Construction and validation of a machine learning-based prediction model for in-hospital acute kidney injury in patients with lung cancer complicated with sepsis: clinical and nursing applications.

BACKGROUND: Acute kidney injury (AKI) represents a critical complication in patients with lung cancer (LC) complicated by sepsis, exerting a substantial adverse impact on their prognosis. However, to date, no studies have been conducted to predict the risk of AKI in this specific patient population. The primary objective of this study is to develop and rigorously validate seven machine learning (ML) algorithms tailored to predict the probability of in-hospital AKI in patients with LC complicated by sepsis, while simultaneously evaluating their utility in clinical and nursing workflows. METHOD

Development and validation of the N-DROP score: A penalized logistic regression model for predicting 30-day in-hospital mortality in patients with nursing- and healthcare-associated pneumonia.

BACKGROUND: Nursing- and healthcare-associated pneumonia (NHCAP) refers to pneumonia occurring in nursing-home residents or individuals with frequent healthcare exposure. Although the Age, Dehydration, Respiratory Status, Orientation Disturbance, Low Blood Pressure (A-DROP) score is widely used to assess community-acquired pneumonia severity in Japan, it does not account for frailty or functional status, potentially limiting its predictive accuracy for NHCAP. This study aimed to develop an improved mortality prediction model for NHCAP. METHODS: We developed and validated a penalized logistic

Nurse Perceptions of End-of-Life Care Quality Across Long-Term Care, Home, and Social Model Hospice Home Settings: A Qualitative Descriptive Study.

This qualitative descriptive study explored nursing perspectives on end-of-life care quality across 3 distinct community settings: long-term care facilities, patients' homes, and social model hospice homes. Nurses provide most of the direct end-of-life care in these settings, yet their perspectives remain understudied. Their perspective is essential for improving end-of-life care quality and supporting patients and families during this critical time. Semi-structured interviews were conducted with 11 licensed nurses who had provided end-of-life care across all 3 settings. Participants were recr

A nurse-led bedside diagnostic model using cervical tracheal breath sounds to detect silent aspiration in dysphagia patients: A prospective diagnostic accuracy study.

BACKGROUND: Silent aspiration poses significant clinical risks in dysphagia patients, contributing to recurrent respiratory infections and aspiration pneumonia that ultimately result in elevated hospitalization rates and increased mortality. While video fluoroscopic swallowing studies (VFSS) and fiberoptic endoscopic evaluation of swallowing (FEES) remain the diagnostic gold standards, their high costs create substantial barriers to efficient large-scale screening. Conventional bedside swallowing assessments demonstrate limited sensitivity for silent aspiration detection due to inherent subjec

How to bring nurse-led eHealth symptom monitoring in daily care: the Belgian SMILe integrated care model for allogeneic stem cell transplantation adaptation project.

The SMILe (allogeneic SteM cell transplantation faciLitated by eHealth) integrated care model (ICM) offers a transformative approach to routine transplantation care by combining eHealth technology (a patient-facing App for home symptom registration) with monitoring, self-management support and care coordination carried out by an advanced practice nurse (APN) team. Implementing the SMILe-ICM in a setting with an existing eHealth infrastructure and APN support requires context-specific adaptations. Our aim was to adapt the SMILe-ICM to our local setting, guided by a prior context analysis. We fo

Adapting Beyond the Machine: A Roy Adaptation Model-Guided KTH Nursing Pathway and Real-World Outcomes in Maintenance Hemodialysis: A Propensity Score-Matched Cohort Study.

BACKGROUND: Maintenance hemodialysis (HD) carries a heavy psychosocial and self-management burden that routine nursing addresses incompletely. Whether a pathway coupling the Knowledge-Treatment-Health (KTH) structure to the four adaptive modes of the Roy Adaptation Model (KTH-RAM) improves outcomes is unknown. METHODS: We analysed records of 322 adults on maintenance HD at a single centre between October 2021 and October 2025; 161 received routine nursing and 161 the KTH-RAM pathway (non-randomised). Fifteen baseline covariates (demographics, comorbidity, vascular access, laboratory values, d
