth's initiative and a complexity-adjusted payment mo…

IMPORTANCE: Pregnant people with opioid use disorder (OUD) are at high risk for potentially avoidable maternal morbidity. The majority of pregnant people with OUD receive health insurance through state Medicaid programs, but there is little comprehensive data on the burden of severe maternal morbidity (SMM)-a composite measure of adverse maternal health outcomes-among this high-risk group.
OBJECTIVE: To estimate rates of SMM among Medicaid-enrolled pregnant people with OUD from 2016 to 2018.

OBJECTIVE: To examine changes in prenatal opioid prescription exposure following new guidelines and policies.
STUDY DESIGN: Cohort study of all (262,284) Wisconsin Medicaid-insured live births 2010-2019. Prenatal exposures were classified as analgesic, short term, and chronic (90+ days), and medications used to treat opioid use disorder (MOUD). We describe overall and stratified temporal trends and used linear probability models with interaction terms to test their significance.
RESULT: We found 42,437 (16.2%) infants with prenatal exposure; most (90.5%) reflected analgesic opioids. From 201…

Despite tobacco cessation medications being a first-line treatment for quitting smoking, a majority of Medicaid programs require health care providers to obtain prior authorization before prescribing them. We examined the impact of Colorado's Medicaid program removing its prior authorization requirement for these drugs on their use and estimated the additional number of Coloradoans who used these therapies in 2023 because of the policy change. The findings indicate that these requirements decrease low-income people's use of these medications. (Am J Public Health. 2025;115(1):42-46. https://doi…

BACKGROUND: The Cystic Fibrosis Foundation Patient Registry (CFFPR) provides valuable clinical and demographic data but includes limited information on health services and medications provided outside of CF Care Centers. Linking CFFPR to claims databases such as national Medicaid data could address these data gaps.
METHODS: Linkage algorithms based on state of residence, gender, and date of birth were utilized to match individuals with CF diagnostic codes in national Medicaid databases (2016) to individuals in the CFFPR (2015-2016). Subsets of individuals with partial social security numbers …

Our objective was to examine occurrence of both conditions in Medicaid; and compare Medicaid service use and cost, and chronic conditions among adults with Down syndrome and autism to those with Down syndrome alone and those with autism alone. We used ICD9 and ICD10 codes in Medicaid claims and encounters from 2011 to 2019 to identify autism and Down syndrome in adults > 18 years. We then calculated costs, claims, hospitalizations, long term care days, and chronic conditions, and compared by group- autism alone, Down syndrome alone, Down syndrome + autism. Between 2011 and 2019, there were 519…

INTRODUCTION: Medicaid family planning waivers can increase access to health care services and have been associated with lower rates of unintended pregnancy, which is associated with a higher risk of negative birth outcomes such as preterm birth and low birthweight. The objective of this study was to test the effect of Georgia's Medicaid family planning waiver, Planning for Healthy Babies (P4HB), on pregnancy characteristics and birth outcomes.
MATERIALS AND METHODS: We used the Pregnancy Risk Assessment Monitoring System (PRAMS) survey data in pre- (2008-2009) and two post-periods (2012-2013…
