OBJECTIVE: To discuss a case of enchondroma initially appearing as a pathologic fracture in a metacarpal bone. The recommended treatment for a pathologic fracture of an enchondroma in the hand is reviewed. Additionally, a reasonable list of differential considerations is presented with accompanying radiographic and advanced imaging characteristics.

The TMC joint is an articulation with special articular surfaces adapted to produce simple (nonrotatory) and complex (rotatory) metacarpal movements. Its articular anatomy and biomechanics are closely related to the pathogenesis of osteoarthritis. The joint works under high transarticular compressive-shearing forces. One hundred nine patients with metacarpal synostosis were examined to determine the best treatment for this condition. The fourth and fifth metacarpals are most frequently affected, and bilateral involvement occurs often. Wrap-around partial great toe transfer, a one time dream, is now a well established and universally accepted method of thumb reconstruction. In this technique, part of the soft tissue of the great toe are wrapped around and shaped to the size of the graft from iliac bone in such a manner that a thumb of normal dimensions and shape is produced. Instead of the iliac bone graft, we found great merit in using the second metacarpal from the traumatised hand to be reconstructed. In this paper its use and merits are elaborated.

We report a prospective study of 245 open digital fractures of the hand in 201 patients. Management followed clear guidelines set out in protocol. The incidence of infection (2.04%) and non-union (2.45%) was low. Proximal phalangeal fractures and comminuted fractures did worst and associated significant soft tissue injuries had a particularly deleterious effect on the outcome. A classification based on the different soft-tissue components involved is proposed. We briefly present software for performing metacarpophalangeal pattern (MCPP) profile analysis, which runs on generally available low-cost IBM (PC, XT, AT) and compatible PCs. The program is easy for the medical geneticist to handle and apply. We compared the mean MCPP of our own patients with Ullrich-Turner syndrome with the mean MCPP for Ullrich-Turner patients originally published by Poznanski.

Forty patients with fractures of the fifth metacarpal neck (boxers' fractures) having a volar angulation of 30 degrees or more were treated by closed reduction and external immobilisation. A combination of malleable aluminium alloy splint and plaster of Paris cast was used. The results show a significant improvement in fracture angulation after reduction and at follow-up (3/5 weeks) using this form of external splintage. There were no complications. Our results compare favourably with those of other recently published series. We advocate the use of this method of treatment for these fractures

Geometric properties of the third metacarpal bone were compared between the young and adult Standardbred and Thoroughbred racehorse. The change in shape during growth and superimposed training was dramatic in both breeds but the Thoroughbred showed the greatest difference in the minimum moment of inertia as the animal matured. Males had larger moments of inertia throughout the length of the diaphysis than did females. The differences in geometric properties of the third metacarpal bone between the Thoroughbred and Standardbred were related to the incidence of fatigue fractures which are common
