OBJECTIVE: To review the audiological and otoscopic status of Chinese children and young adults seen at a cleft lip and palate clinic in Hong Kong and compare with previous studies of Western clinical populations.
DESIGN: The hearing health status of the patients was analyzed on the basis of the results of their otoscopic, pure tone audiometric, and tympanometric examinations. The patients' sex, age, race, and type of cleft pathology were examined for their potential relationship to hearing health status.
PARTICIPANTS: The records of 180 predominantly Chinese patients attending the Cleft Lip

INTRODUCTION: Exposure to prolonged microgravity is associated with cephalad fluid shifts that potentially could affect tympanomastoid ventilation and auditory mechanisms. We conducted a retrospective review of NASA astronaut on-orbit hearing assessments (OOHAs) and middle-ear pressure measurements obtained during International Space Station missions to search for evidence of spaceflight-related hearing changes and disturbances of middle-ear aeration.
METHODS: Nine NASA astronauts conducted on-orbit hearing assessments and tympanometry at least twice while on space station missions. Kuduwave

Background and Objectives: Patency of the aditus ad antrum is essential for middle ear and mastoid ventilation and may influence surgical outcomes in chronic otitis media (COM). Although preoperative computed tomography (CT) is commonly used to assess patency, its reliability remains uncertain. This study compared preoperative CT findings with intraoperative saline and suction testing and evaluated the impact of intraoperative assessment on surgical planning. Materials and Methods: This observational study included 55 patients with mucosal COM who underwent middle ear surgery with preoperative

Introduction The Eustachian tube regulates middle ear functions such as ventilation and pressure normalization. Eustachian tube dysfunction (ETD) is defined as the failure of the Eustachian tube to maintain one or more of its functions. It is a common condition that is associated with other middle ear disorders such as cholesteatoma, tympanic membrane atelectasis, and otitis media with effusion (OME). This study aims to assess ETD prevalence and risk factors in the Qassim region. Methodology This cross-sectional study was conducted in the Qassim region of Saudi Arabia during the period from Se

OBJECTIVE: To estimate the impact of treatment with middle ear ventilation tube insertion (VTI) in children with otitis media (OM) on the risk of cholesteatoma on a national level.
METHODS: Data were obtained from the Danish National Patient Register, the National Health Service Register and Statistics Denmark. Cumulative incidence proportions were estimated by the Kaplan-Meier method and hazard ratios with Cox regression analysis. The first surgically treated middle ear cholesteatoma in a child (STMEC1) was considered an event.
RESULTS: A total of 217,206 children, born after December 31, 1…

OBJECTIVE: Histological studies show that amniotic fluid cellular content (AFCC) causes in the middle ear a foreign body reaction, the extent and severity of which depends upon the massiveness of contamination. We studied how factors related to birth and environment affect proneness to acute otitis media (AOM) in infants born through meconium contaminated amniotic fluid.
METHODS: From the birth records of infants born from 1996 to 2000 a list was made of those born through meconium contaminated amniotic fluid with pulmonary aspiration and tracheal suctioning, followed by treatment in a neonat

BACKGROUND: The hearing results of otosurgery are still unsatisfactory. Even after successful implantation of middle ear prostheses there often remains an air bone gap of 30 dB or more. As possible reasons dislocation of the prostheses due to scar growth, changes in prostheses' attachment or ventilation disorders are being discussed. Decreased stapes mobility, which has been judged only manually up to now, is supposed to be a further reason.
METHOD: We are introducing a new electromagnetic probe. The output signal of this device is proportional to the impedance of the stapes-annular ligament

OBJECTIVES: Recently, determinants of frailty have become an increasingly recognized perioperative risk stratification tool. This study examines the predictive value of a 5-factor modified frailty index (mFI-5) on perioperative morbidity and mortality in patients undergoing otologic surgery, with a subgroup analysis based on surgery site.
STUDY DESIGN: Cross-sectional analysis.
SETTING: National surgical quality improvement program dataset 2005-2019.
PATIENTS: Current procedural terminology (CPT) codes were used to identify patients undergoing all otologic surgeries.
INTERVENTIONS: Otologi

BACKGROUND: Otitis media with effusion (OME) is a leading cause of acquired conductive hearing loss in children. When performing an adenoidectomy for OME, surgeons must decide whether to supplement the procedure with a simple myringotomy or a myringotomy with tympanostomy tube (TT) insertion. While TT offers extended middle ear ventilation, it carries a risk of long-term complications. This systematic review of randomized controlled trials (RCTs) aimed to compare the efficacy and safety of these two approaches.
METHODS: A comprehensive search of PubMed, Scopus, CENTRAL, and Web of Science was…

PURPOSE: Chronic otitis media with effusion (COME) is a common condition in children and a leading cause for hearing loss and ventilation tubes (VT) insertion. Among other risk factors, it is suggested that Helicobacter pylori (H. pylori) infection may have a role in the pathogenesis of COME. Previous studies have reached different results, and therefore, there is a need for further data on the relationship between H. pylori infection and COME development in children. Our objective is to investigate the presence of H. pylori in the middle-ear fluid (MEF) from children with COME.
METHODS: A Co

Recurrent and chronic otitis media (OM) are often refractory to antibiotics due to bacterial persistence in biofilm within the middle ear. In vitro and in vivo studies have demonstrated that antimicrobial proteins and peptides (AMPs) are bactericidal against otopathogens, indicating potential therapeutic value for recalcitrant OM. We measured concentrations of 6 AMPs and 14 cytokines in middle ear effusion (MEE) from 67 children undergoing ventilation tube insertion for recurrent acute OM. Sixty one percent of children had bacterial otopathogens detected in their MEE, 39% by PCR and 22% by PCR

PURPOSE: Learning to use a surgical microscope is a fundamental step in otolaryngology training; however, there is currently no objective method to teach or assess this skill. Tympanostomy tube placement is a common otologic procedure that requires skilled use of a surgical microscope. This study was designed to (1) implement metrics capable of evaluating microscope use and (2) establish construct validity.
STUDY DESIGN: This was a prospective cohort study.
METHODS: Eight otolaryngology trainees and three otolaryngology experts were asked to use a microscope to insert a tympanostomy tube int

OBJECTIVES/HYPOTHESIS: To quantify otolaryngologic surgery utilization in patients with achondroplasia, and to identify any changes in utilization over the past four decades.
STUDY DESIGN: Retrospective cohort study.
METHODS: A retrospective cohort study of 1,374 patients with achondroplasia enrolled in the CLARITY retrospective cohort study at four centers of multi-specialty care for patients with achondroplasia. Otolaryngologic surgeries are presented by birth cohort decade. The main outcomes were number of primary and additional otolaryngologic procedures; age at surgery; likelihood of re

IMPORTANCE: Tympanic membrane retraction (TMR) is a relatively common otological finding. However, no consensus on its management exists. We are looking especially for a treatment strategy in the military population who are unable to attend frequent follow-up visits, and who experience relatively more barotrauma at great heights and depths and easily suffer from otitis externa from less hygienic circumstances.
OBJECTIVE: To assess and summarize the available evidence for the effectiveness of surgical interventions and watchful waiting policy in patients with a tympanic membrane retraction.
